The clinical efficacy of scalp acupuncture for tinnitus: A systematic review and meta-analysis.

BACKGROUND: No single treatment is considered to be universally effective for tinnitus. Scalp acupuncture has been explored as a potential treatment. OBJECTIVE: This systematic review and meta-analysis aim to evaluate the clinical efficacy of scalp acupuncture in treating tinnitus. METHODS: A comprehensive search of relevant databases was conducted to identify randomized controlled trials comparing scalp acupuncture with a control treatment for tinnitus. The clinical efficacy rate and reduction in tinnitus severity were assessed using relative risk (RR) and standardized mean difference (SMD),

Proliferation or adaptation? Differences across race and sex in the relationship between time served in prison and mental health symptoms.

Guided by stress proliferation and adaptation perspectives, this study investigates competing hypotheses for the relationship between time served in prison and mental health symptoms. Drawing on data from the Survey of Inmates in State Correctional Facilities in 2004 (N = 12,118), our findings suggest that time served is correlated with mental health symptoms, but that the association differs across race and sex. White males exhibit fewer mental health symptoms at longer exposures to prison, while black males and black females exhibit more symptoms. We conclude that both incarceration dosage (

Comparative effectiveness and safety of intra-arterial chemotherapy and intravenous chemotherapy for retinoblastoma: A systematic review and meta-analysis.

Intra-arterial chemotherapy (IAC) has emerged as a targeted alternative to intravenous chemotherapy (IVC) for retinoblastoma; however, the comparative effectiveness and safety of these approaches remain incompletely defined. We performed a systematic review and meta-analysis to evaluate whether IAC-based regimens are associated with improved clinical outcomes compared with IVC in pediatric retinoblastoma. Twelve studies were included. Comparative studies contributed to the quantitative synthesis, whereas selected non comparative IAC series were summarized qualitatively to provide contextual ev

Heterogeneity in the Association Between Pneumococcal Vaccination and the Risk of Severe Community-Acquired Pneumonia in Elderly Inpatients: A Causal Forest Analysis.

Background: Community-acquired pneumonia (CAP) is a leading cause of morbidity and mortality in the elderly. While pneumococcal vaccination is a core preventive measure, it remains unclear whether its association with severe CAP is uniform across all elderly subgroups. Our study aimed to evaluate the overall association of pneumococcal vaccination with the risk of severe CAP in hospitalized patients aged ≥ 65 years and to explore potential heterogeneity in this association using a causal forest model. Methods: We conducted a retrospective cohort study of patients discharged between January 202

Effects of Blood Pressure Lowering by Hematoma Location in Acute Intracerebral Hemorrhage.

BACKGROUND AND OBJECTIVES: Early intensive blood pressure (BP) lowering is now recognized as an effective approach to improve outcome after acute intracerebral hemorrhage (ICH). However, whether the effect varies by hematoma location is uncertain. We aimed to determine the heterogeneity of the treatment effect of intensive BP lowering on functional outcome by hematoma location. METHODS: We conducted a pooled analysis of individual patient-level data (IPD) from all 4 Intensive Blood Pressure Reduction in Acute Cerebral Hemorrhage Trials (INTERACT) and the second Antihypertensive Treatment of A

BACKGROUND: The susceptibility vessel sign (SVS) on baseline MRI in acute ischemic stroke patients has been associated with better outcomes … received IVT + EVT. SVS was present in 92% of the participants (n = 181). There was no evidence for treatment effect heterogeneity regarding the post-interventional radiological and clinical effects of IVT + EVT versus EVT alone with strata of SVS. In SVS+ participants, IVT favored pre-interventional reperfusion (aOR 7.95, 95% CI 1.42-44.46), whereas in SVS-patients, it did not …

Association of race and survival in patients treated with apalutamide: Pooled analysis of two phase 3 trials.

BACKGROUND: Clinical studies have shown that outcomes of patients with prostate cancer could vary depending on race. In this study, the authors sought to determine if the treatment effect of apalutamide, an androgen receptor pathway inhibitor (ARPI), on overall survival (OS) varies depending on the race of the patient. METHODS: This pooled analysis includes individual patient data from two phase 3 trials, TITAN and SPARTAN, which randomized patients to androgen deprivation therapy (ADT) ± apalutamide in metastatic hormone-sensitive and nonmetastatic castration-resistant prostate cancer, respe

Efficacy of evinacumab by genotype and low-density lipoprotein receptor function in patients with homozygous familial hypercholesterolaemia: A subanalysis from the ELIPSE open-label extension study.

BACKGROUND AND AIMS: Homozygous familial hypercholesterolaemia (HoFH) is a rare genetic disorder caused primarily by variants in both alleles of the gene encoding the low-density lipoprotein receptor (LDLR). This subanalysis of the ELIPSE open-label extension (OLE) study assessed the efficacy of evinacumab, an angiopoietin-like 3 inhibitor, by genotype and LDLR function in patients with HoFH. METHODS: Patients aged ≥12 years with HoFH on stable lipid-lowering therapies received evinacumab 15 mg/kg intravenously every 4 weeks. Patients were grouped according to genotype: bi-allelic monogenic i

Age and Sex as Moderators of Sleep Architecture in Schizophrenia, Bipolar Disorder, and Unipolar Depression: A Case-Control Polysomnographic Systematic Review and Meta-Regression.

Sleep architecture alterations have been shown in major psychiatric disorders, but the contribution of demographic factors to between-study heterogeneity remains insufficiently characterized. This study reports a transdiagnostic systematic review and meta-regression of case-control polysomnographic studies comparing schizophrenia, bipolar disorder, and unipolar depression with healthy controls. The factors age and sex as possible moderators across harmonized case-control polysomnographic studies have been analyzed. Eligible studies reported diagnostic criteria, treatment status, age, and sex.

Can overall results of clinical trials be applied to all patients?

It is generally assumed that the overall results of a clinical trial are generalisable to all patients in the trial and all similar future patients; in other words, that the relative treatment effect in individual patients is similar to the overall trial result. Although this assumption underpins the application of trial results to clinical practice, it has rarely been tested. By independently derived prognostic models, the results of the European Carotid Surgery Trial and the UK-TIA Aspirin Trial were reanalysed to find out whether relative treatment effect varied with absolute baseline risk

Short communication: Meta-analysis on therapy of bovine endometritis with prostaglandin F2α-An update.

The objective of the study was to update a prior meta-analysis on the efficacy of the treatment of bovine endometritis with PGF2α. Clinical endometritis in cattle is defined as the presence of a purulent or mucopurulent vaginal discharge after 20 or 26 d, respectively, postpartum. Although the prevalence and impact of this disease are well known, the questions of whether and how to treat endometritis are still controversially discussed. Hence, to ensure timeliness of scientific results, we updated an existing meta-analysis published in 2013 that showed no treatment effect of PGF2α in cases of

Safety and efficacy of pilocarpine, cevimeline, and diquafosol compared to artificial tears for the treatment of dry eye: protocol for a systematic review.

BACKGROUND: Dry eye disease (DED) is a condition that compromises the ocular surface and affects millions of people around the world. In recent years, a scheme has been proposed for the treatment of DED, with the use of artificial tear being the mainstay of treatment. In this scheme, the use of secretagogues is suggested as part of the treatment for patients with moderate to severe affectation. With this systematic review, we aim to evaluate the effectiveness and safety of secretagogues for DED. METHODS: Electronic databases will be searched; we will include randomized controlled trials that

Olanzapine effectiveness on weight gain in anorexia nervosa patients: systematic review and meta-analysis.

BACKGROUND: Anorexia nervosa (AN) is a complex psychiatric disorder marked by extreme caloric restriction, intense fear of weight gain, and distorted body image perception. Not only influence physical health, but it also creates serious psychological burdens. Treatment of AN is especially daunting due to its multi-dimensional nature. Recent research indicates that a pharmacological approach, such as the prescription of atypical antipsychotics like Olanzapine, may help treat both the nutritional deficiency and psychological distress related to the disorder. By allowing an individual gain weight

Heterogeneous Effects of Sodium-Glucose Cotransporter-2 Inhibitors on Acute Kidney Injury: A Causal Learning Approach.

BACKGROUND: Sodium-glucose cotransporter-2 inhibitors (SGLT2is) have been associated with lower risk of acute kidney injury (AKI), but existing studies rarely explore heterogeneous treatment effects or underlying causal pathways. We applied a comprehensive causal-learning framework to evaluate both overall and subgroup-specific effects of SGLT2i therapy on AKI. METHODS: Using a new-user, active-comparator target trial emulation in the OneFlorida+ data (2014-2023), we estimated individualized and average treatment effects with a doubly robust meta-learner, assessed heterogeneity via subgroup a
