Human African trypanosomiasis: connecting parasite and host genetics.

In West and Central Africa, the protozoan parasite Trypanosoma brucei (T. b.) gambiense causes a chronic form of Human African trypanosomiasis (HAT) that might last several years, whereas T. b. rhodesiense refers to an acute form in East Africa that lasts weeks to months. Without treatment, both forms can cause death. Diagnosis relies on detecting parasites in blood, lymph or cerebrospinal fluid. HAT was no longer considered a public health problem in the 1960s, but it returned to alarming levels in the 1990s. After intensifying case detection and treatment, WHO recently declared the situation…

Trypanosomiasis control, Democratic Republic of Congo, 1993-2003.

In the Democratic Republic of Congo (DRC), human African trypanosomiasis (HAT) reached unprecedented levels in the 1990s. To assess recent trends and evaluate control efforts, we analyzed epidemiologic and financial data collected by all agencies involved in HAT control in DRC from 1993 to 2003. Funds allocated to control populations, as well as to the population screened, doubled from 1993 to 1997 and from 1998 to 2003. The number of cases detected decreased from 26,000 new cases per year in 1998 to 11,000 in 2003. Our analysis shows that HAT control in DRC is almost completely dependent on i…

Prenatal diagnosis of congenital Chagas' disease (American trypanosomiasis).

The prenatal diagnosis of congenital transmission of Chagas' disease in a pregnant woman with the indeterminate form of the disease is reported. Sonography revealed fetal hydrops at 31 weeks' gestation. Anti-Trypanosoma cruzi IgM and IgG antibodies were negative in the fetal blood sampled by cordocentesis, but T. cruzi trypomastigotes were found in its buffy coat. Owing to anemia, in utero exchange transfusion was undertaken, but fetal demise ensued. Labor was induced and a stillborn infant weighing 2030 g was delivered. The pathological examination revealed placentitis and meningoencephalitis

African trypanosomiasis: haematogenic brain parasitism early in experimental infection through bypassing the blood-brain barrier, with considerations on brain trypanosomiasis in man.

A hematogenic invasion of the brain in suckling NMRI mice infected with Trypanosoma brucei rhodesiense was initiated by means of a mechanical damage of the blood-brain barrier. The brain was punctured after development of a blood infection. Brain infection was found in 31 out of 32 animals examined. Trypanosomes are initially capable of rapid multiplication. The number of parasites was highest during the 1st week. From the middle of the 2nd week the number of parasites decreased continuously, alongside increasing atrophy. In the 3rd and 4th week only rare degenerating or ghost trypanosomes wer

Understanding Sex-biases in Kinetoplastid Infections: Leishmaniasis and Trypanosomiasis.

BACKGROUND: Leishmaniasis, Chagas disease (CD), and Human African Trypanosomiasis (HAT) are neglected tropical diseases in humans caused by intracellular parasites from the class Kinetoplastida. Leishmaniasis is one infectious disease that exhibits sex-bias not explained solely by behavioral or cultural differences. However, HAT and CD have less well documented and understood sex-related differences, either due to a lack of differences or insufficient research and reporting. METHODS: This paper reviews the rate of disease and disease severity among male and females infected with CD, HAT, and …

Veterinary Chagas Disease (American Trypanosomiasis) in the United States.

Veterinary Chagas disease is a persistent threat to humans, dogs, and other wild or domestic mammals that live where infected triatomine "kissing bug" insect vectors occur across the Americas, including 28 states in the Southern United States. Animals infected with the Trypanosoma cruzi parasite may be asymptomatic or may develop myocarditis, heart failure, and sudden death. It is difficult to prevent animal contact with vectors because they are endemic in sylvatic environments and often disperse to domestic habitats. Challenges for disease management include imperfect diagnostic tests and lim

Gambiense human African trypanosomiasis: the bumpy road to elimination.

PURPOSE OF REVIEW: Gambiense human African trypanosomiasis (gHAT), a disease that has killed hundreds of thousands as recently as the 1990s, could be on the verge of elimination or even eradication. This review describes recent developments that give us reasons for optimism as well as some caveats. RECENT FINDINGS: New developments in diagnostic and vector control tools, and especially in treatment, make it possible to strive for elimination of transmission of gHAT by 2030, perhaps even eradication. SUMMARY: Gambiense human African trypanosomiasis is a deadly infectious disease affecting Wes…

Human African Trypanosomiasis - A rare case report from India.

A young female patient from Ahmedabad city presented with acute febrile illness and bicytopenia (leukopenia and thrombocytopenia). She returned to India after recent visit to East Africa. Human African Trypanosomiasis (Sleeping sickness) was diagnosed by identification of Trypanosoma brucei rhodesiense in peripheral blood smear. She treated successfully with suramin. In India, we account this as second case of HAT after first report before 18 years in the published literature.

Accelerating towards human African trypanosomiasis elimination: Issues and opportunities.

Human African trypanosomiasis (HAT) has been an alarming global public health issue. The disease affects mainly poor and marginalized people in low-resource settings and is caused by two subspecies of haemoflagellate parasite, Trypanosoma brucei and transmitted by tsetse flies. Progress made in HAT control during the past decade has prompted increasing global dialogue on its elimination and eradication. The disease is targeted by the World Health Organization (WHO) for elimination as a public health problem by 2020 and to terminate its transmission globally by 2030, along-side other Neglected …

Mathematical Modelling of Human African Trypanosomiasis Using Control Measures.

Human African trypanosomiasis (HAT), commonly known as sleeping sickness, is a neglected tropical vector-borne disease caused by trypanosome protozoa. It is transmitted by bites of infected tsetse fly. In this paper, we first present the vector-host model which describes the general transmission dynamics of HAT. In the tsetse fly population, the HAT is modelled by three compartments, while in the human population, the HAT is modelled by four compartments. The next-generation matrix approach is used to derive the basic reproduction number, R 0, and it is also proved that if R 0 ≤ 1, the disease…

Treatment options for second-stage gambiense human African trypanosomiasis.

Treatment of second-stage gambiense human African trypanosomiasis relied on toxic arsenic-based derivatives for over 50 years. The availability and subsequent use of eflornithine, initially in monotherapy and more recently in combination with nifurtimox (NECT), has drastically improved the prognosis of treated patients. However, NECT logistic and nursing requirements remain obstacles to its deployment and use in peripheral health structures in rural sub-Saharan Africa. Two oral compounds, fexinidazole and SCYX-7158, are currently in clinical development. The main scope of this article is to di…
