Surgical techniques for vitiligo: a review.

Although the treatment of vitiligo has improved during the last decade, therapy is still not satisfying for many patients. This is probably due to the fact that the aetiopathogenesis is unknown. Several treatment modalities, such as PUVA, UVB and local corticosteroids are currently used in the treatment of active vitiligo. However, these treatments usually induce incomplete repigmentation. Surgical methods intended to repigment leucoderma are an interesting therapeutic option if patients have stable disease. Two types of surgical techniques are available: tissue grafts and cellular grafts, wit…

Characteristics of vitiligo in Korean children.

Eighty Korean children (ages 8 months-12 years) with clinical and/or histopathologic diagnoses of vitiligo were evaluated; 39 boys and 41 girls. The mean age at first visit was 7.9 years and the mean age at onset was 5.6 years. The most common site of onset was the head/neck area (58.8%), followed by the trunk and lower limbs. The children were compared with a control group of 422 adults with vitiligo. Children comprised 16% of the total vitiligo patients and adults comprised 84%. A family history of vitiligo was found in 11 (13.8%) children, compared to 10.7% in the adult group; poliosis in 2…

Intrinsic and extrinsic pathomechanisms in vitiligo.

Vitiligo is the most commonly acquired hypomelanosis, and is restricted to a limited cutaneous territory (focal/segmental vitiligo [SV]) or generalized in symmetric patches (nonsegmental vitiligo [NSV]). In the majority of cases, vitiligo corresponds to a loss of melanocytes, first in the epidermal compartment, and later in the follicular reservoir where most melanocytic stem cells are probably situated. There are many data currently supporting an impaired redox status of the epidermal melanin unit as a primary defect leading to inappropriate immune responses in NSV. SV is probably a mosaic de…

Effectiveness of psoralen photochemotherapy for vitiligo.

To evaluate the response to oral psoralen ultraviolet-A (PUVA) photochemotherapy in patients with vitiligo in Taiwan, we retrospectively reviewed the clinical data of 21 vitiligo patients treated from 1990 to 1998. Diagnosis included generalized vitiligo in 16 patients, focal vitiligo in three, and acrofacial vitiligo in two. All patients were treated two to three times per week over a period of 3 to 19 months with 0.2 to 0.4 mg/kg of trioxsalen 2 hours before exposure to long-wave ultraviolet light irradiation. The results for PUVA therapy showed 10 patients (48%) had an excellent response (7…

Pattern of familial aggregation of vitiligo.

BACKGROUND AND DESIGN: Vitiligo is a disorder whose cause is not well understood. This study was undertaken to clarify whether genetic factors are involved in the pathogenesis of vitiligo. Data on 160 white kindreds living in the United States have been collected. Each family was ascertained through a proband afflicted with vitiligo. The nature and extent of familial aggregation and other relevant epidemiologic features have been determined. RESULTS: The mean age at onset of vitiligo is about 19 years in male and about 24 years in female individuals. The percentage of probands reporting one o…

Association of Pemphigus Vulgaris with Vitiligo.

A patient had vitiligo for 10 years with development of new lesions for the last. Four months. Concomitant with the recent activation of vitiligo, he developed pemphigus vulgarism. Corticosteiroid therapy controlled pemphigus vulgaris, and also halted the development of new vitiligo lesions. Histopathologic study revealed inflammatory infiltrate at the dermo-epidermal junction along with basal cell layer dissolution suggesting atuoimmune, origin of vitiligo.

The Neuro-Endocrinal Regulation in Vitiligo.

Vitiligo is an acquired depigmentation disorder characterized by the loss of melanocytes. The specific etiology of vitiligo is not fully understood, but it is thought to result from a complex interplay of factors, including autoimmune responses, intrinsic melanocyte dysfunction, genetic susceptibility, oxidative stress, and neurogenic imbalances. The disfiguring nature of vitiligo significantly impacts the mental and physical health of patients, and psychological stress can further induce and exacerbate vitiligo. Recent research has underscored the potential mediating roles of the hypothalamic…

Audiological Profile of Patients with Vitiligo.

INTRODUCTION: Vitiligo is one of the most common causes of hypopigmentation; it occurs when melanin production is impaired due to the loss of melanocytes, which are present throughout the skin, in the epidermis, and in hair follicles, and are also seen in the mucosa, uvea, and several structures of the inner ear. Scientists have investigated the association between vitiligo and sensorineural hearing loss (SNHL); some believe there is a connection, while others disagree. OBJECTIVE: To evaluate the relationship between vitiligo and SNHL. METHODS: The present observational study was conducted f…

JAK inhibitors for the treatment of vitiligo.

Vitiligo is an autoimmune disease involving melanocyte-targeting T cells initiated by environmental and genetic factors. Steroids and tacrolimus have been used as topical treatments. Recently, novel topical agents targeting Janus kinase (JAK), a family of tyrosine kinases that regulates cytokine signaling, have emerged. Ruxolitinib is the first approved in vitiligo therapy. Furthermore, ritlecitinib is currently under clinical trials for oral treatment of active vitiligo. In this review, we discuss the possibility of topical JAK inhibitors as promising options for the treatment of vitiligo wit…

An Overview of the Biological Complexity of Vitiligo.

Vitiligo is a skin disease that affects all ethnicities and genders and is characterized by the loss of pigment essentially due to the selective loss of melanocytes. Although it is generally considered a systemic disease associated with polymorphisms in genes involved in the immune response, vitiligo is also considered an oxidative imbalance-associated disease. It represents a multifactorial pathology in which some genetic predisposition and epigenetic factors coupled with some critical biochemical and molecular pathways could play a pivotal role. The aim of this work was thus to review some o…

Camouflage of Vitiligo by Medical Tattooing.

Vitiligo is an acquired skin depigmentation disorder that affects 0.5-2% of the world population. It is characterized by loss of the natural brown melanin pigment of the skin clinically manifested as few or many white patches on the skin and microscopically with the total absence of me-lanocytes in the epidermis. The change in appearance caused by vitiligo can affect persons' emotional and psychological well-being and may cause them to alter their lifestyle. The social complication of vitiligo depends on ethnicity and on geography and local opinion, which may deem vitiligo contagious. The aim …

Psychosocial considerations in the management of vitiligo.

Vitiligo is a depigmenting skin disease that can carry significant psychosocial impact. Health care providers play a crucial role in shaping patients' understanding of their condition, approach to treatment, and coping mechanisms. In this contribution, we review the psychosocial aspects of vitiligo management, including the debate over the "disease-ification" of vitiligo, its effects on quality of life and mental health, and methods to holistically assist affected individuals beyond just treatment of the vitiligo itself.

Trained immunity in the pathogenesis of vitiligo.

Vitiligo is caused by an autoimmune reaction against melanocytes leading to melanocyte loss. The cause of vitiligo is an interaction between genetic susceptibility and environmental factors. Both the adaptive immune system-through cytotoxic CD8+ T cells and melanocyte specific antibodies-and the innate immune system are involved in these immune processes in vitiligo. While recent data stressed the importance of innate immunity in vitiligo, the question remains why vitiligo patients' immune response becomes overly activated. Could a long-term increase in innate memory function, described as tra…
