Fifth Lumbar L5 Perineural Cyst with Unusual Radiculopathy: Traction Plexopathy.

Perineural cysts are cystic dilations and are frequently seen in relation to the lumbosacral spine. We describe a case of a fifth lumbar (L5) perineural cyst with unusual radicular symptoms and discuss the possible role of traction plexopathy caused by the cyst. A 38-year-old male presented with a longstanding history of back pain and right side thigh pain. This pain radiated from the buttocks to the lateral and anterior aspect of the thigh. He described the pain as pins and needles/burning with no significant relief with medications or rest. Imaging of the lumbar spine revealed a cystic lesio…

LONGITUDINAL MICROPERIMETRY EVALUATION AFTER INTRAVITREAL OCRIPLASMIN INJECTION FOR VITREOMACULAR TRACTION.

PURPOSE: To determine whether improvements in microperimetry testing are associated with anatomic resolution after ocriplasmin treatment in patients with symptomatic vitreomacular adhesion (VMA)/vitreomacular traction and relatively preserved baseline best-corrected visual acuity (BCVA). METHODS: Patients with vitreomacular traction received a single 125-μg intravitreal ocriplasmin injection and were followed longitudinally for 6 months with optical coherence tomography, BCVA testing, and microperimetry. Visual function changes were compared between eyes with and without VMA resolution on opt…

Enzymatic vitreolysis with recombinant tissue plasminogen activator for vitreomacular traction.

AIMS: The aim of our research was to gain data about the efficacy of intravitreal injections of a recombinant tissue plasminogen activator (rTPA) in dissolving vitreoretinal tractions (VRTs). MATERIALS AND METHODS: The study group consisted of patients of our Ophthalmology Clinic who had received an injection of rTPA (TPA Group) for an existent vitreomacular traction confirmed by optical coherence tomography and stereoscopic examinations. The control group consisted of patients who had declined treatment despite the existence of a vitreomacular traction confirmed by the same diagnostic method…

Elastic Bandage Traction Technique for Reduction of Distal Tibial Fractures.

In the present technique report, we describe a useful noninvasive traction technique that uses a 6-inch elastic bandage that can be obtained in every operating room and can be easily applied around the patient's ankle and the surgeon's waist to offer a stable traction force during minimally invasive plate fixation of distal tibial fractures. This technique frees the surgeon's hands to focus on applying other forces, such as rotational, varus, or valgus forces, to reduce the fracture and stabilize the reduction and alignment during percutaneous insertion and fixation of the plate. This techniqu…

[Identification of mesenteric traction syndrome using laser speckle contrast imaging].

A mesenteric traction syndrome manifests in some patients undergoing major abdominal surgery and is identified by flushing, accompanied by hypotension and tachycardia. We used laser speckle contrast imaging to quantify blood flow in forehead skin of patients undergoing Whipple's operation. In two patients with similar blood pressure (-50 mmHg) and profound drop in systemic vascular resistance (-40%), forehead skin perfusion increased three-fold in one patient, while it was unchanged in a patient for whom flushing was not evident.

Alopecia in an ophiasis pattern: traction alopecia versus alopecia areata.

We present a case series of 3 black women who presented with alopecia along the anterior and posterior hairline on physical examination. The initial clinical suspicion was traction alopecia from tension placed on the hair and traumatic removal of hairweaves. Two cases were supported histologically as traction alopecia, while the remaining case was alopecia areata in an ophiasis pattern. Interestingly, the case of alopecia areata was associated with the mildly traumatic removal of a weave. Traction alopecia may present in an ophiasis pattern from hair care practices. Although clinical history a…

Optical coherence tomography in spontaneous resolution of vitreomacular traction syndrome.

Vitreomacular traction syndrome (VTS) is a vitreoretinal interface abnormality. The disorder is caused by incomplete posterior vitreous detachment with persistent traction on the macula that produces symptoms and decreased vision. Most symptomatic eyes with VTS undergo a further decrease in visual acuity. Spontaneous complete vitreomacular separation occurs infrequently in eyes with VTS. Surgical intervention may be considered if severe metamorphopsia and decreased visual quality occur. Herein, we report 2 typical cases of idiopathic VTS with spontaneous resolution of vitreo-retinal traction d…

Non-diabetic diffuse macular edema associated with extrafoveal vitreous traction.

BACKGROUND: The causative role of diffuse macular edema in various etiologies is often undetermined. OBJECTIVES: To describe an association between extrafoveal vitreous traction and non-diabetic diffuse macular edema secondary to various ocular entities. METHODS: In a retrospective study of eyes with non-diabetic diffuse macular edema, charts and optical coherence tomography scans demonstrating extrafoveal vitreous traction were analyzed. Excluded were diabetic patients and eyes that had vitreofoveal traction. A control group (n = 12) allowed for mapping of normal macular thickness. RESULTS…

These findings are consistent with the exertion of tractional forces at both pterygium ends. In eye abduction, medial canthus structures (PS, caruncule) may be dragged temporally. Thus, pterygium-related corneal traction may not always significantly increase in eye abduction. PS temporal ectopia may influenc …

Optic disc traction syndrome associated with central retinal vein occlusion.

OBJECTIVE: To describe the association between ischemic central retinal vein occlusion (CRVO) and the development of optic disc (vitreopapillary) traction, as verified by optical coherence tomography (OCT). METHODS: In a prospective, noncomparative, observational patient series, 3 women aged 51 to 64 years developed an ischemic type of CRVO. One patient underwent cannulation of the central retinal vein with injection of a tissue plasminogen activator. In each eye, the contour of the optic nerve head could not be accurately detected because of overlying fibrous tissue. Each patient underwent O…

Old principles revisited--traction splinting for closed proximal phalangeal fractures.

Proximal phalangeal fractures are commonly caused by a blunt injury to the dorsum of the hand. This typically results in a closed transverse fracture with volar angulation, which is an inherently unstable fracture pattern. K-wiring and internal fixation have the disadvantage of damage to the soft tissue envelope, risk of infection and wire loosening. We describe 15 patients treated with an alternative method of skin traction and splintage in our regional trauma centre. Fractures were reduced under X-ray and reduction was maintained using a combination of a malleable metal splint and tape to pr…

Foveal cysts: a premacular hole condition associated with vitreous traction.

OBJECTIVE: To define the clinical findings, cause, and outcome of patients with foveal cysts due to vitreous traction. METHODS: Follow-up of 18 patients with foveal cysts and no posterior vitreous detachment (PVD). Changes were documented in visual acuity, the appearance of the fovea, or the development of a macular hole or PVD. We studied 8 eyes using the retinal thickness analyzer. RESULTS: On follow-up, 9 of 23 eyes did not develop a PVD and still had a foveal cyst; 8 of 23 developed a full-thickness macular hole; 4 of 23 developed a PVD with resolution of the cyst; and 2 eyes underwent v…

Superior oblique damage from the traction suture at cataract surgery.

A 41-year-old woman underwent uneventful phacoemulsification and posterior chamber lens implantation in the right eye. Approximately a month later similar surgery was performed in the left eye. The surgery was performed with the patient under retro-bulbar anesthesia with a combination of bupivacaine (Marcaine) 0.75% and lidocaine (Xylocaine) 4% administered through a lower lid injection. Immediately after the patch was removed, the patient was aware of constant vertical diplopia. Over the ensuing 6 months the symptom worsened. Orbital computed tomography showed no abnormalities.

Dynamic circle traction for severely comminuted intra-articular finger fractures.

Treatment of severely comminuted unstable intra-articular finger fractures has three goals: reconstruction of the articular surfaces, prevention of redislocation of those surfaces, and proper healing of the capsuloligamentous apparatus. Dynamic circle traction (DCT), as described by Schenck (1986), is based on these principles and is the first method of treatment combining traction with intermittent or continuous passive motion. In order to improve on this method we applied continuous, instead of intermittent, passive motion in a newly developed DCT-device (continuous dynamic circle-traction =…
