Mycobacterium-mediated chemokine expression in pleural mesothelial cells: role of C-C chemokines in tuberculous pleurisy.

Pulmonary tuberculosis is characterized by granulomatous inflammation with an extensive infiltration of mononuclear phagocytes, but the mechanisms of phagocyte recruitment to the pleural space is unknown. In this study, pleural fluid from patients with tuberculosis contained significantly (P<.001) more biologically active MIP-1alpha and MCP-1 (C-C cytokines) than did effusions from patients with congestive heart failure. Antigenic MIP-1alpha and MCP-1 was detected by immunocytochemistry in pleural biopsy sections of patients with tuberculous pleurisy. In vitro, pleural mesothelial cells stimul

Medical thoracoscopy for exudative pleural effusion: an eight-year experience from a country with a young population.

BACKGROUND: With the exception of areas with high prevalence of tuberculosis, medical thoracoscopy is becoming the diagnostic modality of choice for exudative pleural effusions. The aims of this study were to determine the diagnostic yield and safety of medical thoracoscopy for exudative pleural effusions and ascertain the etiology of such effusions in Qatar. METHODS: This is a retrospective-descriptive study of 407 patients who underwent diagnostic medical thoracoscopy for exudative pleural effusions from January, 2008 till December, 2015 at the only tertiary referral center performing this

Interleukin 32 as a Potential Marker for Diagnosis of Tuberculous Pleural Effusion.

Accurate differential diagnosis is the key to choosing the correct treatment for pleural effusion. The present study aimed to assess whether interleukin 32 (IL-32) could be a new biomarker of tuberculous pleural effusion (TPE) and to explore the biological role of IL-32 in TPE. IL-32 levels were evaluated in the pleural effusions of 131 patients with undetermined pleural effusion from Wuhan and Beijing cohorts using an enzyme-linked immunosorbent assay method. Macrophages from TPE patients were transfected with IL-32-specific small interfering RNA (siRNA), and adenosine deaminase (ADA) express

Exosomal microRNAs of tuberculous pleural effusion orchestrating TGF-β signaling mediate pleural fibrosis.

Tuberculosis pleural effusion (TPE) accounts for 30 ~ 80% of all pleural effusions in developing countries and remains a major contributor to global morbidity and mortality. Although TPE is often absorbed after anti-tuberculosis treatment, approximately 60% of patients develop pleural thickening and pleural fibrosis. However, the underlying mechanisms of pleural fibrosis followed TPE remain poorly understood. In this study, firstly we found that exosomes isolated from TPE were associated with pleural fibrotic changes both in vivo and in vitro, accompanied by activation of the TGF-β signaling p

Quantitation in inflammatory pleural disease to distinguish tuberculous and paramalignant from chronic non-specific pleuritis.

AIM: To determine by morphometry if pleural biopsies with the histopathological diagnosis of "non-specific pleuritis", malignant, and tuberculous disease could be distinguished morphologically from those with truly non-specific disease. METHODS: Each pleural biopsy was reviewed taking into account three compartments of reference: the visceral/parietal mesothelial compartment, the submesothelial screen compartment, and the submesothelial adipose tissue compartment. Normal connective tissue, granulation tissue, fibrocellular proliferation, fibrin, polymorphonuclear cells, mononuclear cells, and

Filariasis presenting as isolated pleural effusion: A case report and mini review.

Isolated pleural effusion is a rare manifestation of filariasis that mimics tuberculosis, especially in endemic regions. We describe a case of lymphocytic and exudative pleural effusion showing microfilaria on pleural fluid cytology. A retrospective review of all cases of filarial pleural effusion reported after 2000 was conducted to evaluate the association between filariasis and pleural effusion as well as to screen the features that can help in accurate detection of these patients. The analysis suggested a causal association between the parasite and the development of pleural effusion with

Accuracy of Xpert MTB/RIF assay for the diagnosis of tuberculous pleural effusion.

BACKGROUND: Tuberculosis poses a severe threat to human health. At present, compared with the traditional diagnostic methods for tuberculosis pleural effusion, such as Löwenstein-Jensen culture, pleural biopsy, and Ziehl-Neelsen smear microscopy, Xpert MTB/RIF was regarded as an emerging technology for its efficiency. The Xpert MTB/RIF accuracy for tuberculous pleural effusion diagnosis was evaluated in this systematic study. MATERIALS AND METHODS: We searched the relevant literature published before January 2021 in PubMed, Cochrane, EMBASE, and Web of Science databases. Utilizing Review Mana

[Two cases of IgG4-related disease with pleural effusion].

We describe 2 cases of IgG4-related disease with pleural effusion in elderly men. Both patients had elevated serum IgG4 levels, and the characteristics of their pleural effusion were similar. Patient 1 had pericardial effusion and retroperitoneal fibrosis, and a biopsy specimen from the pericardium showed infiltration of abundant IgG4-positive plasma cells with fibrosis. Because his pleurisy, pericarditis and retroperitoneal fibrosis responded to steroid therapy, we diagnosed pleurisy associated with IgG4-related disease. Patient 2 had been treated with steroids because of IgG4-related sialade

Multiple Cold Abscesses Without Respiratory Symptoms Revealing Multifocal Tuberculosis: A Case Report.

Soft-tissue tuberculosis is an uncommon extrapulmonary manifestation and may be difficult to recognize when respiratory symptoms are absent. We report a 33-year-old, HIV-negative Sri Lankan woman without diabetes or known pharmacological immunosuppression who presented with three months of progressively enlarging, minimally tender superficial collections over the left wrist, right knee, and right ankle, accompanied by anorexia and unquantified weight loss. The swellings had minimal local inflammatory signs. She was anemic and hypoalbuminemic, with elevated inflammatory markers. Separate draina

Transforming growth factor beta-1 as a predictor of fibrosis in tuberculous pleurisy.

BACKGROUND AND OBJECTIVE: To clarify the influence of transforming growth factor beta-1 (TGF-beta(1)) in the development of pleural thickening in tuberculosis (TB), the levels of TGF-beta(1) in pleural fluid and in serum of patients with pleural TB and transudative effusions were determined. METHODS: TGF-beta(1) was quantified in 58 pleural fluid and serum samples of patients with TB (n = 50) or transudative effusions (n = 8). Pleural thickening evaluated by high-resolution CT was scored as 0 (<3 mm); 1 (>or=3 and <10 mm) or 2 (>or=10 mm). RESULTS: The highest pleural fluid TGF-beta(1) level

Diagnostic value of nucleic acid amplification tests for tuberculous pleural effusion.

Diagnosing tuberculous pleural effusion (TPE) is challenging for pulmonologists and laboratory scientists. The gold standards for TPE diagnosis are pleural fluid Ziehl-Neelsen staining, Mycobacterium tuberculosis (Mtb) culture and pleural biopsy. These tools have limitations, including low sensitivity, long turnaround time and invasiveness. The nucleic acid amplification test (NAAT) is a rapid and minimally invasive tool for diagnosing TPE. This review summarizes the diagnostic accuracy of available NAATs for TPE, with a focus on the evidence from systematic reviews and meta-analyses. The NAAT

Five-year Experience of Using Single Port Rigid Thoracoscopy in Patients of Undiagnosed Exudative Pleural Effusion at Tertiary Center in North India.

BACKGROUND: Pleural effusion is one of the common pleural diseases encountered by pulmonologists worldwide. Malignancy and tuberculosis are the two most common etiologies in such patients, who have completely different prognoses. Pleural biopsy is the gold standard investigation to diagnose various causes of pleural effusion. Rigid Medical Thoracoscopy is a semi-invasive tool to evaluate the pleura morphologically and take an image-guided biopsy from the pleura for a definitive diagnosis. MATERIALS AND METHODS: This was a single-center, retrospective data-based study. Data were collected betw
