The prevalence and risk factors of allergic and respiratory symptoms in a regional cohort of extremely low birth weight children (<1000 g).

BACKGROUND: Children who were <1000 g (ELBW extremely low birth weight) at birth more frequently present with wheezing which is the most common reason that pediatric consultation is sought. Therefore asthma is diagnosed very often. However is the asthma that is diagnosed in ELBW subjects atopic in origin, or is there a different etiology? AIM: To determine if ELBW infants are at higher risk for the development of allergic and respiratory symptoms and to establish if there were any specific risk factors for these symptoms. METHODS: 81 children born with a mean birthweight of 845 g (91% of ava

Lower airway inflammation before and after house dust mite nasal challenge: an age and allergen exposure-related phenomenon.

BACKGROUND: Upper and lower airways allergic disease is currently considered unitarily. Allergic inflammation in one site can extend to other sites of the respiratory tract. OBJECTIVE: To evaluate bronchial inflammation before and after allergen-specific nasal challenge (ASNC) in rhinitic and asthmatic children, considering the different levels of allergen exposure, i.e. summer (low) and winter (high). METHODS: Fourteen children with rhinitis and 15 with rhinitis and asthma, all monosensitized to mites and 10 healthy controls were studied. Nasal IgE were measured before ASNC in summer and in

Long-Term Clinical and Biological Outcomes of Biologic Therapy in Severe Asthma: 24-Month Real-World Cohort Study from Romania.

Background: Severe asthma remains associated with substantial morbidity despite optimized inhaled therapy. Biologic agents targeting type 2 inflammation improve clinical outcomes; however, real-world evidence regarding the durability of these effects beyond the first treatment year remains limited. The present study extends the follow-up of a previously reported real-world cohort in which 12-month outcomes of biologic therapy were evaluated. Methods: We conducted a retrospective observational longitudinal study of adults with severe asthma treated with omalizumab, benralizumab, or dupilumab at

Quality of Life and Airway Inflammation in Children With Asthma: Integrating Clinical and Functional Measures Through the International Classification of Functioning (ICF).

BACKGROUND AND PURPOSE: Asthma is one of the most prevalent chronic diseases in childhood and a major cause of respiratory morbidity, school absenteeism, and restrictions in physical activity. Although asthma control is commonly assessed using clinical and inflammatory markers, its functional impact on daily activities and participation is less frequently addressed. Fractional exhaled nitric oxide (FeNO) is a well-established biomarker of eosinophilic airway inflammation; however, its integration with quality-of-life measures within a biopsychosocial framework, such as the International Classi

Implementing MRI in the radiotherapy treatment position for patients with hepatobiliary malignancies: A staged feasibility evaluation.

BACKGROUND: Magnetic resonance imaging (MRI) is increasingly recommended in radiotherapy planning for patients with hepatobiliary malignancies due to its superior soft-tissue contrast. However, implementing MRI in the radiotherapy treatment position, particularly when incorporating motion-management strategies such as abdominal compression (AC) and exhale breath-hold (EBH), remains operationally challenging. This evaluation aimed to establish and evaluate a clinically deliverable MRI-in-treatment-position workflow for hepatobiliary radiotherapy. METHODS: A single-centre staged implementation

Unrecognized ciliary motility disorders in neutrophilic severe asthma exacerbations.

BACKGROUND: Airway bacterial infections are frequent in severe asthma and are often under-appreciated as contributors to symptoms and exacerbations. We report our experience using integrative diagnostic methods to identify ciliary motility disorders as contributors to neutrophilic exacerbations in patients with severe asthma. METHODS: Targeted exome sequencing for primary ciliary dyskinesia (PCD) was performed on 52 patients with severe asthma who met predefined criteria (≥ 3 respiratory infections or intense sputum neutrophilia, within a 2-year period, along with evidence of type 2 inflammat

Quantitative mapping of cerebrovascular reactivity amplitude and delay with breath-hold BOLD fMRI when end-tidal CO2 quality is low.

Cerebrovascular reactivity (CVR), the ability of cerebral blood vessels to dilate or constrict in response to a vasoactive stimulus, is a clinically useful measure of cerebrovascular health. CVR is often measured using a breath-hold task to modulate blood CO2 levels during an fMRI scan. Measuring end-tidal CO2 (PETCO2) with a nasal cannula during the task allows CVR amplitude to be calculated in standard units (vascular response per unit change in CO2, or %BOLD/mmHg) and CVR delay to be calculated in seconds. The use of standard units allows for normative CVR ranges to be established and for C

Higher nicotine dependence and greater smoking abstinence in parental than non-parental smokers: a secondary analysis of smoking cessation trials.

OBJECTIVE: Parental smokers account for approximately one-third of smokers worldwide and often exhibit distinct smoking and quitting behaviors compared to non-parental smokers. Understanding these differences is crucial for developing targeted cessation interventions; however, current evidence remains limited. METHODS: This secondary analysis pooled individual participant data from 10 community-based smoking cessation trials conducted in Hong Kong between 2010 and 2020 (N = 10,871 adult daily smokers). We compared parental smokers (those with at least one child) and non-parental smokers in te

Impact of positive end-expiratory pressure and recruitment maneuver on healthy lungs in dogs assessed by functional and anatomical monitoring methods.

INTRODUCTION: Atelectasis is a common occurrence during anesthesia, and positive end-expiratory pressure (PEEP) ventilation and recruitment maneuvers (RM) can be used to mitigate this. However, both techniques may be associated with side effects in healthy lungs, and close monitoring is indicated. This study aimed to evaluate the effects of PEEP and RM in healthy dogs and to compare functional lung monitoring methods by electrical impedance tomography (EIT), volumetric capnography (VCap), and blood gas analysis with the gold-standard anatomical monitoring provided by computed tomography (CT).

Increased Serum EphA2 is Associated with Disease Severity in Pediatric Patients with Asthma.

BACKGROUND: Asthma is the most prevalent chronic inflammatory airway disease in children, with increasing incidence and prevalence. Ephrin type-A receptor 2 (EphA2) belongs to the Ephrin (Eph) family. It is predominantly found in bronchial epithelial cells and may play a potential role in mediating airway inflammation in asthma. However, this study aimed to evaluate the association between a novel biomarker, EphA2, in two distinct pediatric asthma populations stratified by disease severity. MATERIALS AND METHODS: Serum levels of interleukins (IL-1β, IL-4, IL-6, IL-8, IL-13), tumor necrosis fa

Isolating the effects of carbohydrate and lipid availability on exercise-induced skeletal muscle signalling in males.

Training with low carbohydrate availability can increase AMP-activated protein kinase (AMPK) activation, but whether increased AMPK activation is the result of low carbohydrate availability per se or concurrent increases in fatty acid availability/oxidation is unclear. This study assessed the independent effects of carbohydrate and fatty acid availability on exercise-induced skeletal muscle AMPK activation and downstream signalling. Eight active males who were aged between 18 and 60 years with a body mass index in the range 18.0-30.0 kg m-2 cycled on three occasions for 60 min at 95% of lactat

Exploring dupilumab for asthma: from mechanistic insights to clinical outcomes, safety, and cost-effectiveness.

Type 2 (T2) inflammation underlies a substantial subset of moderate-to-severe asthma, contributing to persistent symptoms and frequent exacerbations. Dupilumab, a fully human immunoglobulin G subclass 4 (IgG4) monoclonal antibody, targets the interleukin-4 receptor alpha (IL-4Rα), thereby inhibiting both interleukin-4 (IL-4) and interleukin-13 (IL-13) signaling-which are key cytokines driving T2 inflammation. This review examines the formulation, pharmacological profile, clinical efficacy, safety, and cost-effectiveness of dupilumab in the treatment of asthma, with an emphasis on its role acro

Eosinophilic Bronchiectasis: Prevalence, Severity, and Associated Features-A Cohort Study.

Background: Bronchiectasis (BE) has been traditionally associated with neutrophilic inflammation, but eosinophilic bronchiectasis (EB) has recently emerged. Data about prevalence, clinical features, and disease severity are lacking. This study aimed to assess the EB prevalence, compare EB with non-EB, evaluate the Type-2 (T2) high endotype in BE (T2-high EB) versus non-T2-high EB, and identify EB predictors. Methods: We conducted a prospective study involving 153 BE patients. The data collected included clinical, radiological, and microbiological findings. BE severity was assessed using the br

Host and pathogen factors that influence variability of Mycobacterium tuberculosis lipid body content in sputum from patients with tuberculosis: an observational study.

BACKGROUND: High proportions of Mycobacterium tuberculosis cells in sputum containing triacylglycerol-rich lipid bodies have been shown to be associated with treatment failure or relapse following antituberculous chemotherapy. Although lipid body determination is a potential biomarker for supporting clinical trial and treatment decisions, factors influencing variability in sputum frequencies of lipid body-positive (%LB+) M tuberculosis in patients are unknown. We aimed to test our hypothesis that exposure to host-generated NO and M tuberculosis strains are factors associated with differences i

Treating severe paediatric asthma with mepolizumab or omalizumab: a protocol for the TREAT randomised non-inferiority trial.

INTRODUCTION: A minority of school-aged children with asthma have persistent poor control and experience frequent asthma attacks despite maximal prescribed maintenance therapy. These children have higher morbidity and risk of death. The first add-on biologic therapy, omalizumab, a monoclonal antibody that blocks immunoglobulin (Ig)E, was licensed for children with severe asthma in 2005. While omalizumab is an effective treatment, non-response is common. A second biologic, mepolizumab which blocks interleukin 5 and targets eosinophilic inflammation, was licensed in 2018, but the licence was gra

Effects of ethanol and sex on propionate metabolism evaluated via a faster 13C-propionate breath test in rats.

BACKGROUND: Alcoholism is regarded as a risk factor for vitamin B12 (VB12) deficiency. Because VB12 serves as a coenzyme of methylmalonyl-CoA mutase, a key enzyme in propionate metabolism, the 13C-propionate breath test (PBT) has been studied as a non-invasive diagnostic modality for VB12 deficiency. However, the conventional PBT requires 2 h, which is inconvenient in clinical practice. We hypothesized that a faster PBT can be used to evaluate propionate metabolism and is more easily adaptable for clinical practice. AIM: To evaluate a faster PBT for assessing the effects of long-term ethanol
