A case of coronoid process hyperplasia is presented. The primary presenting complaint and clinical feature was restriction of opening, although there was marked facial asymmetry in the area of the right zygoma. Presurgical documentation of this anatomic variation by routine radiography and computed tomography with sagittal reconstruction is presented.

Ionizing radiation used in treating the head and neck area produces oral side effects such as mucositis, salivary changes, trismus and radiation caries. Sequelae of cancer chemotherapy often include oral stomatitis, myelosuppression and immunosuppression. Infections of dental origin in compromised patients are potentially lethal. Specific programs to eliminate dental pathology before radiation and chemotherapy, and to maintain oral hygiene during and after therapy, will minimize these complications.

While coronoidotomy and coronoidectomy remain established procedures in maxillofacial surgery, exceptionally enlarged coronoids pose a special challenge, as their intraoral retrieval may be difficult or impossible. Classical coronoidotomy alone in such situations may risk reattachment and recurrence of trismus. The technique described allows creation of sufficient gap and interpositioning at the coronoid base, allowing achievement of satisfactory mouth opening while minimising the risk for reattachment and relapse.

A 34-year-old woman from the Philippines showed difficulty in opening the mouth. She had no wounds in the last one mouth, but was treated with an obstetric procedure after stillbirth 16 days before in the Philippines. She showed trismus, rigidity and muscle pain, but no muscle spasms, dyspnea and autonomic dysfunction. In this case, it is supposed that the obstetric procedure is the origin of the injury. In Japan, this is the first report of maternal tetanus.

Sialolithiasis is an uncommon disorder in childhood. Initially asymptomatic, symptoms may appear gradually. These can vary from moderate discomfort to severe pain with large glandular swelling accompanied by trismus. The correct interpretation of symptoms and a proper investigation for localization of salivary stones are important for effective treatment. A case of submandibular sialolithiasis in a 9-year-old girl is used to exemplify the problems had in clinical diagnosis.

Garré's chronic sclerosing osteomyelitis is a rare, nonsuppurative form of chronic osteomyelitis, typically seen in children and adolescents, and characterized by periosteal bone proliferation. This case report presents an unusual manifestation in a 20-year-old male with recurrent swelling of the left cheek and trismus, in whom imaging and clinical evolution supported the diagnosis. The management approach and follow-up imaging are discussed, and the case is contextualized within the existing literature.

A 70-year-old woman was admitted to our hospital due to difficulty in moving her limbs. She had trismus and a necrotic and contaminated wound in her left lower leg. A diagnosis of tetanus was confirmed and intensive care was started. On the second day, her blood pressure fell and a ST segment elevation on electrocardiography (ECG) was detected. She was diagnosed with takotsubo cardiomyopathy by echocardiogram and improved undergoing conservative therapy.

Trigeminal schwannomas are commonly located in the intracranium, and extracranial schwannomas limited to the infratemopral fossa (ITF) are extremely rare. Trigeminal mandibular schwannoma was found in the left ITF of a 48-year-old man who presented with a 1-year history of left facial pain and trismus. The schwannoma was completely removed by a transmandibular transpterygoid approach with no major complications. This case is discussed with a review of the literature on this rare occurrence, focusing on the surgical approach.

The clinical features of a modified form of tetanus, termed 'subacute tetanus', occurring in non-immune patients are presented as manifested in five patients. Subacute tetanus has a good prognosis and favourable outcome. Trismus and abdominal rigidity may be minimal or absent. Nocturnal, brief generalized muscle spasms, palpably contracted sternomastoid muscles and spastic limbs are common features of the disease. The pathophysiology of the nocturnal muscle spasms requires elucidation.
