Urinary microbiome community types associated with urinary incontinence severity in women.

BACKGROUND: Urinary microbiome (urobiome) studies have previously reported on specific taxa and community differences in women with mixed urinary incontinence compared with controls. Therefore, a hypothesis was made that higher urinary and vaginal microbiome diversity would be associated with increased urinary incontinence severity. OBJECTIVE: This study aimed to test whether specific urinary or vaginal microbiome community types are associated with urinary incontinence severity in a population of women with mixed urinary incontinence. STUDY DESIGN: This planned secondary, cross-sectional an…

[Cystocele and stress urinary incontinence associated with lower urinary tract obstruction].

OBJECTIVES: The association of stress urinary incontinence secondary to urethral hypermobility and lower urinary tract obstruction in the same patient with cystocele is rare, and even represents a contradiction. The objective of our work is to treat to define the characteristics that identify this entity, in comparison with isolated stress urinary incontinence or lower urinary tract obstruction in patients with cystocele. METHODS: We performed a retrospective study in 1168 cases of cystocele in which urodynamic studies were performed. All patients underwent history and neurological and uro-gy…

Management of type III stress urinary incontinence using artificial urinary sphincter.

Type III stress urinary incontinence due to severe intrinsic urethral weakness without significant urethrovesical descensus may be treated by periurethral injection, sling cystourethropexy, bladder neck reconstruction, or artificial urinary sphincter implantation. The rationale for procedure selection depends on a number of patient factors and the surgeon's experience. We herein report on 25 women who were identified as having such incontinence by evaluation which included videourodynamic study and lateral voiding cystography and who were managed by the implantation of an artificial urinary sp…

Artificial urinary sphincter for recurrent female urinary incontinence: indications and results.

A retrospective review of our experience with the artificial urinary sphincter in 32 women is presented. All patients had a history of recurrent urinary incontinence after failed bladder suspension procedures. Of 32 devices 31 were functioning, with an average followup of 2.5 years, and 91 per cent of the patients were dry without pads. Mechanical complications requiring surgical repair occurred in 21 per cent of the patients. Indications for artificial urinary sphincter implantation in the female patient and technical aspects of the procedure are discussed. The artificial urinary sphincter ap…

Urinary Tract Infections Amongst Adults With Intellectual Disabilities With Urinary Incontinence.

BACKGROUND: Between 26% and 52% of adults with intellectual disability experience urinary incontinence (UI). Little is known about the implications of urinary tract infections (UTIs) for this group. The aim was to screen for UTIs in a sample of adults with intellectual disability and UI. METHOD: Twenty adults with intellectual disability and UI recruited via community intellectual disability health care teams and provided a urine sample. Each sample was tested for the presence of UTI bacteria. RESULTS: Half of the sample were found to have a possible or probable UTI. Nine adults had also bee…

Artificial Urinary Sphincter Outperforms Sling for Moderate Male Stress Urinary Incontinence.

OBJECTIVES: To determine the role of slings and artificial urinary sphincters (AUS) in the management of mild and moderate stress urinary incontinence (SUI). METHODS: A retrospective review of our single-surgeon male SUI database was completed. Men having AUS or AdVance sling procedures between 2008 and 2019 were included in the analysis. Those with severe incontinence and/or incomplete pre- or postoperative data were excluded. All patients were evaluated by standing cough test and stratified according to the Male Stress Incontinence Grading Scale. Scores of 0-1 and 2-3 defined mild and moder…

Prevalence of stress urinary incontinence and urge urinary incontinence in multiples sclerosis patients: a systematic review and meta-analysis.

Urinary incontinence (UI), encompassing stress urinary incontinence (SUI) and urge urinary incontinence (UUI), is a prevalent and debilitating condition in patients with multiple sclerosis (MS), profoundly impacting their quality of life. This systematic review and meta-analysis aimed to elucidate the worldwide prevalence rates of SUI and UUI among MS patients. This study was conducted by examining observational studies published between 2000 and 2023. An exhaustive literature search was conducted across databases such as PubMed, MEDLINE, Web of Science, Scopus, ProQuest, and Google Scholar. T…

Is occult stress urinary incontinence truly "occult"?

Stress urinary incontinence (SUI) and pelvic organ prolapse (POP) frequently coexist due to shared pathophysiological mechanisms. In some cases, SUI becomes evident only after the reduction of prolapse, a condition referred to as occult stress urinary incontinence (OSUI). Conventional understanding attributes OSUI to urethral obstruction caused by mechanical kinking or compression from the descending bladder base. This perception has led to ongoing controversy regarding the necessity of concomitant anti-incontinence procedures during POP repair. However, is this conventional interpretation acc…

Women's knowledge and practices regarding urinary incontinence.

BACKGROUND: Urinary incontinence (UI) is a widespread condition affecting a large population globally. Women are disproportionately affected by UI, with the condition becoming more common with advancing age. UI is characterized as involuntary loss of urine during the bladder storage phase, which can significantly diminish a woman's quality of life and impose substantial societal costs. Enhancing women's knowledge is a key factor in reducing the incidence of UI. AIM: To assess the knowledge and practices of women concerning urinary incontinence. METHODS: This descriptive analytical study was …

Can Abdominoplasty Relieve Symptoms of Urinary Incontinence?

INTRODUCTION: Urinary incontinence is a debilitating condition often affecting women after pregnancy or significant weight change. Abdominoplasty is an aesthetic and functional operation often performed in an overlapping cohort. Functional benefits of abdominoplasty include improvements in back pain, urinary incontinence and psychosocial well-being. However, current literature regarding this effect on urinary incontinence is limited. We systematically review the literature to determine the effect of abdominoplasty on urinary incontinence. METHODS: A systematic review of the literature was per…

Stress Urinary Incontinence: An Unsolved Clinical Challenge.

Stress urinary incontinence is still a frequent problem for women and men, which leads to pronounced impairment of the quality of life and withdrawal from the social environment. Modern diagnostics and therapy improved the situation for individuals affected. But there are still limits, including the correct diagnosis of incontinence and its pathophysiology, as well as the therapeutic algorithms. In most cases, patients are treated with a first-line regimen of drugs, possibly in combination with specific exercises and electrophysiological stimulation. When conservative options are exhausted, mi…

Bio-Physics Approach to Urinary Incontinence Disabilities.

BACKGROUND: The terminology of urinary incontinence (UI) and pelvic floor dysfunctions is complex. It affects quality of life and daily activities in personal, social, and professional fields. Managing UI without pharmacologic therapies is effective with a low risk of adverse effects and a large benefit for increasing continence rates. The aim of this preliminary retrospective observational study is to evaluate the effectiveness of the association between manual therapy and focused mechano-acoustic vibrations in women with nonspecific UI. MATERIALS AND METHODS: A group of 15 incontinent women…

Male stress urinary incontinence is often underreported.

UNLABELLED: INTRODUCTION Patient-reported pads per day use is a widely used metric in grading the severity of stress urinary incontinence and guiding surgical decision-making, particularly in mild-to-moderate cases. We sought to compare patient-reported stress urinary incontinence severity by pads per day with objective findings on standing cough test. We hypothesize that patient-reported pads per day often underestimates stress urinary incontinence severity. MATERIALS AND METHODS: We retrospectively reviewed our male stress urinary incontinence surgical database and identified 299 patients w…

[Surgical treatment of female urinary stress incontinence].

Since the description of the sub urethral technique for genuine stress incontinence all the others have been abandoned. The surgical indications have not changed and are related to the poor quality of life of the patient induced by stress incontinence. A careful clinical examination and a pre operative urodynamique investigation should prevent from the failures due to a wrong indication. All the techniques are the same when it comes to the result except the mini slings which still have to be evaluated. There are two types of complications: the ones which are specific to the using of a hexogen

[Urinary incontinence during 12 years after delivery].

INTRODUCTION: First pregnancy and delivery often result in urinary incontinence. The aim of this 12-year cohort study was to estimate the impact of incontinence and the need for treatment. MATERIAL AND METHODS: A total of 305 women were questioned about lower urinary tract symptoms a few days, three months, five years and 12 years after their first delivery. RESULTS: A total of 242 women (79%) answered a questionnaire 12 years after their first delivery. Urinary incontinence increased significantly (p < 0.05) during the 12-year period to a total of 114 symptomatic women (47%); 25 of 102 wome…

Animal models of female stress urinary incontinence.

PURPOSE: Urinary incontinence affects 40% of women in the United States and stress urinary incontinence accounts for a large portion of affected patients. As defined by the International Continence Society, stress urinary incontinence is the involuntary leakage of urine upon effort, exertion, sneezing or coughing. Since the ultimate success of long-term management for any condition is based on an understanding of its pathophysiology, and because the pathophysiology of stress urinary incontinence is incompletely defined, animal models have recently been developed to better understand stress uri…
