[Sudden onset vertigo: imaging work-up].

Vertigo and dysequilibrium are a frequent cause of medical consultation. Clinical evaluation is essential. Some cases of vertigo are diagnosed clinically while others require imaging, sometimes emergently (suspected stroke). MRI is the imaging modality of choice to assess the labyrinth (labyrinthitis? labyrinthine hemorrhage?), internal auditory canal (vestibular schwannoma? other tumor?…) and brain parenchyma including all structures of the auditory pathways: vestibular nuclei, vestibulocerebellar tract, tracts involved with ocular motricity, vestibular cortex… Multiple central etiologies exi

Migraine-associated vertigo: diagnosis and treatment.

Migraine-associated vertigo has become a well-recognized disease entity diagnosed based on a clinical history of recurrent vertigo attacks unexplained by other central or peripheral otologic abnormalities, which occurs in the patient with a history of migraine headaches. There is no international agreement on what spectrum of symptoms should be covered under this diagnosis, or what terminology should be used. The headaches and vestibular symptoms of migraine-associated vertigo may not be temporally associated, which often obscures the association. Diagnostic tests usually show nonspecific abno

[Acute pseudoperipheric vertigo and Wallenberg syndrome].

We report an atypical Wallenberg syndrome presented with an acute vertigo, preceded by headache, simulating a peripheral vestibular pathology. The first Magnetic Resonance Imaging (MRI) study done in urgencies is negative. Presence of headache, strong lateral ataxia and secondary focally neurological symptoms 48h after the beginning of the crises, lead to a repetition of the MRI which shows a right lateral bulbar infarct. We focus on the semiological elements that can guide us to a lateral bulbar pathology in patients with pseudoperypheric symptoms (axial latero-pulsion, ocular laterodeviation

Benign positional vertigo after cochlear implantation.

OBJECTIVE: To identify patients who underwent cochlear implantation (CI) and who subsequently developed benign positional vertigo (BPV) after the procedure and to identify any contributing factors. STUDY DESIGN AND SETTING: Academic tertiary referral center. Cochlear implant recipients' medical records were retrospectively reviewed to identify patients with both vertigo and, more specifically, BPV. Preoperative, intraoperative, and postoperative factors were studied vis-a-vis the development of BPV. RESULTS: BPV was newly diagnosed in 12 patients after CI. The etiology of hearing loss includ

[Additive treatment for central vestibular vertigo].

We have observed in our own practice that numerous patients with primary symptoms of vertigo exhibit cervical segmental muscular imbalance and increased tension in the masticatory musculature. This is frequently associated with functional blockades, especially in the joints of the head and upper cervical spine. Particularly important are special receptors of the small vertebral joints and muscle insertions at the cervicocranial transition. Evidence indicates that there are neuroanatomic structures between these receptors and the central vestibular and cochlear core area of the brain stem, whic

Episodic vertigo: central nervous system causes.

Episodic ataxia type 2 is a prototypical episodic vertigo and ataxia syndrome that is caused by mutations in the calcium channel gene CACNA1A. Recent discoveries regarding the molecular mechanisms that underlie this syndrome provide a model for understanding the more common familial episodic vertigo syndromes, particularly those associated with migraine. Vertigo due to cerebrovascular disease can be of peripheral or central origin, and can mimic more benign peripheral vestibular disorders. Small infarcts in the cerebellum and lateral medulla can present with vertigo without other localizing sy

Management of benign paroxysmal positional vertigo.

Benign paroxysmal positional vertigo (BPPV) is one of the most common balance disorders of the peripheral vestibular system. Diagnosis is made from patient history and the manifestation of specific symptoms. Treatment differs for the individual patient and may include pharmacological, surgical, dietary, and/or therapeutic modalities. Various versions of Modified Liberatory Maneuvers (MLM), and other, similar rehabilitation therapy treatments have been shown to be very effective in the management of the BPPV patient.

[Rare differential diagnosis in "vertigo" symptoms].

The diagnosis of vestibulocochlear nerve and statoacoustic organ lesions is based on the symptoms vertigo, hearing loss and tinnitus. The most common diseases are vestibular neuronitis, Ménière's disease, benign positional vertigo and sudden hearing loss. 6 patients are presented, complaining of at least one of the symptoms vertigo, hearing loss or tinnitus. They were suspected of suffering from a peripheral audiovestibular disease when admitted to the clinic, but all of them revealed other pathologies. A careful history and step-wise otoneurologic diagnostics including clinical examination, o

[Migraine: differential diagnosis in episodic vertigo].

Vestibular symptoms can be the predominant feature of migraine both in children and adults. Attacks of spontaneous or positional vertigo lasting from minutes to days may occur with or without concomitant headache. In the literature three syndromes of vestibular migraine have evolved: basilar artery migraine, benign recurrent vertigo and benign recurrent vertigo of childhood. In clinical practice, however, variants seem to be more frequent than the pure syndromes. Diagnosis is based on the individual constellation of typical precipitants and symptoms of migraine and the efficacy of pharmacologi

Episodic vertigo in basilar artery migraine.

Vestibular symptoms commonly occur in migraine, and episodic vertigo is most frequently seen. Auditory symptoms also occur, but are less common. When Bickerstaff described basilar artery migraine in 1961, he postulated that the many different symptoms were caused by basilar artery ischemia. He documented that neuro-otologic and other symptoms could occur before or during a migraine headache; others later established that these symptoms could also occur during the headache-free period. Case histories of eleven patients with basilar artery migraine are presented in detail. All met the diagnostic

[Epidemiology of benign paroxysmal positional vertigo].

Every ninth patient with a previous history of vertigo at the ENT Hospital of the University of Freiburg suffers from BPPV. The incidence, some important pathogenetic factors and the clinical picture of BPPV are discussed. In young men posttraumatic aetiology dominates, whereas idiopathic BPPV prevails in perimenopausal women. The affected ear tends to be hard of hearing in the high frequencies and is sometimes hypoexcitable by caloric stimulation. The diagnosis is best made by personal examination with Frenzel's glasses in darkness. Electronystagmography, brainstem evoked response audiometry,

Cryo-labyrinthectomy for post-mastoidectomy vertigo.

A case of severe vertigo, following multiple operations for chronic suppurative otitis media is described. Sensitive nystagmus-producing spots in the mastoid cavity were treated by cryosurgery. Despite the absence of histological evidence of sensory epithelium or nerve endings, a good result was obtained.

Evaluating associations of migraine-related vertigo.

OBJECTIVE: We set out to examine detailed phenotypic data from our clinic for associations of vertigo in chronic migraine. BACKGROUND: Vertigo is a non-canonical, common symptom of migraine. Little is known about its associations with other symptoms within the migraine phenotype. There is significant methodological heterogeneity and therefore inadequate overall evidence about the potential differences in efficacy of any migraine treatments in patients with problematic vertigo associated with migraine, compared to those without vertigo. Enhancing understanding of migraine-related vertigo could

[Vertigo and ocular inflammation: Cogan syndrome].

BACKGROUND: Cogan syndrome is a rare inflammatory condition that mainly affects adults and is characterised by inflammation of various ocular structures and by audiovestibular symptoms such as hearing loss and vertigo. CASE DESCRIPTION: A 63-year old woman recently diagnosed with an anterior uveitis presented at A&E with vertigo, nausea, vomiting, tinnitus and headache, and she developed bilateral sudden deafness within days. Blood testing revealed elevated inflammatory parameters, without signs of infection. Additional laboratory and imaging tests showed no abnormalities. We finally diagnose
