Computer modeling to predict effects of implant malpositioning during TKA.

Computer-assisted surgery has focused on alignment of implants and soft tissue balancing but the end results during surgery as they pertain to long-term function of the implants are not yet realized. One parameter that continues to be variable despite the use of computer navigation is the placement of the implants in the transverse plane. The objective of this study was to determine whether implant and anatomic registrations could be used in a computer model (LifeMOD/KneeSIM, LifeModeler, Inc., San Clemente, California) to show differences in the resulting contact patterns of the medial and la

Is the amino acid pattern in medical nutrition therapy crucial for successfully attenuating muscle mass loss in adult ICU patients? Secondary analysis of a RCT.

BACKGROUND AND AIMS: We hypothesized that in long-term immobilized intensive care unit (ICU) patients, both the quantity and quality of prot … of individual amino acids and the sum scores on the extent of skeletal muscle loss by measuring the quadriceps muscle layer thickness during the study period. The significance level was adjusted for multiple testing according to the Bonferroni procedure (α = 0.002). RESULTS: In both groups, the proportion of indispensable amino acids was approximately 41% of the t …

Effects of an eccentric training personalized by a low rate of perceived exertion on the maximal capacities in chronic heart failure: a randomized controlled trial.

BACKGROUND: The eccentric (ECC) training, in spite of its potential interest (slightest request of the cardiorespiratory system) compared with the concentric (CON) training, is not applied during the rehabilitation of the chronic heart failure (CHF). The main reasons are the difficulty personalizing the ECC exercises by avoiding the muscle complications and the lack of information concerning the specific effects on the maximal capacities in CHF patients. AIM: To compare - following a prior study on the feasibility and on the functional impacts - the effects on maximal capacities and tolerance

Successful feed-forward strategies following ACL injury and reconstruction.

The purpose of this study was to elucidate the most successful feed-forward strategies responsible for enhancing dynamic restraint following anterior cruciate ligament (ACL) injury and ACL reconstruction (ACLR). Ten male ACL deficient (ACLD) subjects (18-35 years) together with 27 matched males who had undergone ACLR (14 using a patella tendon graft and 13 using a combined semitendinosus and gracilis graft) and 22 matched-control subjects were recruited. After their knee functionality (0- to 100-point scale) was rated using the Cincinnati Knee Rating System, each subject performed a maximal, c

Muscle recovery at 1 year after the anterior cruciate ligament reconstruction surgery is associated with preoperative and early postoperative muscular strength of the knee extension.

BACKGROUND: The anterior cruciate ligament (ACL) injury often occurs in young athletes, but it also occurs in middle-aged individuals and the elderly during recreational sports activities. Clinical outcomes after the ACL reconstruction depend on postoperative recovery of muscular strength. The current study aimed to evaluate the recovery of knee extension and flexion strength after ACL reconstruction surgery and to examine the relationship between preoperative and postoperative muscle strength by age and the type of graft used. METHODS: From 2007 to 2010, 32 patients (17 men, 15 women; averag

Decision to Return to Sport Participation After Anterior Cruciate Ligament Reconstruction, Part II: Self-Reported and Functional Performance Outcomes.

CONTEXT: Anterior cruciate ligament (ACL) reconstruction (ACLR) is the most commonly used method for helping athletes regain function and return to preinjury activity levels after ACL injury. Outcomes after ACLR have suggested that athletes return to a level of function that would support a return to sport participation; however, in a recent meta-analysis, pooled return rates were only 55%. It is unclear whether this discrepancy is a result of functional impairments. OBJECTIVE: To compare patient-reported outcomes (PROs), dynamic balance, dynamic functional performance, strength, and muscular

Effects of pre-surgery physiotherapy on walking ability and lower extremity strength in patients with degenerative lumbar spine disorder: Secondary outcomes of the PREPARE randomised controlled trial.

BACKGROUND: Degenerative lumbar spine disorders are common among musculoskeletal disorders. When disabling pain and radiculopathy persists after adequate course of rehabilitation and imaging confirms compressive pathology, surgical decompression is indicated. Prehabilitation aiming to augment functional capacity pre-surgery may improve physical function and activity levels pre and post-surgery. This study aims to evaluate the effect and dose-response of pre-surgery physiotherapy on quadriceps femoris strength and walking ability in patients with degenerative lumbar spine disorders compared to

Multifocal tDCS targeting lower-limb cortical areas preserves late-stage endurance and tunes phase-specific coordination during incremental cycling.

BACKGROUND: Multifocal transcranial direct current stimulation (m-tDCS) may modulate distributed motor networks in a polarity-dependent and task-state-dependent manner to support performance near exhaustion. To this end, this study aims to test whether m-tDCS targeting lower-limb-specific cortical areas could optimize late-stage performance and phase-specific muscle coordination during cycling. METHODS: Two independent trials were conducted: (i) a tolerability assessment (Trial 1) and (ii) a randomized, double-blind, sham-controlled parallel study (Trial 2). In Trial 1, participants completed

Effects on Exercise Tolerance and Functional Outcomes of Eccentric versus Concentric Aerobic Exercise in People with COPD: A Systematic Review and Meta-Analysis.

INTRODUCTION AND OBJECTIVES: Chronic obstructive pulmonary disease (COPD) is associated with peripheral muscle weakness, dyspnoea and decreased exercise tolerance. Eccentric aerobic exercise (ECC), characterised by contractions during muscle stretching, is becoming an alternative to concentric aerobic exercise (CON) in this population, due to its reduced cardiorespiratory demand. The main objective of this systematic review, following the PRISMA criteria, is to determine the benefits of ECC versus CON on exercise tolerance in people with COPD. METHODS: A literature search was conducted in Pub

Effects of isoinertial training on muscle power, endurance, isometric strength, and balance: randomized clinical trial in patients with post-ACL reconstruction.

BACKGROUND: ACL reconstruction surgery leads to temporary limitations in knee movement due to weakness in the quadriceps, swelling, and stiffness. Effective therapy is necessary to regain strength and functionality. While flywheel resistance training enhances strength and eccentric loading, there is limited knowledge about its effectiveness in conjunction with conventional rehabilitation methods. This study evaluates the impact of both conventional rehabilitation and isoinertial therapy on muscle power, endurance, isometric strength, and balance in patients following ACL repair. METHODS: A to

Fibromyalgia: Associations Between Fat Infiltration, Physical Capacity, and Clinical Variables.

BACKGROUND: Obesity is a risk factor for the development of fibromyalgia (FM) and generally most studies report increased Body Mass Index (BMI) in FM. Obesity in FM is associated with a worse clinical presentation. FM patients have low physical conditioning and obesity further exacerbates these aspects. Hitherto studies of FM have focused upon a surrogate for overall measure of fat content, ie, BMI. This study is motivated by that ectopic fat and adipose tissues are rarely investigated in FM including their relationships to physical capacity variables. Moreover, their relationships to clinical

The effect of gracilis tendon harvesting in addition to semitendinosus tendon harvesting on knee extensor and flexor strength after anterior cruciate ligament reconstruction.

INTRODUCTION: Anterior cruciate ligament (ACL) reconstruction using hamstring tendons may involve harvesting of the gracilis tendon in addition to the semitendinosus tendon (ST) depending on the size of the ST graft. However, the effect of gracilis harvesting in addition to ST harvesting on muscle strength, such as the hamstring-to-quadriceps (HQ) ratio, remains unclear. Hence, this study aimed to investigate the effect of gracilis harvesting on subsequent knee muscle strength. MATERIALS AND METHODS: Eighty-two patients who underwent ACL reconstruction were included in this retrospective stud
