Trends in Epidemiology and Treatment of Humerus Fractures in the United States, 2017-2022.

Background Fractures of the humerus are one of the more common fractures in the United States and a cause of fragility fractures in the elderly population. This study aims to understand recent trends in the demographic factors correlated with humeral shaft fractures (HSF) and humeral shaft fracture nonunion (HSFN) following open reduction internal fixation (ORIF) and intramedullary nailing (IMN). Methods The TriNetX database was used to query using International Classification of Diseases-10 (ICD10) diagnosis codes for patients who sustained HSF between 2017 and 2022. Patients were then organi

Obesity is associated with increased postoperative complications after operative management of tibial shaft fractures.

OBJECTIVES: To assess the association of obesity and postoperative complications after operative management of tibial shaft fractures. METHODS: Patients who underwent operative management of a tibial shaft fracture were identified in a national database by Current Procedural Terminology (CPT) codes for: (1) open reduction and internal fixation (ORIF) and (2) intramedullary nailing (IMN) procedures in the setting of International Classification of Diseases, Ninth Revision (ICD-9) codes for tibial shaft fracture. These groups were then divided into non-obese, obese, and morbidly obese cohorts u

Fixation of pathological humeral fractures by the cemented plate technique.

Pathological fractures of the humerus are associated with pain, morbidity, loss of function and a diminished quality of life. We report our experience of stabilising these fractures using polymethylmethacrylate and non-locking plates. We undertook a retrospective review over 20 years of patients treated at a tertiary musculoskeletal oncology centre. Those who had undergone surgery for an impending or completed pathological humeral fracture with a diagnosis of metastatic disease or myeloma were identified from our database. There were 63 patients (43 men, 20 women) in the series with a mean age

Treatment of proximal humerus fracture using multiple intramedullary flexible nails.

A total of 61 patients with a proximal humeral fracture was treated between January 1996 and March 1998 by closed reduction and fracture fixation with intramedullary Prévot (or Nancy) nails. Of these, 28 female and 25 male patients with a mean age of 52 years (range 3-91 years) were reviewed clinically and radiologically with a mean follow-up of 17 months (range 4-30 months). The mean Constant score was 63, the mean Neer score 74 and the mean visual analogue scale (VAS) 73. The 14 patients under 24 years old achieved a Constant score of 86, a Neer score of 99 and a VAS of 97, while 13 patients

Staged Treatment of Infected Tibiotalar Fusion Using a Combination Antibiotic Spacer and Antibiotic-Coated Intramedullary Nail.

Pilon fractures are notoriously difficult injuries to treat. The current published data on salvage procedures after failed pilon fractures includes both total ankle arthroplasty (TAA) and tibiotalar fusion, each with its own specific indications. However, no acceptable treatment algorithm addressing the complications of these limb salvage procedures is available. We present the case of a 23-year-old patient, who sustained a complex pilon fracture after a motor vehicle accident. The patient was referred to our institution after an initial fixation attempt, followed by subsequent failed TAA, whi

Modified Masquelet Technique Using Allogeneic Graft for a Gustilo-Anderson Type III-A Open Fracture of the Femur with an 8 cm Bone Defect.

The induced membrane technique was initially described by Masquelet et al. in 1986 as a treatment for tibia nonunion; then, it became one of the established methods in the management of bone defects. Several changes have been made to this technique and have been used in different contexts and different methodologies. We present the case of a 16-year-old girl admitted to our department for a polytrauma after a motorcycle accident. She presented a Gustilo III-A open fracture of the right femoral shaft with a large bone defect of 8 centimeters that we treated with a modified Masquelet technique.

Osteosynthesis of pediatric femoral shaft fractures with flexible intramedullary nailing-experience from developing world.

BACKGROUND: Flexible intramedullary nailing (FIN) has become the standard treatment for pediatric femoral fractures in the 6-14 years age group. The other treatment options include traction plus spica casting and external fixation. In spite of excellent results described, there is disunity in the orthopedic fraternity regarding its usefulness and nail material. The aim of our study is to determine the outcome of pediatric femur fractures treated with Stainless steel FIN. METHODOLOGY: Between 2014 to 2018, 45 children within the age group of 6-12 years with femoral shaft fractures were include

The course of treatment in nonunions of long bones: can we predict when exchange nailing may fail?

PURPOSE: Exchange nailing is often used to treat femoral and tibial nonunions. Its success rate varies with reports of 50-100% success. We sought to determine the trajectory of healing and identify factors associated with the failure of exchange nailing in the treatment of nonunion. METHODS: This was a retrospective chart review at two level I trauma centers. Patients with aseptic femoral and tibial nonunions treated with exchange nailing were included. The primary outcome was union prior to or at 1 year following treatment. Treatment failure was defined as lack of union (defined as lack of b

Evaluation of matrix metalloproteases as early biomarkers for bone regeneration during the applied Masquelet therapy for non-unions.

INTRODUCTION: In the current study, we sought to determine if serum concentrations of MMPs correlate with bone regeneration occurring during the course of the Masquelet-therapy and to identify if MMPs may serve as early biomarkers reflecting successful bone regeneration and tissue remodeling. MATERIAL AND METHODS: This study was designed as a prospective clinical observer study. We compared serum samples over the time of treatment, as a matched-pair analysis, from 10 patients who were treated successfully with the Masquelet-therapy (Responder) with 10 patients who did not respond to the Masqu

Treatment of femoral and tibial diaphyseal nonunions using reamed intramedullary nailing without bone graft.

INTRODUCTION: Intramedullary nailing has long been used successfully in the treatment of aseptic nonunions of the femur and tibia. However, recently the efficacy of reamed intramedullary nailing in the treatment of nonunions of the femur has been questioned by some publications reporting unfavourable results. The purpose of this study is to evaluate the treatment results of femoral and tibial diaphyseal nonunions with intramedullary nailing. PATIENTS AND METHODS: We retrospectively reviewed thirty-two patients with femoral or tibial diaphyseal nonunions who were treated with reamed intramedul

Biomechanical design considerations of a 3D-printed tibiotalocalcaneal nail for ankle joint fusion.

Tibiotalocalcaneal (TTC) arthrodesis treatment using intramedullary nails faces significant challenges due to inadequate bone integration and mechanical stability. This study developed a novel 3D-printed long titanium TTC intramedullary nail incorporating diamond lattice structures and differential thread leads to enhance biological fixation and compression. Four 3D-printed TTC nails (5 mm diameter, 70 mm length) with solid (TTC 1), lattice structure (TTC 2), lattice with longitudinal ribs (TTC 3), and lattice with both longitudinal and transverse ribs (TTC 4) were designed and manufactured. [Intramedullary osteosynthesis of the ulna in revision surgery].

The exact and adequate management of diaphyesal fractures of the forearm is necessary to ensure forearm motion. Unsatisfactory treatment can lead to loss of function and disability of forearm and hand movement, resulting in severe patient morbidity. Since the rotational and angulatory stresses at the fracture site are the superior factors which influence the outcome of the forearm fracture healing after surgical treatment, the implants must fulfill some general prerequisites including high biomechanical stability, less invasive surgical approach and adequate biological features. However, less
