First United Kingdom experience of the novel Osia active transcutaneous piezoelectric bone conduction implant.

PURPOSE: Bone conduction hearing devices are widely used and indicated in cases of conductive, mixed or single-sided deafness where the conventional hearing aids are not indicated or tolerated. This prospective study aims to investigate the surgical and hearing outcomes of a novel active piezoelectric transcutaneous bone conduction device (t-BCD). METHODS: Prospective data were collected from the first 10 patients who underwent implantation with the t-BCD Osia (Cochlear, Australia) (between Dec 2018 and March 2019) in a tertiary referral centre. The main outcome measures include: surgical out

The feasibility of an encapsulated cell approach in an animal deafness model.

For patients with profound hearing loss a cochlear implant (CI) is the only treatment today. The function of a CI depends in part of the function and survival of the remaining spiral ganglion neurons (SGN). It is well known from animal models that inner ear infusion of neurotrophic factors prevents SGN degeneration and maintains electrical responsiveness in deafened animals. The purpose with this study was to investigate the effects of a novel encapsulated cell (EC) device releasing neurotrophic factors in the deafened guinea pig. The results showed that an EC device releasing glial cell line-

Auditory Brainstem Implantation for Adults With Neurofibromatosis 2 or Severe Inner Ear Abnormalities: A Health Technology Assessment.

BACKGROUND: Neurofibromatosis 2 (NF2) is a rare genetic disorder that causes vestibular schwannomas to develop in both eighth cranial nerves. Almost all people with NF2 eventually become completely deaf as a result of progressive tumour enlargement or following surgical or radiotherapy treatment. Other rare abnormal conditions in the inner ears can also cause complete deafness. For people with either indication who are not candidates for cochlear implantation, auditory brainstem implantation is the only treatment option to restore some functional hearing. We conducted a health technology asses…

Assessing temporal modulation sensitivity using electrically evoked auditory steady state responses.

Temporal cues are important for cochlear implant (CI) users when listening to speech. Users with greater sensitivity to temporal modulations show better speech recognition and modifications to stimulation parameters based on modulation sensitivity have resulted in improved speech understanding. Behavioural measures of temporal sensitivity require cooperative participants and a large amount of time. These limitations have motivated the desire for an objective measure with which to appraise temporal sensitivity for CI users. Electrically evoked auditory steady state responses (EASSRs) are neural

Neural response telemetry measures in patients implanted with Nucleus 24.

UNLABELLED: Cochlear implantation has been recommended for children under 24 months of age. The use of objective measures is needed to help speech processor programming. The electrically evoked compound potential (EAP), which can be assessed by neural response telemetry (NRT), is one of those objective measures. AIM: to determine how often the EAP can be recorded by NRT system during surgery and to describe the responses. STUDY DESIGN: Clinical with transversal cohort. MATERIAL AND METHOD: The impedances and NRT were measured in a group of 17 Nucleus 24 implant users. The responses were ana…

A gerbil model of sloping sensorineural hearing loss.

OBJECTIVE: The goal of the overall project is to develop knowledge about cochlear physiology during cochlear implantation and develop procedures for assessing its status during hearing preservation surgery. As a step toward this goal, for this study, we established an animal model of sloping high frequency sensorineural hearing loss that mimics the hearing condition of candidates for combined electric-acoustic stimulation. METHODS: Mongolian gerbils were exposed to band-pass noise using various cutoff frequencies, intensities, exposure times, and survival times. Hearing loss was assessed in f…

Deep brain stimulation, personal identity and policy.

A range of implantable brain-interfacing devices (IBIDs) is currently in use and development for the treatment of movement disorders and disorders of mood, behaviour and thought. These include cochlear implants, deep brain stimulation (DBS), prosthetic limbs, and optogenetic interventions (the combined use of genetics and optics to control individual cells). While implantable non-brain devices, such as implantable cardioverter defibrillators, began receiving US Food and Drug Administration approval in 1980, the development of IBIDs is recent, with the approval of DBS for Parkinson's disease in

In silico framework for benchmarking optogenetic hearing restoration.

Objective. Cochlear implants (CI) partially restore hearing in profoundly hearing-impaired or deaf individuals by electrically stimulating the auditory nerve. A primary bottleneck in electrical CIs is the broad spread of electrical current from each electrode that limits the transfer of spectral information, which might be overcome by future spatially confined optogenetic stimulation. Here we established an in silico framework, FraSCO, to model sound encoding in the human cochlea by an optical CI (oCI) for testing the potential of optogenetic hearing restoration.Approach. The biophysical model

In vitro impact of platinum nanoparticles on inner ear related cell culture models.

So far, it was supposed that the increase of electrical impedance following cochlear implant (CI) insertion was due to technical defects of the electrode, inflammatory and/or formation of scar tissue along the electrode. However, it was recently reported that corrosion of the platinum electrode contacts may be the reason for high impedances. It could be shown that platinum particles were stripped from the electrode surfaces. Its potential cytotoxic effects within the inner ear remains to be examined. In this study in vitro cell culture models of the mouse organ of Corti cell line (HEI-OC1) and

Children with hearing loss can predict during sentence processing.

Listeners readily anticipate upcoming sentence constituents, however little is known about prediction when the input is suboptimal, such as for children with hearing loss (HL). Here we examined whether children with hearing aids and/or cochlear implants use semantic context to predict upcoming spoken sentence completions. We expected reduced prediction among children with HL, but found they were able to predict similarly to children with normal hearing. This suggests prediction is robust even when input quality is chronically suboptimal, and is compatible with the idea that recent advances in

Aging, cognitive load, dementia and hearing loss.

Sensorineural systems play a crucial role in the diagnosis, treatment and management of several neurological disorders. The function of the eye and ear represents a unique window for testing various conditions in cognitive decline or dementia. Touch and smell have also been found to be strongly involved in neurodegenerative conditions, and their decline has been significantly associated with the progression of the disease; hence, the idea that restoring sensory function in cognitively impaired adults might enable a significant improvement in their cognitive status, reducing the worldwide incid

A novel minimally invasive stereotaxic technique to target inner ear neurons in the mouse.

Sensorineural hearing loss (SNHL), a leading cause of disability worldwide, arises from damage to hair cells (HCs) or spiral ganglion neurons (SGNs) within the cochlea. Among its etiologies, auditory neuropathy (AN) is characterized by disrupted signal transmission due to SGN damage. Traditional interventions, such as hearing aids and cochlear implants, provide limited benefit in cases of AN, where neuronal damage impairs signal transduction to the brain. Emerging regenerative therapies, including cell replacement and gene delivery, hold potential to restore SGN function, but their application

[Auditory neuropathies in infants].

Auditory neuropathy is a sensorineural disorder characterized by absent or abnormal auditory evoked potentials, and normal cochlear outer hair cell function. It is believed that a variety of processes are involved in its pathophysiology and their influence on hearing can differ. We describe the diagnostic sequence and management of two new cases of auditory neuropathy in infants. The first case was a girl with no risk factors for hearing loss. Her absence of response to sounds was compared with her twin's reactions. Otoacoustic emissions were present but no auditory evoked potential response w

A review of the effectiveness of otoacoustic emissions for evaluating hearing status after newborn screening.

OBJECTIVES: The purpose of this study was to give a brief review of the effectiveness of otoacoustic emissions for getting frequency-specific information about a hearing-loss problem in newborns after hearing screening. Especially, the advantages of distortion-product otoacoustic emissions (DPOAE) over transiently evoked otoacoustic emissions (TEOAEs) are described. DATA SOURCES: Approximately 186 ears of 104 children aged between 76 days and 15 years and 436 ears of adults with normal hearing and sensory hearing loss. METHODS: Extrapolated DPOAE I/O-functions at frequencies between 1.5 and
