Elastic stable intramedullary nailing versus Kirschner wire pinning: outcome of severely displaced proximal humeral fractures in juvenile patients.

BACKGROUND: Significantly displaced juvenile proximal humeral fractures (Neer-Horowitz type 3 and 4) usually require reduction and fixation. The most commonly used fixation methods are Kirschner wire (K-wire) pinning or retrograde elastic stable intramedullary nailing (ESIN). However, results comparing the long-term outcome of both methods are absent in the literature. The aim of this study was to provide an outcome comparison of both techniques. METHODS: Included were 40 patients treated between 1998 and 2008 and who had complete records concerning operation time, duration of hospital stay,

Morphine Exacerbates Postfracture Nociceptive Sensitization, Functional Impairment, and Microglial Activation in Mice.

BACKGROUND: Emerging evidence suggests that opioid use immediately after surgery and trauma may worsen outcomes. In these studies, the authors aimed to determine whether morphine administered for a clinically relevant time period (7 days) in a tibia fracture orthopedic surgery model had adverse effects on postoperative recovery. METHODS: Mice were given morphine twice daily for 7 days after unilateral tibial fracture and intramedullary pin fixation to model orthopedic surgery and limb trauma. Mechanical allodynia, limb-specific weight bearing, gait changes, memory, and anxiety were measured a

The "double-intra" pertrochanteric femur fracture: an irreducible and redisplaceable fracture pattern.

BACKGROUND: Irreducible pertrochanteric femur fractures (PFFs) are associated with a higher risk of mechanical instability and poor clinical outcomes. Awareness of the specific patterns of these irreducible PFFs can help surgeons better prepare for fracture reduction and reduce unnecessary attempts at closed manipulations. This article aimed to introduce a specific subtype of irreducible PFFs, characterized by intramedullary head-neck varus impaction and the anterior fracture line located intracapsularly, termed "double-intra" PFFs. METHODS: Patients with "double-intra" PFFs treated with ceph

Buried intramedullary implants for paediatric forearm fractures. Does the refracture rate improve?

INTRODUCTION: Unstable forearm fractures may require surgical management by reduction and osteosynthesis with intramedullary needles. This fixation should be removed early if it has been left exposed, but this could increase the risk of refracture in a bone in the period of remodelling. As an alternative we can keep the needles, buried subcutaneously, for a longer time, to protect the bone callus. OBJECTIVE: To assess whether there are differences between using exposed needles with respect to burying them in paediatric patients with forearm fractures. Our hypothesis is that by burying the nee

Breakage of cephalomedullary nailing in operative treatment of trochanteric and subtrochanteric femoral fractures.

INTRODUCTION: Mechanical breakage of cephalomedullary nail osteosynthesis is a rare complication attributed to delayed fracture union or nonunion. This study presents a series of cases of breakage and secondary lag screw dislocation after cephalomedullary nailing. The aim of this study was to identify factors that contribute to cephalomedullary nail breakage. MATERIALS AND METHODS: In a retrospective case series review between 02/2005 and 12/2013, we analyzed 453 patients with trochanteric and subtrochanteric fracture who had been treated by cephalomedullary nailing. Fractures were classified

Simultaneous nailing of skeletal metastases: is the mortality really that high?

INTRODUCTION: The risk of death during simultaneous nailing of pathologic and impending fractures in patients with metastatic disease is believed to be so high that some authors have advocated a staged approach, especially for impending fractures. However, there are limited data to either support or refute the appropriateness of staging of multiple impending or pathologic fractures. QUESTIONS/PURPOSES: We therefore determined the rate of pulmonary mortality and morbidity in patients who underwent simultaneous nailing for metastatic disease of the skeleton. METHODS: We retrospectively reviewe

[Efficacy of interlocking nailing in fractures of the leg].

Closed and Grade-I open multi-fragment, comminuted, and segmental fractures of the femoral and tibial shafts are well established indications for surgical stabilisation, using interlocking nailing. Eighty-two fractures of those categories in 78 patients were handled by the authors, between 1976 and 1987. High efficiency of the approach has been confirmed by early functional treatment, soon consolidation of fragments, within half a year, less complications as compared to other techniques as well as good functional end results which had been recorded in follow-up checks from 88 per cent of the p

Bone radiofrequency ablation combined with prophylactic internal fixation for metastatic bone tumor of the femur from hepatocellular carcinoma.

A 64-year-old man with 6-year history of hepatocellular carcinoma (HCC) was referred to us regarding bone metastasis to the right proximal femur. Although he underwent radiotherapy for pain palliation and local tumor control, the pain persisted and the tumor relapsed 3 months after the radiotherapy and he was thought to be at high risk of pathologic fracture. Given hypervascularity and large tumor size, a prophylactic internal fixation combined with adjuvant radiofrequency ablation (RFA) was proposed to reduce blood loss and prevent viable tumor cells being disseminated. His postoperative cour

Transepicondylar distal femoral pin placement in computer assisted surgical navigation.

UNLABELLED: The use of optical tracking systems in computer assisted surgical navigation requires the rigid fixation of a dynamic reference base to the target bone to be navigated. This report presents the results of a new approach to optical tracker fixation in the distal femur. Four embalmed cadavers were evaluated for pin placement. It was found that placement of pins from medial to lateral parallel to the transepicondylar axis placed the pins well posterior to the center of the intramedullary canal and away from neurovascular structures. Eighty-six consecutive patients underwent total knee

Use of a Minimally Invasive Traction Repositor versus Conventional Manual Traction for the Treatment of Tibial Fractures: A Comparative Study from a Tertiary Hospital in China.

BACKGROUND: Closed reduction and intramedullary nail fixation of tibial fractures may not utilize a fracture table or reduction aids like a femoral distractor, and only manual traction will help aid the reduction process. This study aimed to describe and further investigate the effectiveness of an originally designed minimally invasive traction repositor (MITR) for the treatment of tibial fractures. METHODS: From January 2018 to April 2021, a total of 119 eligible patients with tibial shaft fractures were included and retrospectively assigned to two groups according to different reduction met

Percutaneous cerclage wiring followed by intramedullary nailing for subtrochanteric femoral fractures: a technical note with clinical results.

BACKGROUND: Although intramedullary nailing is an ideal treatment for subtrochanteric femoral fractures, it is technically challenging in fractures extending into the nail entry area and/or involving the lesser trochanter. Although the application of circumferential wire may facilitate reduction in these situations, its use remains controversial due to possible blood supply disturbances to underlying bone. In the present study, we evaluated complex subtrochanteric fractures treated by percutaneous cerclage wiring followed by intramedullary (IM) nailing for anatomical fracture reduction and uni

Establishment of a rabbit's model for modified Capanna technique.

BACKGROUND: Massive bone allograft with an intramedullary vascularized fibula (Capanna technique) represents one of the most widely used methods for the reconstruction of massive bone defect. However, the mechanisms, healing process, and underlying influential factors were poorly understood due to the lack of suitable experimental animal models. METHODS: Critical-sized defects (CSD) in bone were constructed in the proximal tibia of 24 rabbits and randomly divided into two groups. Allogeneic bone segments of the same size as CSDs were obtained from another 12 rabbits and then inactivated. In g

[Retrograde tibial intramedullary nailing with the Flex Nail--treatment of tibial fracture in an unusual case].

We report a case by a 75 year old female patient suffering a third-degree open fracture of the shinbone with a severe damage of the soft tissue. Primary operation was a temporary fixation with a fixateur externe and a radical debridement of the soft tissue with vacuum-sealing. Ten days later we did the definitive surgery. Osteosynthesis was done by a retrograd nailing with the flexible unreamed humerus nail (Flex-Nail) through the medial ankle. The defect of the soft tissue was treated by a muscle flap and meshgraft transplantation. Healing of the soft tissue and beginning consolidation of the

Iatrogenic arterial trauma associated with hip joint surgery: an overview.

Four cases of iatrogenic arterial injuries following hip surgery are reported. An overview of the literature revealed 122 additional cases. In 93 (74%) of these cases the arterial damage occurred following total hip replacement, 27 (21%) were related to hip fracture correction and six cases complicated other hip surgery procedures. Revisional hip surgery was found to be a definite risk factor. Injuries associated with hip replacement differ significantly in various characteristics of those associated with hip fracture correction. There was a 9% mortality and a 17% permanent disability rate in

Internal fixation of proximal humerus fractures using the T2-proximal humeral nail.

INTRODUCTION: Surgical management of proximal humerus fractures remains controversial and there is an increasing interest in intramedullary nailing. Created to improve previous designs, the T2-proximal humeral nail (PHN) (Stryker) has been recently released, and the English literature lacks a series evaluating its results. We present a clinical prospective study evaluating this implant for proximal humeral fractures. METHOD: We evaluated the functional and radiological results and possible complications.
