Radiofrequency ablation of the cavotricuspid isthmus for management of atrial flutter in patients with congenital heart disease after tricuspid valve surgery: A single-center experience.

BACKGROUND: Typical atrial flutter involving the cavotricuspid isthmus (CTI) is the most common reentrant arrhythmia in congenital heart disease and ablation is effective in its management. However, congenital heart disease patients often require surgical interventions on their tricuspid valve that utilize prosthetic material, making CTI ablation technically challenging. OBJECTIVE: To describe the techniques and outcomes of CTI ablation in the presence of prior tricuspid valve repair or replacement. METHODS: We included all patients who had undergone tricuspid valve repair utilizing an annul

[Emergency treatment of arrhythmias in neonates and infants].

Emergency treatment of cardiac arrhythmias was required in 41 newborn and infants aged two days to 9 months (mean 77 days) from July 1977 until September 1981. Heart defects were present in 27 (65.8%). Invasive electrophysiological studies were performed in all patients. The different types of arrhythmias were: bradyarrhythmias in 9 (21.9%): bradycardia to cardiac arrest (5), congenital complete AV-block (3), postoperative complete AV-block (1). Tachyarrhythmias in 32 patients (78.1%): reentry through accessory connections (21), congenital atrial flutter (6), ventricular flutter/fibrillation (…

The Role of Quinidine in the Pharmacological Therapy of Ventricular Arrhythmias 'Quinidine'.

Historically, quinidine was the first medicine used in the therapy of heart arrhythmias. Studies in the early 20th century identified quinidine, a diastereomer of the antimalarial quinine, as the most potent of the antiarrhythmic substances extracted from the cinchona plant. Quinidine is used by the 1920s, as an antiarrhythmic agent to maintain sinus rhythm after the conversion from atrial flutter or atrial fibrillation and to prevent recurrence of ventricular tachycardia or ventricular fibrillation. Its value in chronic prophylaxis of relapse of ventricular arrhythmia was brought under suspic

Comparative Efficacy of Dofetilide Versus Amiodarone in Patients With Atrial Fibrillation.

OBJECTIVES: The study's goal was to compare the efficacy and safety of dofetilide (DOF) versus amiodarone (AMIO) in patients with atrial fibrillation (AF). BACKGROUND: Comparative efficacy of DOF versus AMIO in patients with AF has not been well established. In addition, proarrhythmia has been a concern with DOF therapy. METHODS: Rhythm control was attempted by using DOF in 657 consecutive patients (mean age 72 ± 9 years; 35% women) with AF (n = 528) or atrial flutter and AF (n = 129) between January 2014 and December 2018. RESULTS: DOF was successfully initiated in 573 (87%) of 657 patient

Biphasic versus monophasic shock for external cardioversion of atrial flutter: a prospective, randomized trial.

BACKGROUND: External cardioversion is effective to terminate persistent atrial flutter. Biphasic shocks have been shown to be superior to monophasic shocks for ventricular defibrillation and atrial fibrillation cardioversion. The purpose of this trial was to compare the efficacy of rectilinear biphasic versus standard damped sine wave monophasic shocks in symptomatic patients with typical atrial flutter. METHODS: 135 consecutive patients were screened, 95 (70 males, mean age 62 +/- 13 years) were included. Patients were randomly assigned to a monophasic or biphasic cardioversion protocol. For

GIANT Flutter Waves in ECG Lead V1: a Marker of Pulmonary Hypertension.

Atrial flutter (AFl) may exist with or without underlying structural heart disease. Typical AFl presents as a "sawtooth" pattern on the ECG - with inverted flutter (F) waves in the inferior leads and upright F waves in V1. This morphology offers no direct clues as to the underlying cardiac disorder, if any. Occasionally we have encountered giant F waves, most prominently in lead V1, reaching 5 mv or more in height - sometimes exceeding the QRS voltage. The significance of this pattern has not been investigated and reported on. To determine if giant F waves in V1 provide any insight into the pr

Intra-atrial cardioversion in supraventricular tachycardial arrhythmia.

Cardioversion by rapid atrial stimulation has been carried out in 29 patients with supraventricular tachycarida and 12 patients with "coarse" atrial fibrillation. Atrial stimulation rates of 60-1200/min and electrical impulses of 6-25 mA were used. Intracardial conversion was successful in 86% of the cases presenting supraventricular tachycardia. Cardioversion was not achieved with atrial fibrillation but atrial flutter and atrial tachycardia were both found to be easily converted. Transformation of an arrhythmia into sinus rhythm or into stable atrial fibrillation with a slowed ventricular ra

Sinus of Valsalva Pseudoaneurysm Complicating Infective Endocarditis Causing Right Ventricular Inflow Obstruction.

Sinus of Valsalva pseudoaneurysm (SVpA) is a rare cause of right ventricular inflow obstruction. We report such a case presented with atrial flutter and cardiogenic shock caused by tricuspid valve obstruction by a narrow-necked right SVpA complicating aortic valve infective endocarditis demonstrated on transesophageal echocardiography and cardiac computed tomography. Despite restoration of sinus rhythm, the patient ran a fatal course due to aneurysmal rupture. We show the value of transesophageal echocardiography in evaluating unstable patients with cardiogenic shock, and the need of urgent su

Cardiac arrhythmias in the fetus and newborn.

Arrhythmias are potentially life-threatening problems in the fetus and newborn. Appropriate management depends on accurate diagnosis. Atrioventricular re-entry is the most common type of supraventricular tachycardia in both the fetus and newborn. It should be distinguished from other types of tachycardia - such as atrial flutter, atrial ectopic tachycardia, permanent junctional re-entry tachycardia, and ventricular tachycardia. Neonatal and fetal bradycardias are less common clinical problems. Sustained bradycardia is most often caused by complete atrioventricular block. It can be mimicked by

Intra-atrial reentrant tachycardia after palliation of congenital heart disease: characterization of multiple macroreentrant circuits using fluoroscopically based three-dimensional endocardial mapping.

INTRODUCTION: The anatomic substrate of intra-atrial reentrant tachycardia (IART) following congenital heart surgery is poorly understood, but is presumed to be different than common atrial flutter. METHODS AND RESULTS: To study the mechanisms of IART, we used a new technique for high-density endocardial mapping using recordings from a multipolar basket recording catheter (25 bipolar pairs). For each recording, biplane fluorographic reference points were digitized to obtain the spatial locations of electrode pairs, and activation times were calculated using temporal reference points from the

[Echocardiographic diagnosis of subpulmonic obstruction in hypertrophic cardiomyopathy (a case report) (author's transl)].

The echocardiogram of a patient presenting with a systolic ejection murmur and ECG evidence of left ventricular hypertrophy revealed asymmetric septal hypertrophy but no abnormal systolic anterior motion of the mitral valve as in IHSS. Her pulmonary echo disclosed small A waves, partial valve closure in early systole and coarse systolic fluttering. This motion pattern--which is similar to that of patients with infundibular pulmonary stenosis and which resembles the aortic valve motion pattern in discrete subaortic stenosis--led us to the diagnosis of hypertrophic cardiomyopathy with subpulmoni

Ibutilide: what's clinically proven, what's being investigated?

Ibutilide is a class III antiarrhythmic drug used intravenously for the chemical cardioversion of atrial fibrillation (AF) and atrial flutter (AFL). It mainly acts by blocking the rapid component (Ikr) of delayed rectifier K+ channels, resulting in prolongation of action potential duration in the atrium. Ibutilide fumarate is a methanesulfonamide derivative that is structurally similar to sotalol without the β-adrenoreceptor blocking activity. The success rate of cardioversion of atrial flutter with ibutilide is in the range of 50-70%, whereas its efficacy for the conversion of atrial fibrilla

Masquerading Bundle Branch Block: A Critical ECG Pattern Hidden in Plain Sight.

Masquerading bundle branch block (MBBB) is a rare and often overlooked electrocardiographic (ECG) pattern characterized by right bundle branch block (RBBB) morphology in the right precordial leads combined with features resembling left bundle branch block in the limb leads and/or the left precordial leads. This pattern reflects bilateral conduction system disease and carries a significant risk of progression to high-grade atrioventricular block (AVB). We report the case of an 84-year-old woman admitted for acute decompensated heart failure with preserved ejection fraction and atrial flutter. H

Antiarrhythmic effects of acute betablockade with atenolol on supraventricular tachycardias at rest and during exercise.

In a total of 18 patients, 7 females and 11 male patients with ages ranging from 23 to 70 years (mean: 45.5 +/- 14.5) diagnostic His bundle studies incorporating programmed atrial and ventricular pacing for the induction of tachycardias was performed before and after betablockade with the cardioselective betablocking agent atenolol, in a dose of 5 mg given iv. over 3 to 5 minutes. In 7 patients the pacing procedure could be repeated following ergometric exercise in order to evaluate the influence of a raised sympathetic tone on the conditions initiating paroxysmal tachycardias. At rest, atenol

Runaway pacemaker.

Runaway pacemaker is phenomenon in which pacemaker induces ventricular tachycardia as a result of some malfunction in the device. This entity is not very common in clinical practice. Normally, the pacemaker perceives the inherent cardiac impulse and hence averting the delivery of pacemaker stimulus in susceptible period of cardiac cycle. This is a case of a pacemaker-induced tachycardia (named as runaway pacemaker) in a patient with a history of Single Chamber Ventricular (VVI) pacemaker. A 75-year-old man was admitted with 3 days history of fluttering in the chest, shortness of breath, centra

Nonpharmacologic approaches to the treatment of atrial fibrillation and atrial flutter.

The high prevalence of atrial fibrillation, the associated morbidity and mortality, the absence of safe and effective drug therapy, and an increased understanding of the pathophysiologic basis of atrial fibrillation and flutter have collectively led to the development of novel nonpharmacologic treatments for the management of these arrhythmias, including the CORRIDOR and MAZE surgical procedures, catheter-based ablation and modification of AV conduction, catheter-based ablation of atrial flutter and fibrillation, and internal atrial defibrillation. These surgical and catheter-based techniques
