Vertebral body tethering for Lenke 1A curves: the lumbar modifier predicts less optimal outcomes.

INTRODUCTION: The addition of the L4 "AR" and "AL" lumbar modifier for Lenke 1A idiopathic scoliosis (IS) has been shown to direct treatment in posterior spinal fusion; however, its utility in vertebral body tethering (VBT) has yet to be evaluated. METHODS: A review of a prospective, multicenter database for VBT in IS was performed for patients with Lenke 1A deformities and a minimum of 2 years follow-up. Patients were categorized by their lumbar modifier (AR vs AL). Less optimal VBT outcome (LOVO) was defined as a final coronal curve > 35°, lumbar adding-on, or revision surgery for deformity…

Anterior vertebral body tethering shows clinically comparable shoulder balance outcomes to posterior spinal fusion.

PURPOSE: Posterior spinal fusion (PSF) is the current gold standard in surgical treatment for adolescent idiopathic scoliosis. Vertebral body tethering (VBT) is a fusionless alternative. Shoulder balance is an important metric for outcomes and patient satisfaction. Here we compare shoulder balance outcomes between PSF and VBT. METHODS: In this retrospective review, the pre-operative and post-operative absolute radiographic shoulder height (|RSH|) of 45 PSF patients were compared to 46 VBT patients. Mean values were compared and then collapsed into discrete groups (|RSH| GROUP) and compared. P…

3D-printed vertebral body for anterior spinal reconstruction in patients with thoracolumbar spinal tumors.

OBJECTIVE: A 3D-printed vertebral prosthesis can be used to reconstruct a bone defect more precisely because of its tailored shape, with its innermost porous structure inducing bone ingrowth. The aim of this study was to evaluate the clinical outcomes of using a 3D-printed artificial vertebral body for spinal reconstruction after en bloc resection of thoracolumbar tumors. METHODS: This was a retrospective analysis of 23 consecutive patients who underwent surgical treatment for thoracolumbar tumors at our hospital. En bloc resection was performed in all cases, based on the Weinstein-Boriani-Bi…

Predicting radiological vertebral fractures with a combined physical function and body composition scoring system.

The objective of this study was to investigate the incidence of vertebral fractures (VFx) and the value of physical function (PF) and body composition (BC) for predicting VFx in a Japanese population. This study included 307 subjects (113 men, 194 women) at least 40 years of age who were assessed at community health check-ups in 2008 and 2016. PF was assessed by grip strength and by single-leg stance, timed up-and-go, and 30-s chair stand tests, each scored from 0 to 3 for a possible total of 12 points (higher scores reflect lower function). BC was scored on bioelectrical impedance measurement

Vertebral compression fractures after stereotactic body radiation therapy: a large, multi-institutional, multinational evaluation.

OBJECTIVE The purpose of this study was to identify factors contributing to an increased risk for vertebral compression fracture (VCF) following stereotactic body radiation therapy (SBRT) for spinal tumors. METHODS A total of 594 tumors were treated with spinal SBRT as primary treatment or re-irradiation at 8 different institutions as part of a multi-institutional research consortium. Patients underwent LINAC-based, image-guided SBRT to a median dose of 20 Gy (range 8-40 Gy) in a median of 1 fraction (range 1-5 fractions). Median patient age was 62 years. Seventy-one percent of tumors were ost

Complete cage migration/subsidence into the adjacent vertebral body after posterior lumbar interbody fusion.

A variety of implant-related short and long-term complications after lumbar fusion surgery are recognized. Mid to long-term complications due to cage migration and/or cage subsidence are less frequently reported. Here, we report a patient with a complete cage migration into the superior adjacent vertebral body almost 20 years after the initial posterior lumbar interbody fusion procedure. In this patient, the cage migration/subsidence was clinically silent, but a selective decompression for adjacent segment degenerative lumbar spinal stenosis was performed. We discuss the risk factors for cage …

The Effects of Vertebral Body Tethering on the Intervertebral Discs and Facet Joints: A Numerical Analysis.

Anterior vertebral body tethering (VBT) is a promising technique for the treatment of adolescent idiopathic scoliosis. However, the segments directly treated with VBT can experience substantial loads resulting from the tether pretension, which may alter internal stresses and potentially compromise structures such as the intervertebral discs (IVDs) and facet joints. We aim to investigate the effects of tether within the VBT on the L1-L2 IVD stresses and contact forces of the facet joints, using an extensively calibrated and validated finite element model of the T10-S1 spine. The implant was ins…

An automated algorithm for quantitative morphometry of thoracic and lumbar vertebral bodies in lateral radiographs.

This exploratory study developed and evaluated an artificial intelligence (AI)-based algorithm for quantitative morphometry to assess vertebral body deformities indicative of fractures. To achieve this, 709 radiographs from 355 cases were utilized for algorithm development and performance evaluation. The proposed algorithm integrates a first-stage AI model to identify the positions of thoracic and lumber vertebral bodies in lateral radiographs and a second-stage AI model to annotate 6 landmarks for calculating vertebral body height ratios (C/A, C/P, and A/P). The first-stage AI model achieved …

Multilevel Vertebral Body Osteonecrosis in an Adult Patient: A Rare Case with Review of Literature.

Osteonecrosis, characterized by ischemic bone injury leading to cell death and bone resorption, manifests in various anatomical sites due to multiple etiologies such as radiotherapy and medications. While certain sites like the femoral head and jaw have garnered attention, vertebral body osteonecrosis remains less explored and lacks a standardized classification system. This report presents a unique case of a 52-year-old woman undergoing chemotherapy for diffuse large B-cell lymphoma who developed multilevel vertebral body osteonecrosis, a rare occurrence in English medical literature. The pat…

The island rule explains consistent patterns of body size evolution in terrestrial vertebrates.

Island faunas can be characterized by gigantism in small animals and dwarfism in large animals, but the extent to which this so-called 'island rule' provides a general explanation for evolutionary trajectories on islands remains contentious. Here we use a phylogenetic meta-analysis to assess patterns and drivers of body size evolution across a global sample of paired island-mainland populations of terrestrial vertebrates. We show that 'island rule' effects are widespread in mammals, birds and reptiles, but less evident in amphibians, which mostly tend towards gigantism. We also found that the
