The results of combined cataract extraction and trabeculectomy using separate incisions.

A method of combined cataract extraction with posterior chamber intraocular lens and trabeculectomy using separate incisions was tested in 44 operations on 38 patients. The mean preoperative intraocular pressure (IOP) of 28.1 +/- 11.7 (range 12 to 56) mmHg on maximum medication was lowered to 13.9 +/- 3.4 (9 to 23) mmHg at one year, with half the eyes still requiring topical medication. The IOP was 40 mmHg or more preoperatively in eight eyes and 20 mmHg or more in only two patients at one year. There were no rises in IOP above 20 mmHg in the early postoperative period (days 1 and 2). Visual a…

Trabeculectomy with simultaneous topical application of mitomycin-C in refractory glaucoma.

From May 1981 to Feb. 1989 trabeculectomy with per-operative topical application of mitomycin C was performed on eyes with refractory glaucoma. All eyes had undergone prior one, two or more than two antiglaucoma surgeries without success for IOP control. Other risk factors in the present series of eyes included young age, aphakia, rubeosis and glaucoma secondary to trauma or associated with anterior uveitis. 59 eyes underwent trabeculectomy with mitomycin application. 14 eyes were lost. 45 eyes of 42 cases, 33 eyes of 31 males and 12 eyes of 11 females were followed for a period of 1 to 8 year…

Trabeculectomy with resection of ciliary processes in glaucoma following scleral buckling.

We report a case of glaucoma following scleral buckling that required filtering trabeculectomy. Despite anterior entry to the anterior chamber, swollen ciliary processes were incarcerated in the sclerostomy site, blocking aqueous flow. Resection of the occluding ciliary processes resulted in restoration of aqueous flow through the sclerostomy. This case demonstrates that anterior rotation of the ciliary body and processes plays an important role in the pathogenesis of glaucoma following scleral buckling operation.

Trabeculectomy Outcomes With Standard Postoperative Topical Steroids Versus Intraoperative Depot Corticosteroid.

PRCIS: Intraoperative triamcinolone injection plus a reduced topical steroid schedule was compared with the standard topical steroid regimen post-trabeculectomy. Overall success was similar between the groups, although complete success was more likely with the standard, intensive topical steroid schedule. PURPOSE: Compare trabeculectomy outcomes between patients receiving a standard prednisolone acetate postoperative regimen (Drop group) and those receiving intraoperative periocular injection of triamcinolone (16 mg) combined with a reduced steroid regimen (TAC group). PATIENTS AND METHODS: …

Optimizing Filtration Surgery Procedures in African-Derived Populations: Trabeculectomy and Preserflo.

BACKGROUND: African-derived populations are disproportionately affected by primary open-angle glaucoma (POAG) and face increased risks of surgical failure due to anatomical and genetic factors. This report identifies challenges in glaucoma care and strategies to optimize surgical outcomes in patients from African-derived backgrounds. METHODS: Six UK-based consultant ophthalmic glaucoma surgeons with extensive experience in diverse patient populations convened to discuss challenges and key approaches for optimising glaucoma filtration surgery outcomes in African-derived populations. Their opin

Hidden complication: retained cellulose sponge following trabeculectomy with subconjunctival mitomycin C.

PURPOSE: To report a rare complication of trabeculectomy with adjunctive mitomycin C (MMC) application due to the retention of a cellulose sponge fragment. METHODS: A 62-year-old male patient with pseudoexfoliative glaucoma presented with discomfort in the right eye. Examination revealed a retained sponge eroding the conjunctiva, located 4-5 mm posterior to the limbus. The sponge was removed without complications, and topical steroids and antibiotics were prescribed. RESULTS: The conjunctival area healed spontaneously within 10 days, with complete resolution of inflammation by one month. Fol…

Subconjunctival seeding of ciliary body melanoma after trabeculectomy: A surgical pitfall.

A rare case of ciliary body melanoma (CBM) with subconjunctival extension following prior trabeculectomy is presented. The 66-year-old female patient experienced recurrent ocular discomfort and subconjunctival pigmentation in the left eye. Clinical evaluation identified a pigmented subconjunctival mass, secondary glaucoma, and evidence of previous filtration surgery. Imaging and histopathology confirmed CBM with extraocular extension, and the patient subsequently underwent enucleation; the postoperative recovery was uneventful. This case underscores the importance of considering intraocular tu…

Excessive conjunctival and scleral retraction during trabeculectomy: an unusual intraoperative complication.

Aim: Trabeculectomy is the gold standard surgery for achieving target intraocular pressure (IOP) in glaucoma. Besides the efficiency of trabeculectomy, intraoperative or postoperative complications such as, suprachoroidal hemorrhage, vitreous loss, malignant glaucoma, flat anterior chamber, hypotony, choroidal detachment, endophthalmitis, are also quite important. We present the management of excessive conjunctival and scleral retraction during trabeculectomy: an unusual intraoperative complication. Case report: A 66-year-old woman was referred to our glaucoma unit with progression of primary …

[Glaucomatous visual field loss in eyes undergoing first trabeculectomy in Iceland].

INTRODUCTION: Glaucoma is a degenerative disease in the optic nerve with associated visual field defects (VFD). Trabeculectomy is the most common glaucoma surgery. Surgery is indicated if glaucomatous optic neuropathy progresses despite tolerated medical treatment or in patients with severe VFD. The purpose of this paper is to describe the severity of visual field damage in patients undergoing their first trabeculectomy in Iceland. METHODS: A retrospective review of medical records of all patients with open angle glaucoma that underwent first trabeculectomy at Landspítali University Hospital,…

Congenital ectropion uveae (CEU) with refractory glaucoma: Early trabeculectomy saves vision.

We report the case of congenital ectropion uveae in a 10-year-old boy with intractable unilateral glaucoma but no systemic association. Glaucoma in congenital ectropion uveae is often poorly responsive to medial therapy and requires surgical intervention. Satisfactory results are possible if diagnosis is established early and timely surgery performed, as was the case in our patient.
