INTRODUCTION: There is currently no consensus on the choice between tenotomy and tenodesis of the long head of the biceps tendon in rotator cuff repair. The Popeye sign is often seen as a drawback of tenotomy. The main objective of the present study was to determine the frequency and clinical impact of the Popeye sign.
HYPOTHESIS: The Popeye sign is rarely found clinically, and shows little functional impact following tenotomy.
MATERIALS AND METHODS: A single-center non-randomized prospective study was conducted between February and October 2015 in all patients undergoing rotator cuff surger

OBJECTIVE: To determine which normalization method may best account for confounding individual factors, such as age or BMI, when quantifying fat infiltration on MRI in patients with rotator cuff tears, the effects of normalization using three different muscles (teres major; triceps brachii; teres minor) were compared.
METHODS: Thirty-seven consecutive patients diagnosed with rotator cuff pathology were included. MRI fat-water sequences were used to quantify rotator cuff intramuscular fat (%fat). Three reference muscles (teres major, triceps, teres minor) were used to derive normalized %fat. R

BACKGROUND: Infection after rotator cuff repair (RCR) is uncommon. There are few reports in the literature regarding the management and long-term results of patients in whom deep infection of the shoulder develops after RCR. The objective of this study was to assess the long-term clinical and radiologic outcomes of these patients.
METHODS: We retrospectively reviewed a consecutive series of 764 patients after mini-open RCR in which 9 patients had postoperative infection. The demographic data, clinical and laboratory findings, risk factors, bacteriologic findings, and results of surgical manag

BACKGROUND: In recent studies objective evaluations have demonstrated that arthroscopic rotator cuff repairs can have higher failure rates than open repairs. Thus, there is a need for a stronger tissue-holding stitch for arthroscopical repair. The purpose of this study was to compare the biomechanical properties of traditional open transosseous suture technique and modified Mason-Allen stitches versus double-loaded suture anchors and arthroscopic Mason-Allen stitches in rotator cuff repair.
METHODS: In 20 sheep shoulders the infraspinatus tendons were dissected from their insertion and were r

OBJECTIVE: To determine the most appropriate means to assess the response to treatment in terms of pain and functional impairment in a chronic rheumatic condition (knee osteoarthritis [OA]) and an acute rheumatic condition (rotator cuff syndrome [RCS]).
METHODS: Two prospective studies were conducted consisting of 1,019 outpatients with knee OA and 271 patients with acute RCS. The minimal clinically important improvement and the patient acceptable symptom state were determined for knee OA pain using a visual analog scale, and for knee OA function using the Western Ontario and McMaster Univers

We obtained MR images of 140 painful shoulders in 134 patients to determine the relationship between cystic changes of the humeral head and integrity of the rotator cuff. Cystic changes were observed in 49 shoulders (35%) and the commonest site was in the bare bone area of the anatomical neck, and the second commonest site was at the attachment of the supraspinatus tendon. Cystic changes in the bare bone area were observed equally often in shoulders with or without rotator cuff tears (27% and 18%, respectively) and were more frequently observed in the elderly. Cystic changes at the attachment

BACKGROUND: Muscle synergies are defined as the central nervous system's organizational structure for movement. Muscle synergies change after muscular fatigue, with certain synergies assuming the primary role to compensate for fatigue within another muscle synergy. Owing to the high eccentric forces imposed upon the external rotators (ie, posterior rotator cuff), pitchers that continue to throw while fatigued are at a significantly higher risk of shoulder and/or elbow injury; however, the neuromuscular compensation strategies of baseball players in response to fatigue are currently unknown.

After more than 15 years of experience, arthroscopic shoulder stabilization is becoming less controversial. Historically, recurrence rates following arthroscopic stabilization have been higher than with open stabilization. Although a negligible advantage may exist in terms of expedited postoperative rehabilitation and improved postoperative recovery of motion, critics suggest that its use in contact athletes be limited. The indications for arthroscopic stabilization are expanding, in part, because of improved understanding of the pathophysiology of shoulder instability. Understanding the mecha

BACKGROUND: Reverse total shoulder arthroplasty (RTSA) has continued to increase in clinical utility and popularity as an effective treatment for cuff tear arthropathy (CTA), irreparable rotator cuff tears (RCTs), osteoarthritis, and acute 3- and 4-part proximal humeral fractures. Performing RTSA for acute proximal humeral fractures presents the unique challenges of tuberosity management, bone loss, and instability compared with elective indications such as CTA or irreparable RCTs. The purpose of this study was to compare the clinical outcomes, active range of motion (ROM), radiographic outcom

The ageing workforce is one of the major issues in Europe. Ageing has an important effect on work ability, especially in manual jobs where physical demand is high. Musculoskeletal disorders are the most common health problems among European workers and are able to affect work ability. In order to increase the employment rate of people aged 45-65, measures to sustain work ability and proper age management should be promoted: return to work strategies include adjustment of the job to the worker's health conditions and abilities. The contribution of health surveillance.programs in the management

The combination of interscalene brachial block (ISB) combined total intravenous general anesthesia has been recommended for arthroscopic rotator cuff repair (ARCR) to enhance pain relief and reduce the need for additional analgesics after surgery. We aimed to explore the feasibility of using pupillary pain index (PPI) to evaluate the analgesic effect of ISB in patients undergoing ARCR under sevoflurane anesthesia. This prospective, observational, single-center study took place between October 26, 2022, and October 11, 2023. We included 34 adult patients scheduled for ARCR under combined ipsila

This study aims to explore the neural correlates of postoperative pain and its relationship with preoperative psychological issues in patients with rotator cuff tear (RCT). Functional MRI data were collected from 78 RCT patients and 48 healthy controls (HC). Voxel-wise comparisons assessed regional homogeneity (ReHo) differences between groups. Pearson correlation and mediation analyses investigated the links between clinical data and brain changes. Additionally, machine learning using support vector machines (SVM) classified RCT patients based on postoperative pain intensity. RCT patients sho
