How to manage postmenopausal osteoporosis?

Osteoporosis is characterized by reduced bone mass and disruption of bone architecture, resulting in increased fracture risk. Several therapeutic agents are now available to treat postmenopausal osteoporosis and prevent fractures. Combined calcium and vitamin D supplementation reduce the relative risk of non-vertebral fractures by about 18%. Hormone replacement therapy (HRT) should not be prescribed for osteoporosis in women who do not experience menopausal symptoms. The marked benefits of raloxifene on the reduction in invasive breast cancer and vertebral fracture risk are partially counterba

The use of combination therapy in the treatment of postmenopausal osteoporosis.

In recent years, there has been growing interest in the potential use of combination therapy in the management of osteoporosis in postmenopausal women. Possible regimens include sequential or combined use of anti-resorptive drugs or combinations of anabolic and anti-resorptive agents, given concurrently or in sequence. Combined therapy with anti-resorptive drugs usually produces greater increases in bone mineral density (BMD) than monotherapy but there is no evidence that this results in greater anti-fracture efficacy. The use of bisphosphonates before strontium ranelate or PTH peptides blunts

Association between bisphosphonate use and implant survival after primary total arthroplasty of the knee or hip: population based retrospective cohort study.

OBJECTIVES: To test whether bisphosphonate use is related to improved implant survival after total arthroplasty of the knee or hip. DESIGN: Population based retrospective cohort study. SETTING: Primary care data from the United Kingdom. PARTICIPANTS: All patients undergoing primary total arthroplasty of the knee (n = 18,726) or hip (n = 23,269) in 1986-2006 within the United Kingdom's General Practice Research Database. We excluded patients with a history of hip fracture before surgery or rheumatoid arthritis, and individuals younger than 40 years at surgery. INTERVENTION: Bisphosphonate u

Zoledronic acid and atrial fibrillation in cancer patients.

BACKGROUND AND PURPOSE: Treatment with a bisphosphonate was found to be associated with a significantly increased risk for atrial fibrillation (AF) in a few studies. A recent study showed that once-yearly infusions of intravenous zoledronic acid (ZA) significantly increased the risk of serious AF in postmenopausal women with osteoporosis. This study was conducted to determine the frequency of atrial fibrillation among cancer patients receiving the standard treatment of ZA. METHODS: Patients with bone metastases who presented to our outpatient clinic for any reason (routine control, chemothera

Bone pain reduction in patients with metastatic breast cancer treated with ibandronate-results from a post-marketing surveillance study.

BACKGROUND: Pain relief is an important treatment goal for breast cancer patients with metastatic bone disease and treatment should be associated with a low rate of side effects. This interim analysis of a prospective non-interventional study documents the efficacy and safety of the amino-bisphosphonate ibandronate in the treatment of metastatic bone disease under real-life conditions. PATIENTS AND METHODS: For up to 24 weeks 913 breast cancer patients received IV infusions of 6 mg ibandronate every 3-4 weeks or 50 mg of oral ibandronate once daily. Efficacy variables included pain severity,

Bone mineral density and fracture rate in response to intravenous and oral bisphosphonates in adult osteogenesis imperfecta.

The effect of bisphosphonate treatment on bone mineral density (BMD) and fracture rates was assessed in adults with osteogenesis imperfecta (OI). This observational nonrandomized study included 90 OI adults treated with intravenous pamidronate (n = 28), oral alendronate (n = 10), or oral residronate (n = 17) or not treated (n = 35). There were 63 type I, 15 type III, and 12 type IV OI patients. BMD results were observed for up to 161 months and an average of 52 months of treatment. For type I and grouped type III/IV patients, treatment with pamidronate showed an increasing rate in L1-L4 BMD fr

Physician perception of patient adherence compared to patient adherence of osteoporosis medications from pharmacy claims.

OBJECTIVE: This study explored physicians' perceptions of patient adherence to medications compared with patient adherence derived by administrative data in the treatment of osteoporosis. RESEARCH DESIGN AND METHODS: A study involving written questionnaires from prescribers treating patients with postmenopausal osteoporosis (PMO) compared the questionnaire responses to pharmacy claims of these prescribers' patients' refill patterns. Approximately 2000 physicians from a large US health plan were faxed or mailed a survey. Data from the physician survey were merged with administrative claims dat

[Effects of SERMs on bone health. Discrimination of SERMs from bisphosphonates in the treatment of postmenopausal osteoporosis].

Since the aim of the treatment of osteoporosis is to prevent fractures, the first-line drugs must be chosen according to their antifracture efficacies. In postmenopausal women with osteoporosis, alendronate, risedronate, minodronate, and raloxifene are effective in preventing vertebral fractures, and alendronate and risedronate significantly prevent hip fractures. Because the antifracture efficacy of raloxifene against vertebral fractures is considered similar to that of bisphosphonates, the discrimination of raloxifene from bisphosphonates depends on the strategy of doctors (internal medicine

Skull base osteomyelitis and bisphosphonate use in multiple myeloma: report of two cases and literature review.

OBJECTIVES: Although osteoradionecrosis of the mandible is a well known entity, skull base osteomyelitis involving the temporal bone unrelated to otitis externa in patients with multiple myeloma using bisphosphonates has not been reported. We described a new entity of skull base erosion resulting from osteonecrosis, presenting with malignant features in patients using bisphosphonates for multiple myeloma. METHODS: Two patients with multiple myeloma and prior zolendronic acid use were referred to our cancer center for management of maxillary and temporal bone masses with skull base erosion. Bo

Jaw osteonecrosis in patients treated with bisphosphonates: an ultrastructural study.

Osteonecrosis of the jaw is a severe bone disorder traditionally associated with periodontal disease, local malignancy, chemotherapy, glucocorticoid therapy, or trauma. Recently a growing number of publications reported the occurrence of osteonecrosis of the jaw in patients undergoing treatment with bisphosphonates. The mechanism by which bisphosphonates might contribute to the development of osteonecrosis of the jaw is far from being fully elucidated. Suppression of bone turnover, infection, tissue hypoxia and cellular toxicity were proposed as possible mechanisms by which bisphosphonates may

Targeted therapy options for treatment of bone metastases; beyond bisphosphonates.

Cancer is a major leading cause of death in the western world (following heart diseases). It poses an enormous burden on patients and society with a major impact on healthcare and economy. Once cancers have spread to the skeleton, treatment options are predominantly limited to palliation, treatment of hypercalcemia and prevention of pathological fractures. Despite the elaborate efforts of modern medicine to improve treatment, novel therapies for the treatment of solid tumors in patients with advanced disease, including metastatic bone disease, have generally failed to improve patient overall s

[Secondary osteoporosis UPDATE. Therapy for bone-mineral disease in CKD-5D patients].

Recently, much of the attention given to bone-mineral disorder as the prognostic factor for mortality has focused on their links to chronic kidney disease (CKD-MBD; chronic kidney disease-mineral bone disease), especially in dialysis patients. Bone disease in dialysis patients showed heterogeneity caused by multiple factors other than postmenopausal osteoporosis. Evaluation of the bone mineral density with DEXA and the bone metabolic markers becomes useless for the assessment of bone fragility in dialysis patients. Prevalence of bone fracture in end-stage renal disease patients is 3-4 times hi

Bisphosphonate use and risk of post-operative fracture among patients undergoing a total knee replacement for knee osteoarthritis: a propensity score analysis.

SUMMARY: We have shown that patients with osteoarthritis are at increased risk of fracture after total knee replacement (TKR). We conducted a population-based cohort study to assess the effect of bisphosphonate use on their post-surgery fracture risk. Cox regression adjusted by propensity score suggested a 50-55% reduction in risk of fracture post-surgery. INTRODUCTION: Patients with osteoarthritis have a higher bone mass but similar or higher risk of fracture. We recently demonstrated that patients have an elevated fracture risk after TKR, but it is unknown if bisphosphonate therapy in this

The effect of zoledronic acid on the intrinsic material properties of healing bone: an indentation study.

BACKGROUND: Bisphosphonates are commonly used for the treatment of osteoporosis and have recently been shown to increase bone mineral parameters and strength in endochondral fracture repair. There is concern, however, that BPs may negatively affect bone material properties. METHODS: Nanoindentation was performed on femoral fracture samples of rats that had undergone closed fracture healing for six weeks to determine hardness and elastic modulus. The rats had received either intravenous saline or a single intravenous dose of zoledronic acid at zero, one or two weeks post fracture (n = 3 per gr

Bolus or weekly zoledronic acid administration does not delay endochondral fracture repair but weekly dosing enhances delays in hard callus remodeling.

INTRODUCTION: It has been widely assumed that osteoclasts play a pivotal role during the entire process of fracture healing. Bisphosphonates (BPs) are anti-catabolic agents commonly used to treat metabolic bone diseases including osteoporosis, minimizing fracture incidence. Yet, fractures do occur in these patients and the potential for negative effects of BPs on healing has been suggested. We aimed to examine the effect of different dosing regimes of the potent BP zoledronic acid (ZA) on early endochondral fracture repair and later callus remodeling in a normal bone healing environment. Maximizing effectiveness of bisphosphonate therapy for osteoporosis.

While highly prevalent, osteoporosis is greatly underdiagnosed and undertreated in clinical practice. Even when appropriate treatments are prescribed, patient adherence to bisphosphonate therapy is low. As osteoporosis is a silent disease, and therapy is required for many months before benefit is realized, strategies to increase bone mineral density (BMD) and improve medication adherence are important aspects of every patient's care plan. Osteoporotic bone loss occurs without symptoms, and there are often no warning signs before a fracture occurs. Osteoporotic fractures are the most severe con

Rapid suppression of bone resorption marker levels with ibandronate therapy in a bisphosphonate-naïve population.

This 6-mo open-label study assessed the pattern of bone turnover marker (BTM) level changes in bisphosphonate-naïve women with postmenopausal osteoporosis treated with monthly oral ibandronate 150mg and the correlation between month 1 and month 6 serum C-terminal cross-linking telopeptide of type I collagen (sCTX) levels. The following BTMs were monitored: sCTX, urinary N-terminal telopeptide of type I collagen (uNTX), serum procollagen type 1 N-terminal propeptide (PINP), serum osteocalcin (OC), and serum bone-specific alkaline phosphatase (bALP). BTM levels were measured immediately before d
