Pernicious anemia: new insights from a gastroenterological point of view.

Pernicious anemia (PA) is a macrocytic anemia that is caused by vitamin B(12) deficiency, as a result of intrinsic factor deficiency. PA is associated with atrophic body gastritis (ABG), whose diagnosis is based on histological confirmation of gastric body atrophy. Serological markers that suggest oxyntic mucosa damage are increased fasting gastrin and decreased pepsinogen I. Without performing Schilling's test, intrinsic factor deficiency may not be proven, and intrinsic factor and parietal cell antibodies are useful surrogate markers of PA, with 73% sensitivity and 100% specificity. PA is ma

Late-onset cobalamin C defect with a rare MMACHC c.457 C > T variant: a case report and literature review.

BACKGROUND: Cobalamin C (cblC) defect, also referred to as combined methylmalonic acidemia and homocystinuria, is the most common inborn error of intracellular cobalamin metabolism. Late-onset cblC defect often presents with atypical and heterogeneous manifestations, particularly neuropsychiatric symptoms, which may lead to misdiagnosis and delayed treatment. CASE PRESENTATION: We report a 13-year-old girl with late-onset cblC defect who presented with progressive cognitive impairment, psychiatric and behavioral abnormalities, and motor dysfunction. Because of these atypical manifestations, s

A child with cobalamin C deficiency caused by complex heterozygous variation of c.567dupT and c.80A > G complicated with pulmonary arterial hypertension and hydrocephalus: A case report and literature review.

RATIONALE: Cobalamin C (cblC) deficiency is one of the most common congenital vitamin B12 metabolic abnormalities, and may cause severe neurologic symptoms, gastrointestinal and nephritic symptoms. PATIENT CONCERNS: A 9-month-old boy presented with a 10-day history of progressive dyspnea and weak cough, accompanied by moaning, perioral cyanosis, and poor feeding. The parents also reported significant developmental regression and delay, characterized by an inability to raise his head, sit independently, or vocalize "dada" and "mama": milestones typically achieved by this age. DIAGNOSIS: The p

Acute reversible cognitive dysfunction due to vitamin B12 deficiency: A case report with rare magnetic resonance imaging findings.

Vitamin B12 deficiency can manifest with hematological and neurological abnormalities, though intracranial involvement is less common than spinal cord or peripheral nerve manifestations. We report the case of a man in his 70s who developed acute cognitive decline over 4 days, accompanied with left lower limb weakness and visual disturbances. Neurological examination revealed reduced verbal output, dysarthria, significant cognitive impairment (Mini-Mental State Examination score, 7/30), and left lower limb weakness (Medical Research Council grade, 4/5). Laboratory tests showed pancytopenia (whi

Addressing the Gaps in the Vitamin B12 Deficiency 2024 NICE Guidelines: Highlighting the Need for Better Recognition, Diagnosis, and Management of Pernicious Anaemia.

The 2024 NICE guidelines on vitamin B12 deficiency have significant implications for the diagnosis and management of pernicious anaemia (PA), the commonest non-dietary cause of such deficiency. This perspective discusses the guidelines in relation to PA itself, suggests that clearer diagnostic protocols are required, and calls for clinician education to improve the patient journey for those with PA.

Nitrous oxide induced subacute combined degeneration of the spine cord: A case report.

RATIONALE: In recent years, recreational use of inhaled nitrous oxide (N2O) is on the increase among young people, accompanied by a corresponding rise in reports about its toxicity. Subacute combined degeneration of the spine cord (SCD) is the typical clinical picture of the nervous system disorder caused by N2O intoxication, as a result of metabolic disturbance of vitamin B12. PATIENT CONCERNS, DIAGNOSES, INTERVENTIONS AND OUTCOMES: We report a 28-year-old female of SCD due to prolonged use of N2O, presented with paresthesia and unsteady in walking progressing within 1 month. Symptoms gradua

Clinical signs of nitrous oxide use: case report and review of the literature.

Nitrous oxide is used medically as an anesthetic agent; in the food industry as a propellant for condiments; and recreationally for its euphoric and dissociative effects. We report three cases of nitrous oxide misuse causing severe, symptomatic cobalamin (vitamin B12) deficiency in which signs of nitrous oxide use per se, as well as signs of toxicity, were observed, including characteristic palmar calluses over the metacarpal heads, and frostbite. These signs may assist clinicians in the recognition of nitrous oxide use and the timely diagnosis of nitrous oxide toxicity.

[Relationship between vitamin B12, iron, folic acid, homocystein and vitamin D3 serum levels in orofacial sicca and/or Sjögren's syndrome in a Hungarian patient population].

Introduction: Vitamins D3- and B12, folic acid and/or iron deficiency might cause orofacial sicca symptoms: dry mouth, migrant glossitis, burning mouth, and further associated orofacial symptoms including mucosal atrophy or inflammation. Vitamin deficiencies, on the other hand, might expand the xerostomia-related unpleasant conditions. Little is known about the serum levels of the above-mentioned laboratory parameters, and about the level of homocysteine in patients with orofacial sicca symptoms and/or Sjögren’s syndrome. It is known that patients with autoimmune diseases have a declined level

[Beta-thalassemia and vitamin B12 deficiency and associated complement-mediated hemolysis].

We present the case of a 67-year-old male patient admitted to our clinic due to weakness and repeated dizziness. Due to his severe microcytic anemia in his laboratory tests, he needed a transfusion of 6 units of selected blood in the days following admission. Our patient was diagnosed with beta-thalassemia minor, which was accompanied by a severe deficiency of vitamin B12. Surprisingly, parallel to vitamin B12 deficiency, we detected laboratory abnormalities indicating complement-mediated autoimmune hemolysis. After correcting the vitamin B12 deficiency, the patient's blood count improved, and

Vitamin B12 deficiency.

Of the water-soluble vitamins, vitamin B12 (B12) has the lowest daily requirement. It also has several unique properties including a complex pathway for its absorption and assimilation requiring intact gastric and terminal small intestinal function, an enterohepatic pathway, and several dedicated binding proteins and chaperons. The many causes of B12 deficiency include malabsorption and defects in cellular delivery and uptake, as well as limited dietary intake. B12 is required as a cofactor for only two reactions in humans, the cytosolic methionine synthase reaction and the mitochondrial methy

Vitamine B12 deficiency in children: a diagnostic challenge.

Cobalamin or vitamin B12 (vitB12) is involved in DNA synthesis, haematopoiesis and myelinisation. Consequently, vitB12 deficiency causes various symptoms, such as megaloblastic anaemia, neurologic signs or pancytopenia. Despite possible severe symptoms, vitB12 deficiency can present asymptomatically. We report six paediatric patients with different aetiologies of vitB12 deficiency ranging from a subtle to a more overt presentation. VitB12 deficiency is a diagnostic challenge due to the lack of consensus on normal values of vitB12 and its co-markers (folate, holotranscobalamin, methylmalonic ac

Pernicious anaemia: cause of recurrent cerebral venous thrombosis.

This case of recurrent cerebral venous thrombosis (CVT) highlights hyperhomocysteinemia in pernicious anemia due to vitamin B12 deficiency. No other risk factors such as trauma, infections, coagulation disorders or autoimmune diseases were present. The patient was cured with vitamin B12 and anticoagulation therapy. So, it is worthwhile and important to investigate levels of vitamin B12 and homocysteine in CVT.

Diagnostic Characteristics of 3-Parameter and 2-Parameter Equations for the Calculation of a Combined Indicator of Vitamin B12 Status to Predict Cobalamin Deficiency in a Large Mixed Patient Population.

BACKGROUND: A combined indicator for the determination of vitamin B12 status (4cB12) that employs four markers of vitamin B12 status (i.e., holotranscobalamin, HoloTC; vitamin B12, B12; methyl malonic acid, MMA; and homocysteine, Hcy) has been proposed for the comprehensive assessment of B12 status. We aimed to compare recently published 2- (2cB12) and 3-parameter (3cB12) cB12 equations missing one or two markers of B12 status with the established four-parameter cB12 (4cB12). METHODS: In 3,614 routine samples in which HoloTC, B12, MMA, Hcy and serum folate were measured, cB12 was assessed wit

Diaphragmatic palsy in a 10-month-old boy with infantile tremor syndrome causing respiratory failure with full response to vitamin B12 therapy.

Infantile tremor syndrome (ITS) owing to vitamin B12 deficiency usually presents with tremors, anaemia, pigmentary skin changes, neuro-regression and hypotonia. A 10-month-old boy with ITS and respiratory failure owing to bilateral diaphragmatic palsy who responded to high parenteral doses of vitamin B12 is presented. As far as we are aware, this is the first report of diaphragmatic palsy associated with ITS and vitamin B12 deficiency.

An Impaired Awareness of Hypoglycemia Improved After Vitamin B12 Treatment in a Type 1 Diabetic Patient.

An impaired awareness of hypoglycemia is a serious problem in diabetic patients, which can lead to life-threatening severe hypoglycemia. Recurrent hypoglycemia attenuates the function of the central, mainly hypothalamic, nervous system and it causes an impaired awareness of hypoglycemia. Vitamin B12 deficiency is also associated with the dysfunction of central nervous system. We report a 72-year-old type 1 diabetic patient with vitamin B12 deficiency whose impaired awareness of hypoglycemia improved after 4 weeks of vitamin B12 administration with an increased counter-hormone secretion in resp

Measurement of total Transcobalamin employing a commercially available assay for Active B12.

INTRODUCTION: Vitamin B12 deficiency is mostly caused by insufficient gastro-intestinal absorption and in rare conditions by Transcobalamin (TC) deficiency. Unsaturated Transcobalamin (apoTC) can be measured by a binding assay using radiolabeled cobalamin. The Active B12 test analyzes saturated Transcobalamin (holoTC) and we hypothesize that this test can be used to measure total TC by additional in vitro saturation with cobalamin. METHODS: Serum was saturated in vitro (16 times dilution) with a cyanocobalamin solution and total TC was selectively measured with the Abbott Active B12 test. Apo
