Oral delivery of Mycobacterium bovis bacillus Calmette-Guérin (BCG) in alginate spheres to captive white-tailed deer.

BACKGROUND: Bovine tuberculosis (bTB), caused by infection with Mycobacterium bovis, continues to be an animal and zoonotic concern in many parts of the world, including the United States. Long-standing eradication programs have been successful at lowering prevalence of disease in many countries; however, disease eradication has not been achieved. One major obstacle to eradication is the presence of various wildlife reservoirs for M. bovis, such as white-tailed deer (Odocoileus virginianus), which serve as a source of spill-back to cattle herds. A potential method to reduce intra- and inter-sp

Nutritional supplements for people being treated for active tuberculosis.

BACKGROUND: Tuberculosis and malnutrition are linked in a complex relationship. The infection may cause undernutrition through increased metabolic demands and decreased intake, and nutritional deficiencies may worsen the disease, or delay recovery by depressing important immune functions. At present, there are no evidence-based nutritional guidance for adults and children being treated for tuberculosis. OBJECTIVES: To assess the effects of oral nutritional supplements (food, protein/energy supplements or micronutrients) on tuberculosis treatment outcomes and recovery in people on antitubercul

Clinical application of whole-genome sequencing in the management of extensively drug-resistant tuberculosis: a case report.

BACKGROUND: Whole-genome sequencing (WGS)-based prediction of drug resistance in Mycobacterium tuberculosis has the potential to guide clinical decisions in the design of optimal treatment regimens. METHODS: We utilized WGS to investigate drug resistance mutations in a 32-year-old Tanzanian male admitted to Kibong'oto Infectious Diseases Hospital with a history of interrupted multidrug-resistant tuberculosis treatment for more than three years. Before admission, he received various all-oral bedaquiline-based multidrug-resistant tuberculosis treatment regimens with unfavourable outcomes. RESU

A new attenuated Mycobacterium bovis vaccine protects brushtail possums (Trichosurus vulpecula) against experimental tuberculosis infection.

Vaccination of wildlife against bovine tuberculosis is being actively considered in countries that have wildlife reservoirs of Mycobacterium bovis infection. A newly attenuated strain of M. bovis (WAg533) was produced as part of a programme to develop a better vaccine than BCG to control tuberculosis in brushtail possums in New Zealand. The vaccine efficacy of WAg533 in possums was compared to BCG using three different methods of inoculation (conjunctival/intranasal, oral and sub-cutaneous) followed by aerosol challenge. Overall, WAg533 was a more potent vaccine than BCG and by two methods of

Palatal Tuberculosis: A Rare Manifestation of Extrapulmonary Tuberculosis.

Tuberculosis (TB) remains a relevant public health concern in Portugal, despite the progress achieved in its control and management over recent years. Extrapulmonary manifestations represent a notable proportion of cases, while oral manifestations are uncommon, particularly those affecting the palate, which are rarely reported. The authors describe a case of a 63-year-old Afghan woman with multiple comorbidities, including poorly controlled diabetes mellitus with end-organ damage and heart failure, who presented with a painless ulcerated lesion on the hard palate with erythematous borders and

Respirable Clofazimine Particles Produced by Air Jet Milling Technique Are Efficacious in Treatment of BALB/c Mice with Chronic Mycobacterium tuberculosis Infection.

Tuberculosis (TB) remains a major cause of morbidity and mortality, particularly in low- and middle-income countries where access to health care workers, cold-chain storage, and sterile water sources may be limited. Inhaled drug delivery is a promising alternative to systemic delivery of antimycobacterial drugs, as it enables rapid achievement of high infection-site drug concentrations. The off-patent drug clofazimine (CFZ) may be particularly suitable for this route, given its known systemic toxicities. In this study, micronized CFZ particles produced by air jet milling were assessed for shel

Orofacial tuberculosis: A diagnostic challenge.

Tuberculosis is typically a pulmonary chronic infectious disease with a high prevalence in developing countries which carries a substantial rate of mortality. Extrapulmonary disease may occur, mainly second to the endogenous spread of the pathogen from the primary site. Oral or mandibular involvement represents a minority among the reported cases. A 12-year-old female patient with a diffuse left-sided facial swelling and dull pain that gradually developed over 8 months, presented to us with misdiagnosis and poor management. Examination was found to have a firm swelling involving the buccal reg

[Pulmonary intravascular talcosis: A case report].

INTRODUCTION: Pulmonary intravascular talcosis is a rare condition occurring in intravenous drug users injecting oral medications. Talc results in a foreign-body granulomatous reaction giving a radiological haematogenic miliary appearance mimicking miliary tuberculosis. Drug users represent a population at risk for both these conditions and their distinction may be challenging. CASE REPORT: We reported the case of a man, 33 year-old, intravenous drug addict, detected by the health services because he was the partner of a person who died of contagious and multi-resistant tuberculosis. Tuberculosis (TB) in pregnancy - A review.

Tuberculosis (TB) is a common infectious pathology especially in low-income countries, which may complicate pregnancy. Although pulmonary TB is more common in pregnancy than extra pulmonary TB (EPTB), EPTB is becoming more common especially in those living with human deficiency virus (HIV) co infection or have other comorbidities. The diagnosis of TB may be delayed in pregnancy due to the masking of its symptoms by those of pregnancy. If diagnosed and treated on time both pulmonary TB and EPTB are associated with excellent maternal and perinatal outcome. If, however, there is delay in diagnosi

Metagenomic and paleopathological analyses of a historic documented collection explore ancient dental calculus as a diagnostic tool.

Dental calculus is a microbial biofilm that contains biomolecules from oral commensals and pathogens, including those potentially related to cause of death (CoD). To assess the utility of calculus as a diagnostically informative substrate, in conjunction with paleopathological analysis, calculus samples from 39 individuals in the Smithsonian Institution's Robert J. Terry Collection with CoDs of either syphilis or tuberculosis were assessed via shotgun metagenomic sequencing for the presence of Treponema pallidum subsp. pallidum and Mycobacterium tuberculosis complex (MTBC) DNA. Paleopathologic

Ground-glass opacity as a paradoxical reaction in miliary tuberculosis: A case report and review of the literature.

A paradoxical reaction (PR) is an excessive immune response occurring during antitubercular therapy (ATT), but is rare in patients with miliary tuberculosis. A 78-year-old woman complained of general malaise, loss of appetite, and fever for 10 days. Chest computed tomography (CT) showed diffuse, bilateral, discrete miliary nodules. The patient was treated with ATT for miliary tuberculosis. Nine days after starting the treatment, she developed a spiking fever and worsening malaise. Repeat CT showed new localized ground-glass opacity (GGO) in the right upper lobe. After excluding possible etiolo

The Mycobacterium tuberculosis MmpL3 inhibitor MSU-43085 is active in a mouse model of infection.

MmpL3 is a protein that is required for the survival of bacteria that cause tuberculosis (TB) and nontuberculous mycobacterial (NTM) infections. This report describes the discovery and characterization of a new small molecule, MSU-43085, that targets MmpL3 and is a potent inhibitor of Mycobacterium tuberculosis (Mtb) and M. abscessus survival. MSU-43085 is shown to be orally bioavailable and efficacious in an acute model of Mtb infection. However, the analog is inactive against Mtb in chronically infected mice. Pharmacokinetic and metabolite identification studies identified in vivo metabolism

[Renal tuberculosis treated with rifampicin, isoniazid and ofloxacin].

We describe the results of eight patients with renal tuberculosis treated with rifampin, isoniazid and ofloxacin. Ofloxacin was given orally, 200 mg b.i.d. for 6 months. During the first three months, rifampin (600 mg/daily) and isoniazid (330 mg/daily) were added. All M. tuberculosis strains isolated were sensitive to ofloxacin (MIC 1 mg/l). Follow-up cultures turned to be negative rapidly (during the first month of therapy), and no untoward effects were recorded. All patients had a 12-months follow-up period, and all were clinically cured. The treatment used was well accepted by all patients

[Submental swelling in an Ethiopian man].

A 27-year-old man from Ethiopia had undergone an extraction of a molar in Libya ten months earlier, after which a submental swelling developed. For that reason, an oral and maxillofacial surgery department was consulted. Clinical examination showed a tender, firm-to-the-touch, non-mobile swelling with central ulceration. There was no pus drainage or evident lymphadenopathy. Intraoral examination showed a sanitized dentition with a healed diastema in the area of the extracted tooth. The patient suffered from fatigue, weight loss, night sweats, fever, headache and a cough. A recent chest X-ray,

Complexities in the treatment of coinfection with HIV, hepatitis B, hepatitis C, and tuberculosis.

HIV, hepatitis B virus (HBV), and hepatitis C virus (HCV) are commonly encountered blood-borne infectious microorganisms. Infection with these viruses typically requires long-lasting drug therapy. Coinfections, especially with tuberculosis, pose a challenge to the creation of a regimen with adequate efficacy and minimal drug-drug interactions and adverse effects. We present the case of a young man with a history of intravenous drug misuse who was diagnosed with disseminated tuberculosis and with a triple infection with HBV, HCV, and HIV. The treatment for tuberculosis was initiated first, foll
