Machine Learning Models for Predicting Latent Tuberculosis Infection Risk in Close Contacts of Patients with Pulmonary Tuberculosis - Henan Province, China, 2024.

INTRODUCTION: We explored risk factors for latent tuberculosis infection (LTBI) and developed a risk prediction model using machine learning algorithms. METHODS: Patients with active pulmonary TB in months 3 to 6 of anti-TB treatment in Henan Province, China, July-September 2024 were selected as index cases. Close contacts identified through epidemiological investigation underwent tuberculin-purified protein derivative testing to determine LTBI status. Face-to-face questionnaires were conducted to collect epidemiological data. The dataset was divided into training and testing sets (6:4), usin

Mass school testing for Mycobacterium tuberculosis infection and disease in an adolescent population: A prospective cohort from eastern China.

ObjectivesAdolescents remain underprioritized in tuberculosis (TB) control, with limited evidence guiding screening and preventive strategies. Leveraging a mass school-based screening program in eastern China, we evaluated the prognostic value of tuberculin skin test (TST) induration size for predicting progression to active TB disease among adolescents within a prospective cohort framework. Methods:From 2018 to 2021, TST-based screening was conducted among newly enrolled students across 68 high schools in Jiangsu Province, China. All students were screened at baseline and followed through reco

Chasing shadows: case series of six posterior segment manifestations of ocular tuberculosis.

BACKGROUND: Ocular tuberculosis (OTB) is difficult to confirm microbiologically, so diagnosis often rests on characteristic posterior segment phenotypes supported by systemic immunologic evidence and therapeutic response. This case series from a high-prevalence setting illustrates the heterogeneity of posterior OTB and provides practical diagnostic anchors, treatment approaches, and complication surveillance that are directly applicable to routine uveitis care. CASE DESCRIPTION: Six immunocompetent males (28-46 years) presented with posterior disease entities that included: choroidal granulom

Disseminated tuberculosis with Poncet's disease mimicking childhood-onset systemic lupus erythematosus: a unique case report.

Disseminated tuberculosis in children may present with systemic and immunologic features that overlap with autoimmune diseases, complicating diagnosis and treatment. We report the first pediatric case of microbiologically confirmed disseminated tuberculosis involving multiple sites associated with Poncet's disease, mimicking childhood-onset systemic lupus erythematosus (cSLE). A previously healthy five-year-old boy presented with a four-month history of persistent fever, weight loss, and symmetrical oligoarthritis involving both knees and wrists, resulting in loss of mobility. Laboratory tests

Impact of Diabetes Mellitus in Tuberculosis (TB) Patients on TB Transmission.

BACKGROUND: Diabetes mellitus (DM) increases the risk of tuberculosis (TB) progression and poor treatment outcomes. Rising global DM prevalence presents an emerging threat to TB control. We sought to determine whether DM affects TB transmissibility. METHODS: From 2009 to 2012, we enrolled 3109 microbiologically confirmed pulmonary TB patients and their 12 767 household contacts (HHCs) whom we followed for 1 year for the occurrence of TB infection as measured by a tuberculin skin test and for TB disease. We assessed the association between index patient DM and TB infection and disease occurren

Multiparameter antigen-specific immunoprofiling in subjects with negative IGRA and TST results with potential M. tuberculosis exposures.

INTRODUCTION: Asymptomatic individuals with negative Interferon-γ release assay (IGRA) and tuberculin skin test (TST) results are often labeled as tuberculosis-unexposed individuals. We hypothesize that flow cytometric (FC) detection of antigen-stimulated T-cells in peripheral blood can differentiate IGRA(-)/TST(-) individuals with and without prior potential M. tuberculosis (Mtb) exposure. METHODS: We used FC multiparameter profiling of T-cell activation markers in TST/IGRA-negative individuals at risk for Mtb exposure and in unexposed controls. RESULTS: We studied 54 IGRA(-)/TST(-) subject

Latent tuberculosis infection screening of adult close contacts: a cost-utility analysis.

BACKGROUND: The 2016 National Institute for Health and Care Excellence guidelines recommended the tuberculin skin test (TST), at a 5-mm induration size cut-off, for the diagnosis of latent tuberculosis infection (LTBI) among adult close contacts of active tuberculosis (TB) cases. This study analysed a well-characterised cohort of adult close contacts in London and assessed the cost-effectiveness of LTBI screening strategies with combinations of TST and interferon-γ release assays (IGRAs) in a decision-analytic model. METHODS: Close contacts of pulmonary TB cases who were tested with TST and I

Monocyte Activation in People With HIV and Tuberculosis Coinfection and Effect of Tuberculosis Preventive Therapy: An Analysis of the ACTG A5279/BRIEF TB Trial.

BACKGROUND: Monocyte activation contributes to the pathogenesis of inflammation-driven comorbidities in people with HIV (PWH). We investigated the impact of tuberculin skin test (TST)/interferon-γ release assay (IGRA) status and tuberculosis preventive therapy (TPT) on monocyte activation in PWH. METHODS: We analyzed peripheral blood mononuclear cells from participants from the A5279/BRIEF-TB trial, which compared 1 month of rifapentine/isoniazid (1HP) versus 9 months of isoniazid (9H) as TPT in PWH. All included participants were on suppressive antiretroviral therapy and had available TST or

Machine Learning-based Prediction of Active Tuberculosis in People With HIV Using Clinical Data.

BACKGROUND: Coinfections of Mycobacterium tuberculosis (MTB) and human immunodeficiency virus (HIV) impose a substantial global health burden. Patients with MTB infection face a heightened risk of progression to incident active TB, which preventive therapy can mitigate. Current testing methods often fail to identify individuals who subsequently develop incident active TB. METHODS: We developed random forest models to predict incident active TB using patients' medical data at HIV-1 diagnosis. Training our model involved using clinical data routinely collected at enrollment from the Swiss HIV C

Evaluation of the post-landing surveillance process for people migrating to British Columbia: A retrospective cohort study.

OBJECTIVES: People migrating to Canada are disproportionately impacted by tuberculosis (TB) disease. Prior to arrival, Immigration, Refugees and Citizenship Canada (IRCC) require an immigration medical exam (IME) intended to detect TB disease in all permanent and select temporary residents. People deemed high risk for TB are then referred for post-landing surveillance. This process has not substantially changed in four decades, prompting our review. METHODS: We performed a retrospective cohort study of people referred for post-landing surveillance in British Columbia (BC) from Aug 1, 2020, to

A case report of pancreatic tuberculosis: arterial infusion of isoniazid as an adjunct to oral antituberculosis medications has demonstrated promising therapeutic efficacy.

BACKGROUND: Isolated pancreatic tuberculosis is a rare manifestation of Mycobacterium tuberculosis infection in the human body. CASE PRESENTATION: A 35-year-old male presented at our medical center with upper abdominal distension and pain. Pre- and post-admission Computed Tomography (CT) and Magnetic Resonance Imaging (MRI) scans revealed a pronounced ring-enhanced mass at the head of the pancreas, compressing the adjacent common bile duct. Initially, the radiologist misdiagnosed the mass as a pancreatic neuroendocrine tumor. The patient had no known history of tuberculosis exposure, and the

An unusual case of pleural effusion.

CASE PRESENTATION: A 63-year-old man presented with fever, thoracalgia, weight loss, diffuse lymphadenopathy, and a massive pleural effusion. Extensive laboratory and radiologic investigations for possible autoimmune, infectious, hematologic, and neoplastic conditions all resulted negative. A lymph node biopsy showed a granulomatous necrotizing lymphadenitis, suspicious for tuberculosis. Although mycobacterium tuberculosis (MT) was never isolated and tuberculin skin test resulted negative, diagnosis of extrapulmonary tuberculosis was made and anti-tubercular therapy was started. Despite the st

Progress on diagnosis and treatment of latent tuberculosis infection.

One fourth of the global population has been infected with Mycobacterium tuberculosis, and about 5%-10% of the infected individuals with latent tuberculosis infection (LTBI) will convert to active tuberculosis (ATB). Correct diagnosis and treatment of LTBI are important in ending the tuberculosis epidemic. Current methods for diagnosing LTBI, such as tuberculin skin test (TST) and interferon-γ release assay (IGRA), have limitations. Some novel biomarkers, such as transcriptome derived host genes in peripheral blood cells, will help to distinguish LTBI from ATB. More emphasis should be placed o

CT and 18F-FDG PET abnormalities in contacts with recent tuberculosis infections but negative chest X-ray.

Close contacts of individuals with pulmonary tuberculosis are at risk for tuberculosis infection and the development of active tuberculosis. In current contact investigations, immunologic tests (the tuberculin skin test and interferon-gamma release assay) and chest X-ray examinations are used to dichotomize contacts with Mycobacterium tuberculosis infections into those with active (X-ray abnormalities) versus latent tuberculosis (normal radiographs). This article is a critical review of computed tomographic (CT) and 18-fluorodeoxyglucose positron emission tomographic (PET) findings of incipien

Strong tuberculin response after BCG vaccination is associated with low multiple sclerosis risk: a population-based cohort study.

BACKGROUND: Multiple sclerosis (MS) is characterized by inflammatory lesions in the central nervous system involving pro-inflammatory T-cells. Immune dysregulation is well described in prevalent disease, but it is not known whether this precedes disease development. Bacillus Calmette-Guérin (BCG) vaccination ameliorates MS-like disease in mice. In people vaccinated with BCG, the tuberculin skin test (TST) offers a standardized measure of a T-cell-mediated immune response. We therefore hypothesized that the strength of the TST response after BCG vaccination is associated with subsequent MS risk
