Gadolinium enhancement of the leptomeninges caused by hydrocephalus: a potential mimic of leptomeningeal metastasis.

A patient had severe hydrocephalus and diffuse leptomeningeal enhancement on MR which mimicked leptomeningeal spread of a primary brain tumor. The leptomeningeal enhancement resolved completely after decompression of the hydrocephalus. Data suggest that the leptomeningeal enhancement is caused by vascular stasis induced by the hydrocephalus.

Diagnosis and treatment of leptomeningeal metastases from solid tumors: experience with 90 patients.

The clinical findings and response to treatment of leptomeningeal metastases from solid tumors are analyzed in 90 patients treated at Memorial Sloan-Kettering Cancer Center during the period from January 1975 to February 1980. Patients included those who had either typical clinical findings of leptomeningeal tumor or conclusive laboratory evidence supporting the diagnosis. Carcinoma of the breast (46 patients), lung (23 patients) and melanoma (11 patients) were the common primary tumors. Symptoms of leptomeningeal metastasis occurred as the presenting sign in five patients and as late as ten y

Giant intradiploic pseudomeningocele of occipital bone.

The management of intradiploic CSF collection is controversial. Although it is a benign lesion, even then delay in diagnosis and treatment may lead to significant morbidity. The authors report a very rare case of giant posttraumatic intradiploic pseudomeningocele involving the occipital bone, occipital condyles, and clivus. The pathogenesis and management of intradiploic CSF collection are discussed. This 16-year-old boy presented with a history of enlarging swelling in the suboccipital region associated with headache, lower cranial nerve palsy, and features of high cervical compressive myelop

Anatomical restrictions in the transsphenoidal, transclival approach to the upper clival region: a cadaveric, anatomic study.

OBJECTIVE: Tumours in the clival region are difficult to remove surgically. Before the 1970s, clival tumours had very high mortality and morbidity rates. METHODS: An anatomic dissection was performed on 24 spheno-occipital bone blocks obtained from 28 adult cadavers. The internal carotid artery, paraclival carotid tubercle, sixth cranial nerve and dorsum sellae in the upper clival region were analyzed qualitatively and quantitatively. For the histological evaluation, 4 samples were decalcified and sagittal sections were cut. From the eight blocks obtained, 32 incisions were made in the axial

Intraparenchymal meningioma.

A 54 year-old woman presented with severe headache. Neuroimaging showed an enhancing intra-axial mass in the right superior temporal region associated with severe peritumoral oedema. Preliminary diagnosis was cerebral metastasis. The mass was surgically removed. Histopathology and immunohistochemistry showed the mass to be an intraparenchymal meningioma, which is rare. The relevant literature is reviewed.

[Echographic differential diagnosis of optic nerve widening].

INTRODUCTION: Standardized echography is a well-established examination technique in ophthalmology. Among other things one of the more important uses is the evaluation of the optic nerve and its abnormalities. One aspect is the use of the stretch-test (originally called the "30 degrees-test") for the differential diagnosis of optic nerve widening. Such widening can be caused by solid thickening of the optic nerve and/or its sheaths or by solid infiltration of the subarachnoidal space. More frequent is a widening of the optic nerve as a consequence of sheath distension by increased subarachnoid

Severe hypersomnolence after pituitary/hypothalamic surgery in adolescents: clinical characteristics and potential mechanisms.

OBJECTIVES: After resection of hypothalamic/pituitary tumors, children are at risk for development of hormonal deficiencies, obesity, and hypersomnolence. However, the prevalence and pathophysiology of these complications are unclear. The purpose of this study was to assess the prevalence and severity of hypersomnolence in children after resection of pituitary tumors and to study the potential factors that contribute to this sleepiness if present. We further hypothesized that decrements in orexin levels may contribute to the sleepiness. METHODS: Six children who underwent hypothalamic/pituita

[Giant subarachnoid cysts due to neurocysticercosis: two cases reported fro a rural area in Extremadura].

INTRODUCTION: Neurocysticercosis is the commonest cerebral parasitosis. The most recent cases reported in Spain have occurred in patients from endemic countries. The formation of giant subarachnoid cysts (GSC) is an uncommon complication of neurocysticercosis and its treatment is the subject of debate. We report two cases seen recently in the Valle del Jerte, a rural district in Extremadura. CLINICAL CASES: Case 1. A 64 year old man had had complex partial crises (CPC) since 1985. In 1987 he was diagnosed as having neurocysticercosis with parenchymatous cysts and treated with praziquantel. In

Clinical application of neuroendoscopic techniques.

We treated 126 patients with different neurosurgical diseases by performing endoscopic neurosurgery, endoscopy-controlled microneurosurgery and endoscopy-assisted microneurosurgery. The indications were intracranial cysts in 51 patients, brain cysticercosis in 10 patients, hydrocephalus in 31 patients, and epidermoid cysts in 34 patients. The follow-up period ranged from 6 to 24 months (mean follow-up duration 9 months). After operation, 115 of 126 patients had improvement in their initial symptoms, 7 had no change and 4 developed complications, including subarachnoid hemorrhage (SAH), intrave

Neurocutaneous melanosis: clinical features of large congenital melanocytic nevi in patients with manifest central nervous system melanosis.

BACKGROUND: Patients with a large congenital melanocytic nevus (LCMN) may have associated leptomeningeal melanocytosis with or without central nervous system (CNS) melanomas. These patients are considered to have neurocutaneous melanosis, a disorder that, when symptomatic or otherwise manifest neurologically, carries a poor prognosis even in the absence of malignancy. OBJECTIVE: Our purpose was to identify typical clinical features in patients who have manifest CNS melanosis in association with LCMN. METHODS: The records of 117 patients with LCMN in the New York University Registry of LCMN a

Magnetic resonance imaging of the brain in Aicardi's syndrome. Report of 20 patients.

Aicardi's syndrome consists of multiple anomalies of the eye, brain, and axial skeleton in females, and is associated with early-onset infantile spasms and severe developmental delay. For the present study, magnetic resonance images of 20 patients with Aicardi's syndrome were obtained and the neuroimaging findings catalogued. These were compared to previously reported results and the findings correlated with current theories of fetal brain development. The results revealed a high frequency of cortical migration defects (94%) and a wide variability in the callosal defect, migrational abnormalit

Clinical spectrum of heterozygous LZTR1 variation in a schwannomatosis-clinic cohort: A descriptive study of multisystem features.

BACKGROUND: LZTR1-related disorders (LRDs) include schwannomatosis, Noonan syndrome, and variably expressed multisystem phenotypes, but the full clinical spectrum remains incompletely characterized. We evaluated whether adults carrying pathogenic or likely pathogenic LZTR1 variants show recurrent extracranial and central nervous system findings, while separately presenting descriptive findings from candidate VUS carriers and accounting for the limits of specialty-clinic ascertainment. METHODS: We examined adults assessed at comprehensive schwannomatosis clinics (n = 156), including a genetica

Microsurgical resection of skull base meningioma Operative Nuances, Challenges, and Outcome.

OBJECTIVES: Skull base meningiomas represent a distinct subgroup of intracranial tumours because of their proximity to cranial nerves, major vascular structures, and the brainstem. Although advances in imaging, neuroanaesthesia, and skull base approaches, marked improvement in illumination devices, provision of improved haemostatic agents, greater availability of more precise surgical instruments, and better modalities for skull base reconstruction have led to an inevitable evolution of safe skull base neurosurgery, complete resection remains technically demanding. This study evaluates operati

Integration of intraoperative ultrasound and depth-electrode electrocorticography for resection guidance in epilepsy surgery: technical workflow and feasibility.

BACKGROUND: Complete resection of the epileptogenic zone (EZ) is the strongest predictor of seizure freedom in drug-resistant epilepsy (DRE). However, even in MRI-positive cases with anatomo-electro-clinical concordance, the EZ may not be clearly delineated, complicating intraoperative decision-making. Intraoperative ultrasound (ioUS) provides real-time anatomical feedback, while depth-electrode intraoperative electrocorticography (iECoG) enables electrophysiological delineation of epileptogenic tissue beyond the cortical surface, sampling deep regions not accessible to subdural electrodes. Machine learning-based prediction of postoperative nausea and vomiting after spinal anesthesia: A retrospective observational study.

Postoperative nausea and vomiting (PONV) is a frequent and serious complication after surgery. PONV also reduces patient satisfaction with surgery under spinal anesthesia and increases medical costs due to prolonged hospitalization. The purpose of this study is to apply artificial intelligence (AI) machine learning analysis to identify risk factors for PONV in patients undergoing surgery with spinal anesthesia. This retrospective study used artificial intelligence to analyze data of adult patients (aged ≥20 years) who underwent surgery under spinal anesthesia at Tohoku University Hospital from

The fetal taenia-tela choroidea complex: a radiologic-pathologic correlation from normal anatomy to Dandy-Walker malformation.

BACKGROUND: The taenia-tela choroidea-choroid plexus complex has been proposed as an imaging marker to differentiate Dandy-Walker malformation from Blake's pouch cyst on prenatal MRI. However, radiologic-pathologic correlation remains limited. OBJECTIVE: To characterize the taenia-tela choroidea-choroid plexus complex in normal and Dandy-Walker malformation fetuses and assess its visibility on routine prenatal MRI. MATERIALS AND METHODS: Nine fetal autopsy specimens with Dandy-Walker malformation and eight gestational age-matched controls with normal posterior fossa were reviewed for taenia-
