Computed tomography in Crohn's disease.

Computed tomography, barium examinations, and endoscopy play complementary roles in documenting the extent of disease in patients with Crohn's disease. Computed tomography is most often requested for patients with suspected or known intraabdominal or pelvic abscesses. In addition to documenting the presence and extent of abscess formation, other pathological processes in patients with Crohn's disease, may be demonstrated on computed tomography scans. These include thickening and/or contrast enhancement of inflamed bowel wall, inflammatory changes in the mesenteric and perirectal fat, and fistu…

Computed tomography in moyamoya disease.

Eighteen patients with moyamoya disease were examined by computed tomography (CT). At the first examination, CT findings were normal in one patient and abnormal in 17 patients. Significant abnormalities on precontrast CT were characterized by low density lesions in 14, ventricular dilatation in 9, and cerebral atrophy in 11 patients. Postcontrast CT showed the presence of contrast enhanced lesions in three patients. Follow-up examinations were performed in 13 patients, and the following changes on CT were found in 10 of them: increase or development of low density lesions in 10, ventricular di…

Computed tomography of ureteral obstruction.

Although hydronephrosis can usually be diagnosed by urography and/or pyelography, the etiology of the obstruction may not be apparent. Computed tomography (CT) is usually helpful in evaluation of these cases. In 36 cases in which CT was performed solely to determine the cause of ureteral obstruction of uncertain etiology, it proved to be of value in 33 instances (91.7%). The disease processes encountered in this series included metastatic carcinoma to the ureter or periureteral tissues(22), lymphoma (one), primary ureteral tumor (two), radiolucent ureteral stone (four), adjacent ileocolitis (t…

Computed tomography in hemophilic pseudotumor.

Computed tomography was used in 4 cases of hemophilic pseudotumor. Three of the patients were operated upon. The method proved to make the differential diagnosis safe and to yield valuable information on the extent of the lesion and the involvement of the skeleton and contiguous organs. The information facilitated the decision whether surgery was indicated and the planning of the operation as well as the follow-up of the conservatively treated case.

Computed tomography of urinary calculi.

Computed tomography (CT) was used in the evaluation of nine patients with nonopaque calculi in the upper urinary tract. In each case the calculus was identified as a very high density object (370-586 Hounsfield units) with calcium oxalate and cystine stones having somewhat higher attenuation values than uric acid or xanthine stones. The differentiation between calculi and other "radiolucent" filling defects was readily made since calculi had much higher attenuation values than blood clot or neoplasm. CT of the upper urinary tract may obviate the need for more invasive procedures such as retrog…

Computed tomography in Wilson disease.

Computed tomography (CT) was performed on five patients with the "cerebral form" and one with the "hepatic form" of Wilson disease. The diagnosis was confirmed biochemically and by the presence of Kayser-Fleischer rings in all cases. In four patients, CT was done at the time of diagnosis; in two of these patients, the scan was repeated at intervals after initiation of treatment. CT was abnormal in all patients with the "cerebral form." All CT abnormalities were of low density and were not changed by contrast infusion. The abnormalities involved the basal ganglia in all five patients; in two pa…

Computed tomography in maxillofacial trauma.

Computed tomography (CT) has become the key diagnostic modality in the evaluation of head trauma. Experience with CT in the operative assessment of maxillofacial injuries is limited, however. Plain films and multidirectional tomography have been used until now to define fractures in the facial region. We examined 27 patients sustaining maxillofacial trauma with CT scans. Ten patients were studied in the coronal plane, 12 in the axial plane, and the remaining 5 in both the axial and coronal planes. Polycycloidal tomography in the coronal and/or sagittal plane was obtained in 18 patients for com…

Optic neuritis and computed tomography.

A forty year old Lebanese man presented to Royal Prince Alfred Hospital with unilateral visual loss and disc oedema. Computed Tomography (CT) of his orbit demonstrated diffuse thickening and tortuosity consistent with an optic nerve tumour. Surgical exploration of the orbit was averted when the patient reported subjective visual improvement. Two months later, CT demonstrated an optic nerve which was well within normal limits.

Computed tomography of gallbladder carcinoma.

Twenty-seven gallbladder carcinomas were examined by computed tomography (CT). All patients had abnormal CT findings, suggesting a gallbladder lesion in 26. Based on the major CT findings, gallbladder cancer was classified as "massive," "thickened wall," or "intraluminal." Twenty cases were diagnosed correctly as cancer: on retrospect, definite findings of gallbladder cancer were observed in 24 and equivocal or misleading signs in 3. Some cases of chronic cholecystitis and liver tumor were difficult to differentiate from gallbladder cancer. Direct hepatic extension was easily seen, but lymph-n…

Computed tomography in brainstem hemorrhage.

Seven of eight patients had a clinical diagnosis other than brainstem hemorrhage before computed tomography (CT). Four were diagnosed as infarction, one as intracerebral hemmorrhage, one as hypertensive encephalopathy, and one as subdural hematoma. All survived except one, who died of pulmonary embolus. Although the yeild of posterior fossa lesions on CT is thought to be small, our experience indicates that CT is exceedingly valuable in demonstrating and localizing brainstem hemorrhage. The prognosis of brainstem hemorrhage appears to be better than reported in the literature.

Computed tomography of disseminated coccidiodomycosis.

Fifteen cases of disseminated cerebral coccidioidomycosis (DCC) were evaluated by computed tomography (CT). Of the 14 patients with abnormal CT scans, all but 2 showed definite evidence of hydrocephalus. Abnormalities of the basal cisterns were present on non-contrast scans in 8 cases (53%). Ten (67%) demonstrated abnormal contrast enhancement of basal cisterns and other cerebrospinal fluid (CSF) spaces. White-matter lesions were detected in 6 cases (40%). Hydrocephalus usually follows abnormal enhancement of basal cisterns. A consistent relationship between active disease (as diagnosed by CSF…

Computerized tomography in intracranial hemorrhage.

Three hundred patients with intracranial hemorrhage were studied by computerized tomography (CT). Thalamic-ganglionic hematoma was found in 232; the mortality increased from 25% to 70% if ventricular extension occurred in this group. Lobar hematoma occurred in 45 of these patients, with a mortality of 20%, which was not influenced by ventricular extension. Seven had intraventricular bleeding only; of these, two died. In 12 patients with CT evidence of cisternal blood, angiography demonstrated aneurysms; the location of the blood predicted the location of the aneurysm in six. Multiple spontaneo…

Neonatal EEG and computerized tomography.

The correlation between EEG and CT scan was studied in 57 full-term newborn infants with various neurological abnormalities, in order to clarify pathological processes underlying EEG abnormalities during the neonatal period. The relation between EEG and CT findings changes with the time elapsing after the acute phase of the perinatal brain insult. The same EEG pattern was associated with different CT findings of perinatal brain injury were usually associated with complete obliteration of cerebrospinal fluid spaces, a phenomenon considered to reflect severe brain swelling. Computed tomography of spinal fractures.

Computed tomography (CT) has aided significantly in the diagnosis and management of spinal fractures. The examination is easy, relatively quick, and avoids potentially harmful manipulation. The diagnostic advantages of CT include precise anatomic delineation and indication of extent of fractures, assessment of spinal stenosis, and demonstration of associated peraspinal lesions.

Computed tomography in Behçet disease.

In Behçet disease, CT may demonstrate low attenuation mass lesions with or without contrast enhancement. Since neurologic involvement may precede the typical dermatologic manifestations, Behçet disease should be included in the differential diagnosis of intracranial mass lesions. A case is presented with central nervous system involvement.

Computed tomography of supratentorial astrocytoma.

In a group of 40 astrocytomas the C.T. appearance was correlated with the grade of malignancy. On the plain scan the majority presented an irregular, nonhomogeneous low density lesion, better demarcatable in high than in low grade astrocytomas. Contrast enhancement was seen in 28 of the 32 high grade astrocytomas, 50% of the annular, 25% of the nodular and 25% of the mixed type. Six of the 8 low grade astrocytomas showed no contrast enhancement and 1 showed an annular type lesion. A relationship was found between the degree of contrast enhancement and the vascularity. Mass effect was observed

Computed tomography in pancreatic disease.

Computed tomography (CT) of the pancreas has been evaluated in 50 patients with established exocrine pancreatic disease and 20 patients without pancreatic disease. Increase in size, irregularity in outline and heterogeneity of composition of the pancreas implied disease but were in no way specific to any particular disease entity. In acute pancreatitis, following complete resolution, the pancreas appeared normal whereas incomplete resolution was associated with non-specific swelling and heterogeneity of density. The extent and location of pseudocysts could be accurately delineated. In chronic …
