Endolymphatic hydrops and blood-labyrinth barrier in Ménière's disease.

CONCLUSIONS: The blood-labyrinth barrier is impaired in association with the hydrops grade in Ménière's disease. OBJECTIVES: To investigate the relationship between endolymphatic hydrops and the clinical characteristics of patients with Ménière's disease revealed by 3 T magnetic resonance imaging (MRI). METHODS: A double dose of gadoteridol (Gd; 0.2 mmol/kg) was injected intravenously in 12 patients with Ménière's disease. We performed three-dimensional fluid attenuated inversion recovery MRI and three-dimensional real inversion recovery MRI 4 h later using a 3 T MRI unit. Ten patients had u

Why do vestibular destructive procedures sometimes fail?

Vestibular nerve specimens and one temporal bone, from patients with vestibular symptoms after destructive surgery on the vestibular system, were studied by light microscopy. Surviving nerve axons in three specimens that followed retrolabyrinthine vestibular nerve section (RLVNS) were counted and compared to normative data. Results are consistent with persistence of the central processes of primary vestibular neurons in three specimens from patients who had persistent symptoms and ice-water caloric responses after RLVNS. Incomplete neurectomy probably results from anatomic variations in the pl

Pathogenesis and pathophysiology of Meniére's disease.

Meniére's disease, neither spontaneous nor inducible in animals, is studied only in patients. Natural history (epidemiology) shows a triad of major symptoms: vestibular, auditory, and aural pressure. One in 3 patients has bilateral Meniére's; over full lifespans, bilaterality approaches 50%. Aural pressure (74.1%) and positional vertigo during/between attacks (85.9%) are common. Clinical variants can persist for 25+ years. All forms have delayed onset and can occur years after incitement, after otosclerosis, infections like otitis media, syphilis, or trauma. Endolymphatic hydrops is found in a

[Magnetic resonance tomography imaging of the inner ear of patients with sensorineural hearing loss or vertigo].

BACKGROUND: Progress in surgical techniques in the temporal bone has lead to the demand of exact delineation of this complex anatomical region for preoperative planning. The aim of this study was to assess the potential of a three-dimensional T2 weighted turbo-spin-echo-sequence to depict anatomical details and pathological changes of the inner ear. METHODS: Twenty-five patients presenting with sensori-neural hearing loss and/or vertigo were included in this study. A T2 weighted turbo-spin-echo-sequence was carried out on a 1.5 T imager, employing a surface coil with a diameter of 8 cm. The s

The Vestibular Nuclei: A Cerebral Reservoir of Stem Cells Involved in Balance Function in Normal and Pathological Conditions.

In this review, we explore the intriguing realm of neurogenesis in the vestibular nuclei-a critical brainstem region governing balance and spatial orientation. We retrace almost 20 years of research into vestibular neurogenesis, from its discovery in the feline model in 2007 to the recent discovery of a vestibular neural stem cell niche. We explore the reasons why neurogenesis is important in the vestibular nuclei and the triggers for activating the vestibular neurogenic niche. We develop the symbiotic relationship between neurogenesis and gliogenesis to promote vestibular compensation. Finall

Vestibular compensation: Neural mechanisms and clinical implications for the treatment of vertigo.

After unilateral peripheral vestibular lesions, the neural activity of neurons in the ipsi-lesional medial vestibular nucleus (ipsi-MVe) are markedly decreased, resulting in static and dynamic asymmetries of the vestibulo-ocular and vestibulo-spinal reflexes. Consequently, static vestibular symptoms such as spontaneous nystagmus and postural deviation and dynamic vestibular symptoms such as oscillopsia and swaying gait are induced. However, these behavioral asymmetries gradually recover after the lesion. Progressive balance restoration is termed vestibular compensation, which is divided into t

A Visual-Vestibular Model to Predict Motion Sickness for Linear and Angular Motion.

OBJECTIVE: This study proposed a model to predict passenger motion sickness under the presence of a visual-vestibular conflict and assessed its performance with respect to previously recorded experimental data. BACKGROUND: While several models have been shown useful to predict motion sickness under repetitive motion, improvements are still desired in terms of predicting motion sickness in realistic driving conditions. There remains a need for a model that considers angular and linear visual-vestibular motion inputs in three dimensions to improve prediction of passenger motion sickness. METHO

Seasickness susceptibility and the vestibular time constant: a prospective study.

Human passive motion during boat, car or airplane travel may trigger motion sickness. Seasickness is the most provoking manifestation of motion sickness. It imposes major constraints on quality of life and human performance. Based on seasickness susceptibility the population is usually categorized into susceptible (S) and non-susceptible (NS). During repeated exposure some susceptible individuals undergo habituation and obtain symptoms relief, reflecting a third group of habituating (H) individuals. Recently, accumulative evidence suggests that the vestibular time constant (Tc) is associated w

Comparison of clinical characteristics and vestibular function test results in patients with vestibular migraine and Menière's disease.

OBJECTIVES: The differentiation between Vestibular Migraine (VM) and Meniere's Disease (MD) is difficult because of overlapping symptoms. The study aimed to compare the clinical characteristics and vestibular function test results between VM and MD patients. METHODS: Seventy-one patients with definite VM and 31 patients with definite unilateral MD were included. All patients received Caloric Test (CT), Video Head Impulse Test (vHIT) and Vestibular Evoked Myogenic Potential (VEMP) test within 7 days after visiting the hospital. Results of these tests were compared between groups. RESULTS: Mos

Sox10 Gene Is Required for the Survival of Saccular and Utricular Hair Cells in a Porcine Model.

Pathological changes of the cochlea and hearing loss have been well addressed in Waardenburg syndrome (WS). However, the vestibular organ malformation in WS is still largely unknown. In this study, the differentiation and development of vestibular sensory epithelium and vestibular function caused by SOX10 mutation, a critical gene induces WS, have been studied in minature pig model. Degeneration of vestibular hair cells was found in this Sox10 mutation porcine model. Inner ear phenotype of the SOX10+/R109W miniature pigs showed cochlear abnormalities as well as saccular hypofunction. In the mu

Postural stability and handicap of dizziness after preoperative vestibular ablation and vestibular prehabilitation in patients undergoing vestibular schwannoma resection.

BACKGROUND: Surgical treatment of vestibular schwannoma (VS) leads to acute ipsilateral vestibular loss if there is residual vestibular function before surgery. To overcome the sequelae of acute ipsilateral vestibular loss and to decrease postoperative recovery time, the concept of preemptive vestibular ablation with gentamicin and vestibular prehabilitation before surgery has been developed ("vestibular prehab"). OBJECTIVE: Studying postural stability during walking and handicap of dizziness over a 1-year follow-up period in VS patients undergoing vestibular prehab before surgical treatment

Penetration of the Vestibule Following a History of Stapedectomy.

This case study describes a 72-year-old female with a history of stapedectomy 40 years prior. She presented experiencing vertigo, fogginess, and imbalance for 9 months. Computed tomography (CT) imaging revealed that the prosthesis was displaced into the vestibule by approximately 2.1 to 2.4 mm. The patient was presented with treatment options, including observation, removal, replacement of the prosthesis, and an oval window patch. The patient opted for observation as the symptoms she was experiencing did not significantly impact her quality of life. Although a stapedectomy may fail for a multi

Influence of panoramic cues during prolonged roll-tilt adaptation on the percept of vertical.

The percept of vertical, which mainly relies on vestibular and visual cues, is known to be affected after sustained whole-body roll tilt, mostly at roll positions adjacent to the position of adaptation. Here we ask whether the viewing of panoramic visual cues during the adaptation further influences the percept of the visual vertical. Participants were rotated in the frontal plane to a 90° clockwise tilt position, which was maintained for 4-minutes. During this period, the subject was either kept in darkness, or viewed panoramic pictures that were either veridical (aligned with gravity) or ori

Association of Lymphatic Fluid Volume in the Inner Ear of Beagle Dogs with the Susceptibility to Motion Sickness.

BACKGROUND: This study aimed to quantify total lymphatic fluid spaces of the inner ears volumetrically in the dog in order to find a correlation between the lymphatic volume of the inner ears and motion sickness susceptibility. METHODS: A total of 16 healthy adult Beagle dogs were used to delineate the lymphatic fluid spaces of inner ears by magnetic resonance imag- ing with a 3-dimensional-constructive interference steady-state sequence. Manual segmentation was applied for 3-dimensional reconstruction and volumetric quantification of total lymphatic space. The susceptibility of Beagle dogs t

Efficacy of Galvanic Vestibular Stimulation as a Display Modality Dissociated from Self-Orientation.

OBJECTIVE: We propose and assess galvanic vestibular stimulation (GVS) as a novel means to provide information dissociated from self-orientation. BACKGROUND: In modern user interfaces, visual and auditory modalities dominate information transfer so much that these "processing channels" become overloaded with information. Fortunately, the brain is capable of processing separate sensory sources in parallel enabling alternative display modalities to inform operators more effectively and without increasing cognitive strain. To date, the vestibular system, normally responsible for sensing self-ori

Discordant horizontal-torsional nystagmus: a sign of posterior semicircular canal dysfunction.

In central as well as peripheral vestibular lesions, right-beating horizontal nystagmus is almost always associated with clockwise (top poles of the eyes beating to the right ear) torsional nystagmus when observed and vice versa (concordant nystagmus). This study aimed to determine the etiologies and mechanisms of horizontal and torsional nystagmus beating in the opposite directions (discordant nystagmus). We reviewed the medical records of 16 consecutive patients with discordant horizontal-torsional nystagmus who had been evaluated at the dizziness clinics of Seoul National University Bundang
