Neonatal hip septic arthritis: ultrasound should not influence decision to aspirate.

An 18-week-old boy presented to the emergency department with 24-h history of fever and irritability with a painful left hip held in flexion. Blood tests showed an elevated ESR of 65 mm/h, but white cell count and C-reactive protein were in the reference range. Admission hip ultrasound identified a small effusion (<2 mm at the recess), not large enough to determine turbidity. Joint aspiration under general anaesthesia yielded frank pus and mandated open arthrotomy. Physicians must maintain a high index of suspicion of hip septic arthritis irrespective of ultrasound findings and have a low thre

Typical signs of acute appendicitis in ultrasonography mimicked by other diseases?

BACKGROUND: The diagnosis of acute appendicitis in pediatric patients is difficult. There are patients with positive ultrasonography without clinical or histological confirmation of acute appendicitis. It is essential to recognise these patients to avoid unnecessary surgery. METHODS: During 1 year, we compared the patients with 'false-positive' ultrasonography with those with 'true-positive' and those with 'true-negative' ultrasonography. RESULTS: Eighty-two patients were admitted to our inpatient ward for suspected appendicitis. Ultrasonography was performed on 68 patients. In sixteen cases

Renal ultrasonography not required in babies with isolated minor ear anomalies.

AIM: To determine whether infants with isolated minor anomalies of the external ear are at increased risk of renal malformations. METHODS: Consecutive infants with isolated minor anomalies of the external ear (preauricular skin tags, preauricular sinuses, ear pits, and misshapen pinnae) were offered renal ultrasonography by experienced sonographers over a 41 month period. The prevalence of renal anomalies in such infants was compared with that detected on routine fetal scanning during the same period. RESULTS: Ninety six of 13 136 liveborn infants (7.3/1000, 95% confidence interval (CI) 5.9

Appropriateness of colonoscopy in a university hospital.

An open access endoscopy system allows for the direct scheduling of endoscopies by non-gastroenterologist physicians without prior gastroenterology consultation. The aim of our study was to examine our practice of open access endoscopy by evaluating the appropriateness of referrals for colonoscopy and to determine whether there were differences depending on the specialty of the referring clinician. The indication for colonoscopy was assessed in 499 consecutive outpatients referred for colonoscopy at University Hospital, Kuala Lumpur over a 12-month period. The American Society of Gastrointesti

Functional laryngeal dyskinesia: an important cause of stridor.

Functional laryngeal dyskinesia is a disorder that presents with stridor. Failure to recognize its features can result in inappropriate investigation and treatment for a condition that has a psychogenic origin. The key feature in diagnosis is paradoxical vocal fold adduction on inspiration, as seen on fibre-optic nasendoscopy. This phenomenon together with the associated stridor may disappear after distraction techniques or when the patient is asleep. We present five such cases which presented to hospitals in south west London over a 12 month period.

Unexplained Elevated Vitamin B12: Consider Macro-B12.

INTRODUCTION: Elevated vitamin B12 concentration can be caused by supplementation, liver disease, kidney disease, or myeloid malignancies. Persistent, unexplained elevations of vitamin B12 can raise concern among patients and may lead to invasive diagnostic procedures, including bone marrow biopsy. A potential benign cause of this elevation is macro-B12, a complex of vitamin B12, transcobalamin, and immunoglobulins. Although not bioactive, this complex can cause elevated plasma vitamin B12 concentrations due to reduced clearance. METHODS: Polyethylene glycol (PEG) precipitation is a laborator

Zieve's Syndrome in a Patient With Alcohol Use Disorder and Alcohol-Associated Cirrhosis: A Case Report and Review of the Literature.

Zieve's syndrome is a rare and reversible cause of hemolytic anemia classically defined by the triad of hemolytic anemia, hyperbilirubinemia, and transient hyperlipidemia in the setting of recent heavy alcohol use. It is frequently underrecognized, particularly in patients with alcohol-associated liver cirrhosis, where anemia is often attributed to gastrointestinal bleeding, hypersplenism, or bone marrow dysfunction. This syndrome represents an important diagnostic pitfall because transfusion-refractory anemia may be misattributed to gastrointestinal bleeding or autoimmune hemolytic anemia, le

Bilateral renal lymphangiectasia mimicking renal cystic disease and literature review.

Bilateral renal lymphangiectasia is a rare, benign lymphatic disorder that causes the perirenal, parapelvic, and intrarenal lymphatic channels to dilate. Because it is rare and has varying radiologic appearances, it might be confused with other cystic renal illnesses, such as polycystic kidney disease, which can lead to misdiagnosis and unnecessary treatments. In this case review, a 51-year-old female patient who presented with abdominal pain was diagnosed with bilateral renal lymphangiectasia based on imaging findings. Ultrasonography, computed tomography (CT), and magnetic resonance imaging

Fine-needle aspiration biopsy of thoracic coccidioidomycosis: a single-institutional series of 101 cases.

INTRODUCTION: Coccidioidomycosis is a fungal infection endemic to the southwestern United States, particularly Arizona. Fine needle aspiration biopsy (FNAB) is established as effective in diagnosing infectious diseases. However, existing literature evaluating FNAB of thoracic coccidiomycosis remains limited. We present a large single institution series of thoracic coccidioidomycosis cases diagnosed through FNAB. MATERIALS AND METHODS: The Mayo Clinic Arizona Pathology database was searched for all FNAB cases with a diagnosis of coccidioidomycosis from January 1, 2013, to December 31, 2025. El

Iatrogenic endocardial injury associated with preexisting atrial abnormalities.

Considering the rising number of cardiac catheterization and pacemaker placement procedures being performed each year, it is likely that increasing numbers of deaths associated with complications from these procedures will require certification by the medical examiner. We report 2 cases of iatrogenic atrial injury related to pacemaker placement in which a preexisting cardiac abnormality increased the risk of such complications. One case was associated with an embryologic remnant known as Chiari's network, and the other case was associated with lipomatous hypertrophy of the interatrial septum.

Spontaneous resolution in patients with congenital Brown syndrome.

INTRODUCTION: Brown syndrome is characterized by a restriction of active and passive elevation in adduction. It can be congenital, acquired, intermittent, or constant. Spontaneous improvement has been reported in all types but is less common in congenital and constant cases. METHODS: A retrospective review of patients seen between 1992 and 2002 was conducted at our tertiary referral center of all new referrals with a diagnosis of constant congenital Brown syndrome that was previously untreated. RESULTS: Thirty-two patients were identified (22 female and 10 male patients), with an age range a

Giant cystic Schmorl's nodes: imaging findings in six patients.

OBJECTIVE: We describe the imaging findings of an unusual type of Schmorl's node appearing as giant cystlike lesion of the vertebral bodies. CONCLUSION: Giant cystic Schmorl's nodes are unusual entities; their radiologic appearance differs dramatically from the classic description and is diagnostically challenging. However, the appearance of these nodes on conventional radiographs and especially on MR images is characteristic. Knowledge of this entity would help to eliminate unnecessary invasive diagnostic or therapeutic procedures.

Subcranial approach for the correction of hypertelorism.

The subcranial approach for the osteotomy of the skull base and orbital roof in cases with hypertelorism is described. Advancement of the fronto-orbital segments is performed, avoiding the transfrontal procedure, thus making frontal lobe retraction unnecessary. The advantages of this method are pointed out.

Febrile myoclonus: a missed clinical diagnosis.

Myoclonic jerks presenting with fever have only rarely been reported and have often been misdiagnosed as febrile seizures. The presentation may be dramatic enough to provoke unnecessary investigation and treatment. Considering the benign nature of this condition, it is important that the condition is recognized by the physician so that hospitalisation and diagnostic procedures such as lumbar puncture and electroencephalogram are prevented. The authors report three cases of febrile myoclonus and review the subject.

Tension-band arthrodesis in the finger joints.

Tension-band arthrodesis was employed in 26 finger joints with various pathological lesions. This is a simple procedure based on the principles of compression osteosynthesis. Stability thus achieved makes immobilization of the hand unnecessary and allows full motion of all adjacent joints.

Anterior skull base surgery without prophylactic airway diversion procedures.

OBJECTIVE: Although anterior skull base surgery has become a relatively safe and effective procedure, postoperative complications remain a serious problem. One of the most devastating complications of anterior skull base procedures is tension pneumocephalus (TP). In order to prevent TP, authors have recommended the use of prophylactic airway diversion procedures, such as prolonged endotracheal intubation or prophylactic tracheostomy. However, these procedures may mask neurologic deterioration, delay treatment, and prolong rehabilitation. The purpose of this study was to determine the need for

Appropriateness of coronary artery bypass graft surgery performed in northern New England.

OBJECTIVES: The goal of this study was to assess the concordance between the American College of Cardiology (ACC) and the American Heart Association (AHA) 2004 Guideline Update for Coronary Artery Bypass Graft Surgery and actual clinical practice. BACKGROUND: There is substantial geographic variability in the population-based rates of coronary artery bypass graft (CABG) procedures, and in recent years, there have been several public concerns about unnecessary cardiac care. The actual rate of inappropriate cardiac procedures is unknown. METHODS: We evaluated 4,684 consecutive isolated coronar
