Intra- and perioperative complications of the LeFort I osteotomy: a prospective evaluation of 1000 patients.

The LeFort I osteotomy has become a routine procedure in elective orthognathic surgery. The authors report the occurrence of intra- or perioperative complications in a series of 1000 consecutive LeFort I osteotomies performed within a 20-year period. In total, 64 (6.4%) patients experienced complications. Anatomical complications affected 26 (2.6%), patients, including 16 (1.6%) with a deviation of the nasal septum and 10 (1.0%) with non-union of the osteotomy gap. Extensive bleeding that required blood transfusion occurred in 11 (1.1%) patients exclusively after bimaxillary corrections; in 1

Correlation between computed tomography, magnetic resonance imaging and cross-sectional anatomy of the head of the guinea pig (Cavia porcellus, Linnaeus 1758).

The current work aimed to study the anatomical features of the guinea pig's head by two medical imaging techniques: computed tomography (CT) and magnetic resonance imaging (MRI), and their correlation with the anatomical cross-sectional images. Six adult healthy guinea pigs were used in the present study. Two heads were imaged by CT scanner and then by MRI. The examined heads were cut sagittally and transversely, and two skulls were macerated. The anatomical features were identified on the anatomical sections and compared with the tomographic and MRI images obtained. Data were presented as thr

Occipital petalia as a predictive imaging sign for transverse sinus dominance.

OBJECTIVES: Occipital petalia is an anatomic description where one of the occipital lobes protrudes towards the contralateral side. Transverse sinus (TS) asymmetry might cause diagnostic challenges with regards to thrombotic or compressive pathologies involving these sinuses. In this study, we investigated the association between occipital petalia and TS hypoplasia on MRI studies. METHODS: In 264 subjects with no neurological complaints, occipital bending was determined on T1-weighted images based on the angulation of interhemispheric fissure between occipital lobes with respect to an imagina

Association of Smoking Habits and Height of Residual Bone on Implant Survival and Success Rate in Lateral Sinus Lift: A Retrospective Study.

The primary aim was to evaluate the association of patient-related factors, biomaterials, and implant characteristics on complications' rate of sinus graft surgery and on implant survival rate in grafted sinus. Secondary aims were to measure bone remodeling around implants and patient satisfaction. A retrospective cohort study was designed. Patients who had computerized tomography (CT) before sinus surgery (T0), orthopantomography after implant surgery (T1) and at follow-up (T2), were included. Specific forms were used to collect clinical data. Radiographic measures were: height of residual bo

How I do it: the stapled ileal J pouch at restorative proctocolectomy.

BACKGROUND: Ileal pouch-anal anastomosis (IPAA) following proctocolectomy is the preferred option for patients with medically refractory ulcerative colitis, indeterminate colitis, and familial adenomatous polyposis. However, it remains a procedure associated with morbidity and mortality. Pelvic sepsis, pouch fistulae, and anastomotic dehiscence predispose to pouch failure. We report our experience with an adaptation for the formation of the stapled ileal J pouch using the GIA™ 100 stapling device (Covidien, Mansfield, Massachusetts, USA). When creating the J pouch, we remove the bevelled plast

Occlusion of the sigmoid sinus after surgery via the presigmoidal-transpetrosal approach.

OBJECT: In this paper the authors report on sigmoid sinus occlusion as a surgical complication in seven of 143 operations in which a presigmoidal-transpetrosal approach was used. METHODS: Five patients (Cases 1-5) developed occlusion within 40 days after surgery, and in the remaining two (Cases 6 and 7) occlusion was detected 5.4 and 6.4 years postsurgery by means of cerebral venography, which was performed in 40 of the remaining 138 patients. Of the two patients with occlusion of the hypoplastic transverse sinus, one (Case 1) did not develop symptoms and the other (Case 2) developed brain ed

Computed tomography reveals the endocranial anatomy of Crocodylia: Implications for phylogenetic relationships and ecomorphological convergence across Crocodylomorpha.

Living crocodylians (alligators, caimans, gavials and 'true' crocodiles) are the remnants of a broader clade, Crocodylomorpha, that displays high ecomorphological diversity, particularly in skull shape. However, there are several instances of ecomorphological convergence between distant lineages, especially pertaining to the independent acquisition of a long, slender, longirostrine snout. In some instances, it is difficult to determine whether extinct species belong to one longirostrine lineage or another, with this problem especially prevalent with regards to the gavialoid crocodylian clade.

Ross procedure with personalized external aortic root support.

The Ross-Personalized External Aortic Root Support procedure is a surgical aortic valve replacement technique in which the autologous pulmonary valve is transposed in the aortic position to replace the malfunctioning aortic valve and a homograft is implanted in the pulmonary position. To prevent autograft dilatation, a Personalized External Aortic Root Support prosthesis is included in the proximal autograft anastomosis and wrapped around the ascending aorta. The aorta is transected transversely, the aortic valve is resected, and the coronary arteries are mobilized and cut out of the sinuses,

Ultrasound of the Fetal Veins Part 3: The Fetal Intracerebral Venous System.

The study of the intracerebral venous system in the fetus can only be achieved by means of high-resolution ultrasound equipment with sensitive color Doppler. In the past two decades, there has been a growing interest in the ultrasound examination of the fetal brain with few studies reporting on the brain vasculature during various stages of gestation. In comparison to other fetal venous systems, reports on the assessment of the fetal cerebral venous system are still scarce. This article presents a review on the fetal intracranial venous system with detailed discussions on the anatomy of the su

Case report: atypical presentation of jugular foramen mass.

INTRODUCTION: Jugular foramen lesions are often associated with pathology of adjacent structures due to either compression or direct invasion. Common presenting symptoms include pulsatile tinnitus, a neck mass, hearing loss, and cranial nerve palsies, leading to changes in taste, vocal cord paralysis, dysphagia, and sternocleidomastoid/trapezius weakness (A. Hakuba, K. Hashi, K. Fujitani, et al., Jugular foramen neurinomas. Surg Neurol 1979; 11:83-94). This patient was found to have a jugular foramen mass after presenting with the unusual constellation of visual changes and headache. CASE PRE

Role of ethnicity in the determination of the transverse sigmoid sinus Junction (TSSJ): A prospective hospital-based radiological study in adult Sabah, East Malaysia.

The Transverse Sigmoid Sinus Junction (TSSJ) is a critical anatomical landmark in neurosurgery, particularly in cerebello-pontine angle (CPA) procedures. Despite its significance, there is limited research on the anatomical variations of the TSSJ, particularly regarding ethnic differences. This prospective radiological study aims to investigate the potential influence of ethnicity on the anatomical differences of the TSSJ in the adult participants from Sabah. This prospective hospital-based cross-sectional radiological study included 180 adult patients who underwent high-resolution CT imaging,

Transverse-sigmoid sinus dural arteriovenous fistula presenting with parkinsonism.

Dural arteriovenous fistula (DAVF) is rarely associated with parkinsonism. A 52-year-old woman presented with a rare case of DAVF manifesting as parkinsonism and subsequently akinetic mutism. She showed dramatic recovery after endovascular treatment. We also review 10 published reports of DAVF presenting with parkinsonism. The clinical features of these cases at presentation was more closely compatible with lower body parkinsonism or vascular parkinsonism rather than Parkinson's disease. Most lesions are located at the transverse-sigmoid sinus (TSS) with venous reflux into the straight sinus w

Fulminant idiopathic intracranial hypertension mimicking Chiari I malformation in a pediatric patient: diagnostic value of MRI and promising outcomes with venous sinus stenting.

Idiopathic intracranial hypertension (IIH) is a rare but potentially serious condition in children, with the risk of permanent vision loss in rapidly progressing or treatment-resistant cases. One diagnostic challenge is that cerebellar tonsillar descent-a feature sometimes seen in IIH-can mimic Chiari I malformation, potentially leading to misdiagnosis and unnecessary, risky surgery. We report the case of a 13-year-old boy with fulminant idiopathic intracranial hypertension (IIH), who presented with rapidly worsening visual symptoms and severe papilledema over the course of a few days. Initial

Introduction of Universal WON-Point for Posterior Cranial Fossa Puncture in Monitoring Infratentorial Intracranial Pressure.

BACKGROUND AND OBJECTIVES: Intracranial pressure (ICP) monitoring is a gold standard in brain injury management. While supratentorial ICP monitoring is widespread, monitoring in the posterior cranial fossa is rare and lacks a standardized location. To define and validate a novel anatomic reference point-the WON point-for minimally invasive and standardized infratentorial ICP (iICP) probe placement. METHODS: The WON point was defined as the outer third toward the ear along a line connecting the center of the external auditory canal on the side of ICP probe placement to the mastoid tip on the c

Intradiploic Epidermoid Cyst Causing Transverse Sinus Compression in an Adolescent With Papilledema: A Rare Mimic of Idiopathic Intracranial Hypertension.

Idiopathic intracranial hypertension (IIH) typically presents in young, obese women with symptoms such as headache, pulsatile tinnitus, and visual disturbances due to raised intracranial pressure. While IIH is commonly diagnosed in the absence of identifiable pathology, it is essential to rule out secondary causes that may mimic its presentation. Because calvarial lesions causing venous outflow obstruction are rare but clinically significant, early imaging is critical to differentiate secondary from idiopathic cases. We report the case of a 16-year-old female with a history of obesity, hyperte

Image-guided lateral suboccipital approach: part 1-individualized landmarks for surgical planning.

OBJECTIVE: Being situated close to the transverse and sigmoid sinus, the asterion has traditionally been viewed as a landmark for surgical approaches to the posterior fossa. Cadaveric studies, however, have shown its variability in relation to underlying anatomic structures. We have used an image-guidance technology to determine the precise anatomic relationship between the asterion and the underlying transverse-sigmoid sinus transition (TST) complex in patients scheduled for posterior fossa surgery. The applicability of three-dimensional (3-D) volumetric image-rendering for presurgical anatom
