The watery diarrhoea, hypokalaemia and achlorhydria syndrome is a rare cause of secretory diarrhoea. In this case report, we highlight a young female with watery diarrhoea, hypokalaemia and achlorhydria syndrome as a consequence of a vasoactive intestinal peptide producing composite adrenal phaeochromocytoma-ganglioneuroma. The neuropeptide vasoactive intestinal peptide (VIP) is involved in the neuroimmunomodulation of the intestine. In the present study, specimens from the sigmoid colon of ulcerative colitis (UC) and non-UC patients were examined for immunohistochemistry and in vitro receptor autoradiography. The influence of several classes of drugs on the melatonin-induced vasorelaxation of isolated rat aorta were examined. Melatonin caused a dose-dependent relaxation of precontracted (30 mM KCl) aorta. This relaxant response was blocked by preincubating vessels with antagonists of vasoactive intestinal peptide. Substance P antagonists did not alter the response to melatonin. PURPOSE: The underlying mechanisms for cerebral blood flow (CBF) abnormalities in acute bacterial meningitis (ABM) are largely unknown.
