Video-assisted thoracoscopic surgery for ectopic mediastinal parathyroid tumor: subxiphoid and lateral thoracic approach.

BACKGROUND: This study aimed to investigate the initial results of an endoscopic surgical approach for the treatment of intramediastinal ectopic parathyroid adenoma and to evaluate the effectiveness of a single-incision resection using the subxiphoid approach. METHODS: Five cases of patients (1.89%) were diagnosed with ectopic mediastinal parathyroid tumor and underwent resection from 2008 to 2017 in Fujita Health University Hospital. They were retrospectively analyzed. RESULTS: Four patients underwent single-port mediastinal tumor resection using the subxiphoid approach and 1 patient underw

Postero-apical thoracic schwannoma with cervical extension resected by complete video-assisted thoracoscopic surgery.

Schwannomas or neurilemmomas are benign tumors developed from the peripheral nervous system. Complete video-assisted thoracic surgery (cVATS) has set itself over the years as the preferred approach for the removal of small mediastinal neurogenic tumors. However, in case of apical location, complete VATS seems challenging because of proximity with the subclavian artery and/or elements of the brachial plexus. In case of a cVATS procedure, some authors prefer enucleation instead of resection, with a higher risk of relapse. We present two cases of cVATS resection of thoracic apical schwannomas.

Single Versus Multi-Incisional Video-Assisted Thoracic Surgery: A Systematic Review and Meta-analysis.

BACKGROUND: Video-Assisted Thoracic Surgery (VATS) is conventionally performed through multiple small incisions (C-VATS). Recent studies have reported encouraging results with the single-incision VATS (S-VATS) over the conventional technique. However, these studies were either small in size, unfocused, nonuniform, retrospective, lacking follow-up information, or focused on pain. We aim to validate previously reported results in a single large meta-analysis, including only the best evidence studies available. METHODS: Systematic review of the PubMed archive was conducted to include only full E

Postoperative pain experiences in Chinese adult patients after thoracotomy and video-assisted thoracic surgery.

AIMS AND OBJECTIVES: To investigate the developing course of postoperative pain in a Chinese sample of patients following thoracic surgery. … nificant difference in pain was found between those undergoing a thoracotomy (61%) and those having video-assisted surgery (34%). Many patients whose immediate postoperative pain was poorly managed tended to report high pain levels after discharge and continued to have clinically important chronic pain 6 months after surgery. The pain intensity mean score during movemen …

Resection of pulmonary endometriosis using video-assisted thoracoscopic surgery under preoperative CT-guided marking.

Pulmonary endometriosis is a gynecological disorder in which endometrial tissue grows outside of the uterine cavity. Usually, the ectopic implants are located in the pelvis and manifest as dysmenorrhea, chronic pelvic pain, or infertility. Pulmonary endometriosis sometimes occurs in the pleurae and can result in catamenial pneumothorax; however, true pulmonary endometriosis, tissue growing in the lung itself, is rare. We report a 22-year-old patient with pulmonary endometriosis and catamenial hemoptysis. Pulmonary endometriosis was proved histologically and treated successfully by wedge resect

Bilateral video-assisted thoracoscopic surgery resection for multiple mediastinal myelolipoma: report of a case.

Myelolipoma in the mediastinum is an extremely rare entity. In this report, we present the case of a 79-year-old asymptomatic man who had three bilateral paravertebral mediastinal tumors. The three tumors were resected simultaneously using bilateral three-port video-assisted thoracoscopic surgery (VATS). There has been no evidence of recurrence within four years after the operation. Multiple bilateral mediastinal myelolipomas are extremely rare. There are no reports in the English literature of multiple bilateral thoracic myelolipomas that were resected simultaneously using bilateral VATS. We

Video-assisted thoracoscopic surgery for recurrent pneumothorax in pulmonary lymphangioleimyomatosis with tuberous sclerosis complex.

Pneumothorax in pulmonary lymphangioleiomyomatosis (LAM) with tuberous sclerosis complex (TSC) is a difficult condition to manage. Video-assisted thoracoscopic surgery (VATS) may play a role in diagnosis and treatment of this situation. We present a case of right recurrent pneumothorax due to LAM with TSC in whom VATS was performed for pathological diagnosis and mechanical pleurodesis. The unique presentation of LAM in TSC was also discussed.

Left main bronchus rupture during video-assisted thoracoscopic surgery resection of a bronchogenic cyst.

Video-assisted thoracoscopic surgery (VATS) is an approach to resection of bronchogenic cysts (BC) which is effective and viable. There may be complications as a result of pericystic adhesions to neighboring structures during dissection. This report describes an unusual case of an operative complication, the rupture of the main left bronchus. The initial VATS approach was converted into a right thoracotomy to remove the cyst and suture the bronchus once adequate exposure was obtained. Bronchus reconstruction failed and a few hours later, it was necessary to perform a left thoracotomy. A left p

Penetrating neck injury to the superior thoracic artery managed by video-assisted thoracoscopic surgery.

Penetrating trauma to the axillary artery and its branches is uncommon and associated with high morbidity and mortality. Open exploration is mandated in hemodynamically unstable patients, but surgical exposure can be difficult due to the concentration of vital structures and complex anatomy in this region. Computed tomographic angiography is a potential diagnostic modality in hemodynamically stable patients. In these patients, endovascular therapies may provide a feasible means of controlling hemorrhage while minimizing surgical complications. A high incidence of concomitant intrathoracic inju

[CT-guided hookwire localization of small solitary pulmonary nodules in video-assisted thoracoscopic surgery].

OBJECTIVE: Video-assisted thoracoscopic surgery (VATS) provides a minimally invasive approach to resect small solitary pulmonary nodules (SSPN). The aim of this study is to evaluate the efficacy and safety of preoperative CT-guided hookwire localization for SSPN in VATS. METHODS: Hookwire was used to localize 26 SSPN under CT guidance in 24 patients (14 male, 10 female, age range 21-61 years, mean 52.3 years), preoperatively, and wedge resection was performed through VATS. The lesion size, distance from the lesion to parietal pleura, the time of localization and complications were evaluated.

Simultaneous resection of bilateral intralobar and extralobar pulmonary sequestrations with video-assisted thoracoscopic surgery.

The term pulmonary sequestration is applied to a pulmonary lobe or portion of a lobe that is supplied by an anomalous systemic artery and drain either into the systemic or pulmonary veins. The conditions are divided into intralobar pulmonary sequestration, in which the sequestration is situated inside the visceral pleura of a normal lobe, and extralobar sequestration, in which the sequestration is surrounded by its own pleura. Most sequestrations are unilateral; bilateral sequestrations are rare. We report the case of a synchronous bilateral intralobar and extralobar pulmonary sequestrations r

Video-assisted thoracic surgery lobectomy with bronchoplasty for lung cancer: initial experience and techniques.

BACKGROUND: Many surgeons think video-assisted thoracic surgery is too complex to be applied to bronchoplasty; therefore, our institution tried to develop some safe and reliable techniques for video-assisted thoracic surgery bronchoplasty. METHODS: One hundred thirty-four patients with lung cancer underwent curative video-assisted thoracic surgery lobectomy including mediastinal dissection at Iizuka hospital between October 2001 and September 2006. Five of these patients underwent radical lung lobectomy with bronchoplasty using video-assisted thoracic surgery. A minithoracotomy was performed

The efficacy of epidural analgesia after video-assisted thoracoscopic surgery: a randomized control study.

PURPOSE: Video-assisted thoracoscopic surgery (VATS) is known to reduce the severity of pain after surgery. However, it has not yet been established whether epidural anesthesia/analgesia (EA) is necessary after VATS. We therefore conducted a randomized control study to examine whether or not EA is necessary for pain control after VATS. PATIENTS AND METHODS: Forty-six patients undergoing VATS were randomly allocated to one of 2 groups: 24 who were given EA after the procedure (EA group) and 22 who were not (NEA group). Patients in the EA group received a continuous infusion of fentanyl and bup

Incidence of chest wall paresthesia after needlescopic video-assisted thoracic surgery for palmar hyperhidrosis.

OBJECTIVE: Chest wall paresthesia is a reported sequela of thoracotomy and Video-Assisted Thoracic Surgery (VATS) which is distinct from wound pain. Although needlescopic VATS confers less post-operative pain and better cosmesis, the incidence of paresthesia after needlescopic VATS has not been quantified. METHODS: For homogeneity of the patient cohort, we studied 50 patients who received bilateral needlescopic VATS sympathectomy (T2-T4 excision) for palmar hyperhidrosis using 2 or 3 mm instruments during a 36-month period at a single institute. A standard questionnaire was administered by te

Differentiated operative strategy in minimally invasive, video-assisted thyroid surgery results in 196 patients.

To date, experience in minimally invasive thyroid surgery has been limited to unilateral lobectomy and total thyroidectomy. There are no reports regarding selective operative strategy, guided by morphology and function, which is widely accepted in endemic goiter regions. To analyze the efficiency and outcome of tissue-preserving thyroid surgery using a minimally invasive video-assisted technique (MIVA-T), a total of 196 patients were operated on for thyroid nodules between February 1999 and October 2003. Concurrent primary hyperthyroidism was treated in 22 (11%) cases. Indications for operatio

[Usefulness of video-assisted thoracic surgery techniques in intra-thoracic mass resection in children].

The progressive advances in Minimally Invasive Surgical Procedures, are applied presently to the surgical treatment of intra-thoracic injuries in children by way of Video-Assisted Thoracic Surgery (VATS) techniques. We present the complete findings of two intra-thoracic masses, in an infant and in a school-aged child, with the use of the VATS technique. The operative technique is detailed in each case. We conclude that the lack of invasiveness and cost-efficiency of VATS the technique, constitute it as being the first choice in the treatment of intra-thoracic masses in children.

Video-assisted thoracoscopic surgery of mediastinal bronchogenic cysts in adults: a single-center experience.

BACKGROUND: Mediastinal bronchogenic cysts are rarely diagnosed in adults, hence surgical experience is limited particularly with regard to video-assisted thoracoscopic surgery. In support of the thoracoscopic approach we report our single-center experience in this rare entity. METHODS: Between June 1995 and December 2002, a nonselected series of 12 consecutive patients presenting with mediastinal bronchogenic cysts underwent video-assisted thoracoscopic surgery. Six cysts (50%) had been diagnosed 2 to 22 years prior, only three of which became symptomatic. In asymptomatic patients (n = 7) su
