Early Voice and Swallowing Disturbance Incidence and Risk Factors After Revision Anterior Cervical Discectomy and Fusion Using a Multidisciplinary Surgical Approach: A Retrospective Cohort Evaluation of a Prospective Database.

BACKGROUND AND OBJECTIVES: Dysphagia and vocal cord palsy (VCP) are common otolaryngological complications after revision anterior cervical discectomy and fusion (rACDF) procedures. Our objective was to determine the early incidence and risk factors of VCP and dysphagia after rACDF using a 2-team approach. METHODS: Single-institution, retrospective analysis of a prospectively collected database of patients undergoing rACDF was enrolled from September 2010 to July 2021. Of 222 patients enrolled, 109 patients were included in the final analysis. All patients had prior ACDF surgery with planned

Multidisciplinary Management of Brain Metastases from Non-Small Cell Lung Cancer in the Era of Immunotherapy.

Brain metastases from non-small cell lung cancer often cause neurologic symptoms which lead to initial diagnosis or identification of recurrence. In other patients, they are identified on surveillance imaging or when a patient undergoing treatment develops neurological symptoms. Patients with symptomatic lesions should be started on dexamethasone and evaluated by a neurosurgeon as soon as possible. If feasible, surgery should be offered to decrease intracranial pressure, alleviate symptoms, and prevent irreversible neurological damage. Postoperative stereotactic radiosurgery (SRS) to the resec

Jugular bulb and skull base pathologies: proposal for a novel classification system for jugular bulb positions and microsurgical implications.

OBJECTIVE There is no definitive or consensus classification system for the jugular bulb position that can be uniformly communicated between a radiologist, neurootologist, and neurosurgeon. A high-riding jugular bulb (HRJB) has been variably defined as a jugular bulb that rises to or above the level of the basal turn of the cochlea, within 2 mm of the internal auditory canal (IAC), or to the level of the superior tympanic annulus. Overall, there is a seeming lack of consensus, especially when MRI and/or CT are used for jugular bulb evaluation without a dedicated imaging study of the venous ana

The long term outcome of micturition, defecation and sexual function after spinal surgery for cauda equina syndrome.

BACKGROUND: Cauda equina syndrome (CES) is a rare neurologic complication of lumbar herniated disc for which emergency surgical decompression should be undertaken. Despite the common belief that the restoration of functions that are affected by CES can take several years postoperatively, follow up seldom exceeds the first year after surgery. Long term outcome of especially micturition, defecation and sexual function-which are by definition affected in CES-are unknown. The aim of this study is to evaluate 1) postoperative long term outcome of micturition, defecation and sexual function in CES p

Residual tumor volume versus extent of resection: predictors of survival after surgery for glioblastoma.

OBJECT: The impact of extent of resection (EOR) on survival for patients with glioblastoma (GBM) continues to be a point of debate despite multiple studies demonstrating that increasing EOR likely extends survival for these patients. In addition, contrast-enhancing residual tumor volume (CE-RTV) alone has rarely been analyzed quantitatively to determine if it is a predictor of outcome. The purpose of this study was to evaluate the effect of CE-RTV and T2/FLAIR residual volume (T2/F-RV) on overall survival. METHODS: A retrospective review of 128 patients who underwent primary resection of supr

Extra- and intracerebral course of the recurrent artery of Heubner.

BACKGROUND: The aim of the current study was to analyse the extra- and intracerebral course of the recurrent artery of Heubner (RAH) to provide detailed information for neurosurgeons operating in this area. MATERIALS AND METHODS: The material for this study was obtained from cadavers (ages 31-75 years) during routine autopsies. A total of 70 human brains (39 male and 31 female) were examined. The material was collected not later than 48 h post-mortem. People who died due to neurological disorders were not included into the study. Right after dissection the arteries were perfused with either a…

Skull base tumor model.

OBJECT: Resident duty-hours restrictions have now been instituted in many countries worldwide. Shortened training times and increased public scrutiny of surgical competency have led to a move away from the traditional apprenticeship model of training. The development of educational models for brain anatomy is a fascinating innovation allowing neurosurgeons to train without the need to practice on real patients and it may be a solution to achieve competency within a shortened training period. The authors describe the use of Stratathane resin ST-504 polymer (SRSP), which is inserted at different…

Endoscopic third ventriculostomy for obstructive hydrocephalus due to intracranial hemorrhage with intraventricular extension.

Object Endoscopic third ventriculostomy (ETV) is well accepted for obstructive hydrocephalus of various etiologies. Nevertheless, it is seldom considered in intracranial hemorrhage even in cases involving obstruction of the CSF circulation. Methods Between May 1993 and April 2008, 34 endoscopic procedures were performed for hemorrhage-related obstructive hydrocephalus with an intraventricular component. All patients were prospectively followed up. Special attention was paid to presurgical clinical status, type of hemorrhage, type of surgery, postsurgical clinical status, postsurgical ventricul

Surgical anatomy of the cervical and infraclavicular parts of the long thoracic nerve.

OBJECT: There is insufficient information in the neurosurgical literature regarding the long thoracic nerve (LTN). Many neurosurgical procedures necessitate a thorough understanding of this nerve's anatomy, for example, brachial plexus exploration/repair, passes for ventriculoperitoneal shunt placement, pleural placement of a ventriculopleural shunt, and scalenotomy. In the present study the authors seek to elucidate further the surgical anatomy of this structure. METHODS: Eighteen cadaveric sides were dissected of the LTN, anatomical relationships were observed, and measurements were obtaine

Complications of angioma surgery--personal experience in 191 patients with cerebral angiomas.

In the last years, treatment decisions of arteriovenous malformations (AVMs) were influenced by the improvement of stereotactic radiosurgery and were revolutionized by development of embolization techniques. The aim of this report was to examine the results, effectiveness, and complications associated with angioma surgery. 191 patients with AVMs were operated by the first author between 1981 and 1996. Angioma localization was distributed as follows: frontal 51 (26.7%), temporal 44 (23%), parietal 45 (23.6%), and occipital 24 (12.6%). Twelve (6.3%) AVMs were located in the cerebellum and 15 (7.

Reanimation of the paralyzed face.

The challenge of reconstruction in the paralyzed face is to provide symmetry both at rest and in active expression. Although functional considerations must take precedence, the patient with unilateral facial palsy faces social stigmata that are exceptionally difficult. The best reconstructions in late paralyses fall far short of natural facial expression. Conley, one of the pioneers in facial nerve rehabilitation, reflected the frustration of dealing with limited techniques: It has been assumed by many surgeons that involuntary emotional communication is through the facial nerve, but this has

Neurosurgical management of paediatric central nervous system tumours in low, middle and high-income countries: a multi-centre, international, cross-sectional study.

Paediatric central nervous system (CNS) tumours remain a leading cause of cancer-related mortality worldwide. This may partly be because the distribution of vital diagnostic and therapeutic resources is highly uneven across the world. This study characterised paediatric neuro-oncology services across countries of differing economic and development statuses to identify potential sources of disparities in care. This web-based survey was completed by doctors managing CNS tumours at different healthcare facilities internationally. Questions covered participant demographics, institution type, insti
