The authors relate the results of the treatment of 151 patients with the remarkable painful syndrome of long standing associated with lumbar osteochondrosis. In 74 patients of the main group, peridural administration of the synthetic opiates dipidolor and phentanyl provided effective analgesia in the lack of sympathetic denervation, arterial hypotension and tissue deafferentation. The selective action of the synthetic morphine-like opiates did not reduce the activity of the endogenous antinociceptive structures or destroyed the reflex mechanisms of sanogenesis. Based on the sufficient clinical material with the use of the current research methods including computer-aided tomography the author provides evidence in favour of a possibility of carrying out successful conservative therapy in the majority of patients with the compression syndromes of lumbar osteochondrosis. Such conservative therapy is of paramount socioeconomic importance, since the percentage of invalidism in the youngest capable age often increases just after the surgical intervention.

The authors review febrile attacks of schizophrenia and the febrile period of the malignant neuroleptic syndrome in schizophrenia. Twenty-five schizophrenia patients were observed to establish the effect of the ECT on their status. The ECT was found to be highly effective in combination with intensive infusion therapy aimed at the correction of homeostasis. The efficacy of anticholinesterase (ACE) drugs was studied and compared in patients with alcoholic polyneuropathy (AP) subjected to electroneuromyographic (ENMG) studies. The treatment efficiency was compared to blood carbamazepine++ levels in patients with trigeminal neuralgia which were treated with the drug over a long period. The parameters displayed a good (r = 0.70) correlation that resulted in a tendency to increase treatment effect with carbamazepine++ plasma concentration increase. The treatment proved most efficient and side effect-free in a concentration range of 5 to 10 micrograms/ml. The data on blood carbamazepine++ concentrations may be helpful in designing a controlled course of treatment.

Immunological investigation was performed in patients with trigeminal neuralgies and painful myofascial temporomandibular joint dysfunction before and after treatment. The data suggest a connection between the state of humoral immunity and the peculiarities of pain syndrome in trigeminal neuralgia and clinically related prosopalgies that was most probably due to different mechanisms of realization of pain syndrome.

The results of 20 years of investigation in the light of current literature data are indicative of the trigeminal neuralgia being a separate disease with its specific etiology and pathogenesis. In 93 to 97% of the patients compression of a trigeminal branch or radicle were primary causes of the disease. This provided a basis for the secondary allogenic system of paroxysmal type which explains the high efficacy of a number of antiepileptic drugs. With a lack of a steady effect of a conservative treatment, a surgical decompression of the trigeminal branch or radicle is a method of choice.

In patients with thyrotoxic myopathy, histological investigation showed the decrease in mean diameter of skeletal muscles, and increased collateral branching of distal axons. The authors describe the dopamine deficiency syndrome in children with the disease beginning during their first year and peculiar dystonia++ symptoms resulting in a total immobilization and speech loss. All the symptoms of the disease can be removed by low doses of Nakom and reappeared upon the drug withdrawal. Tyrosine hydroxylase studies performed in these patients at various stages of the disease showed an unusual pattern of the enzyme activity differing from that seen in children with similar pathology failing to improve dramatically under the Nakom treatment.
