Laparoscopic adaptation of highly selective vagotomy procedures associated with no mortality, low morbidity and no reports of diarrhoea and dumping syndrome has been reported. Parietal cell vagotomy (PCV) was used for a variety of gastrointestinal conditions in 658 patients. To test the hypothesis that vagally mediated metabolic signals from the liver affect feeding, we investigated the influence of selective hepatic vagotomy on the anorectic effect of different metabolites in rats. Changes in the gastric mucosa after a partial gastrectomy may predispose to neoplasia, but epithelial proliferation in such patients has not been studied well. Highly selective vagotomy was performed on five dogs. Postoperatively, gastrin cell (G cell) hyperplasia occurred in all dogs. Mean preoperative G cell numbers increased from 350 to 530/cm mucosal length (p less than 0.02). Antral tissue gastrin also increased by 100 per cent (6.7 x 10(6) to 13.7 x 10(6) pg/gm tissue, p less than 0.05). Basal and stimulated serum gastrin were unchanged following highly selective vagotomy. The cause for G cell hyperplasia is not clear, but is probably multifactorial.

Between 1971 and 1976 selective proximal vagotomy (SPV) was performed in 526 patients with uncomplicated duodenal ulcer and in 37 with gastric ulcer. The operative mortality was 0.18%. The post-operative course was without complication in 90%. In a randomized controlled trial 50 patients with duodenal ulcer treated by proximal gastric vagotomy (P.G.V.) without drainage were compared with 50 who underwent selective vagotomy and gastrojejunostomy. The clinical results were assessed in 99 patients one to four years after operation. Thirty-two duodenal ulcer perforations were treated by suture and supraselective vagotomy. Perioperative mortality was nil. The recurrence rate was 6.2% against 17% after elective supraselective vagotomy performed by the same surgeons during the same period. Results of this technique for treatment of perforated duodenal ulcer are compared with those of 28 simple sutures and 32 truncal vagotomies.
