Craniofacial characteristics in unilateral complete cleft lip and palate patients with congenitally missing teeth.

INTRODUCTION: Congenitally missing permanent teeth are common in patients with clefts. This retrospective study was conducted to evaluate the craniofacial characteristics in patients with unilateral complete cleft lip and palate with congenitally missing permanent teeth. METHODS: A series of 73 consecutive patients with nonsyndromic unilateral complete cleft lip and palate were enrolled. Evaluation of congenitally missing permanent teeth was based on the panoramic films taken from 7 to 11 years of age. The cephalometric films taken around 9 years of age were used to compare the craniofacial m

Evaluation of the severity of malocclusions in children affected by osteogenesis imperfecta with the peer assessment rating and discrepancy indexes.

INTRODUCTION: Osteogenesis imperfecta is a heritable disorder affecting bone and tooth development. Malocclusion is frequent in those affected by osteogenesis imperfecta, but this has not been studied in detail. The purpose of this study was to describe and quantify the severity of malocclusions in patients with osteogenesis imperfecta. METHODS: Articulated dental casts were obtained from 49 patients diagnosed with osteogenesis imperfecta (ages 5-19 years; 28 female) and 49 age- and sex-matched control subjects who did not have osteogenesis imperfecta. Both groups were seeking orthodontic tre

Correction of an asymmetric maxillary dental arch by alveolar bone distraction osteogenesis.

This case report describes a new surgical orthodontic approach involving alveolar bone distraction osteogenesis for correction of an asymmetric maxillary dental arch. The treatment was combined with conventional orthognathic surgery to improve the mandibular lateral deviation. This new treatment strategy produced an ideal dental arch and a symmetric facial appearance efficiently and effectively.

Patient with oligodontia treated with a miniscrew for unilateral mesial movement of the maxillary molars and alignment of an impacted third molar.

This report describes the treatment of a 20-year-old woman with a dental midline deviation and 7 congenitally missing premolars. She had retained a maxillary right deciduous canine and 4 deciduous second molars, and she had an impacted maxillary right third molar. The maxillary right deciduous second molar was extracted, and the space was nearly closed by mesial movement of the maxillary right molars using an edgewise appliance and a miniscrew for absolute anchorage. The miniscrew was removed, and the extraction space of the maxillary right deciduous canine was closed, correcting the dental mi

Asymmetrical distalization of maxillary molars with zygomatic anchorage, improved superelastic nickel-titanium alloy wires, and open-coil springs.

INTRODUCTION: In nongrowing patients with skeletal Class II malocclusion, premolar extraction or maxillary distalization can be used as camouflage treatment. Zygomatic anchorage enables distalization in uncooperative or noncompliant patients. We describe 1 such procedure in a 24-year-old woman. METHODS: We used novel improved superelastic nickel-titanium archwires combined with nickel-titanium open-coil springs to provide a constant and continuous low force to the dentition. RESULTS: We were able to successfully eliminate the protrusive profile and correct the Class II molar relationship usi

Is the growth pattern in mouth breathers comparable with the counterclockwise mandibular rotation of nasal breathers?

INTRODUCTION: The aim of this longitudinal study was to investigate mandibular rotation and angular remodeling in mouth-breathing children compared with nasal-breathing children. METHODS: The sample included 55 severely obstructed mouth-breathing children (mean age, 6.1 years) and 55 nasal-breathing children (mean age, 7.3 years). The cephalograms at baseline and after 1 year were traced and measured, and superimpositions were made to determine true mandibular rotation, apparent rotation, and angular remodeling. The significance level was set at 5%. RESULTS: The mouth-breathing children had

Changes in the pattern of patients receiving surgical-orthodontic treatment.

INTRODUCTION: The characteristics of patients who seek and accept orthognathic surgery appear to be changing over time but have not been well documented in the 21st century. METHODS: Records for patients who had orthognathic surgery at the University of North Carolina from 1996 to 2000 and from 2006 to 2010 were reviewed to collect data for changes in the prevalence of patients with mandibular deficiency (Class II), maxillary deficiency or mandibular prognathism (Class III), long face, and asymmetry problems. The changes were compared with those in previous time periods and at other locations

Botulinum neurotoxin type A in the masseter muscle: effects on incisor eruption in rabbits.

INTRODUCTION: Botulinum neurotoxins are responsible for the paralytic food poisoning, botulism. Commercial formulations such as botulinum neurotoxin type A are increasingly used for various conditions, including cosmetic recontouring of the lower face by injection of the large masseter muscles. The paralysis of a major muscle of mastication lowers occlusal force and thus might affect tooth eruption. The purpose of this study was to investigate the effects of unilateral masseter muscle injection of botulinum neurotoxin type A on the rate of eruption of incisors in a rabbit model. We hypothesize

Long-term skeletal and dental stability of mandibular symphyseal distraction osteogenesis with a hybrid distractor.

INTRODUCTION: The purpose of this study was to examine the long-term skeletal and dental stability of mandibular symphyseal distraction osteogenesis (MSDO) with a tooth-borne and bone-borne hybrid distractor. To differentiate the effects of MSDO from the orthodontic movement and relapse, each phase of treatment was analyzed. METHODS: Twenty-five patients were included in the study, ranging in age from 12.0 to 30.9 years at the initiation of treatment (mean, 15.8 ± 4.8 years). Of this group, 16 patients were recalled at a mean of 7.5 ± 0.9 years (range, 6.3-9.6 years) after distraction for lon

Effects of rapid maxillary expansion in hyperdivergent patients.

INTRODUCTION: This retrospective cohort study was performed to evaluate the skeletal and dental changes in the short and long terms in hyperdivergent patients treated with rapid maxillary expansion and fixed appliances. METHODS: The sample consisted of 143 patients who had rapid maxillary expansion with a Haas-type expander followed by edgewise therapy. Two groups were established: a normal vertical dimension group (mandibular plane angle >20° and <27°; n = 52) and a hyperdivergent group (mandibular plane angle ≥27°; n = 91). Lateral cephalograms were taken before treatment (average age, 11.5

Mandibular anterior bony support and incisor crowding: Is there a relationship?

INTRODUCTION: The aim of this study was to test the null hypothesis that increased irregularity of the mandibular incisors is associated with a reduction in the alveolar support on cone-beam computed tomographic sections. METHODS: From a sample of 1100 digital volumetric tomographs, 125 tomographs of subjects with Class I malocclusion (mean age, 21.6 ± 4.8 years) were selected for this study. An irregularity index was used to categorize these tomographs as having mild, moderate, or severe crowding. All tomographs were taken by using an iCAT (Imaging Sciences International, Hatfield, Pa) imagi

Orthodontics and temporomandibular disorders: a curriculum proposal for postgraduate programs.

In a previous article, we reported the results of a survey of American and Canadian orthodontic postgraduate programs to determine how the topics of occlusion, temporomandibular joint, and temporomandibular disorders were currently being taught. Based on the finding of considerable diversity among those programs, we decided to write a curriculum proposal for temporomandibular disorders that would be compatible with and satisfy the current curriculum guidelines for postgraduate orthodontic programs. These guidelines arose from a combination of the requirements published by the American Dental A

Three-dimensional retraction of anterior teeth with orthodontic miniplates in patients with temporomandibular disorder.

Vertical dimension control is critical for adults with Class II high-angle malocclusions. If the patient has a temporomandibular disorder, this requirement is exacerbated. When extraction is planned, the treatment challenge is further increased. This article reports on a new biomechanical design (C-therapy) appropriate for anterior segment retraction with excellent control of the vertical dimension, even in a Class II high-angle patient with a temporomandibular disorder. Maximum retraction of the maxillary anterior segment can be achieved with a lingual retractor and a palatal miniplate withou

Role of CCR2 in orthodontic tooth movement.

INTRODUCTION: Cytokines and chemokines regulate bone remodeling during orthodontic tooth movement. CC chemokine ligand 2 (CCL2) is involved in osteoclast recruitment and activity, and its expression is increased in periodontal tissues under mechanical loading. In this study, we investigated whether the CC chemokine receptor 2 (CCR2)-CCL2 axis influences orthodontic tooth movement. METHODS: A coil spring was placed in CCR2-deficient (CCR2(-/-)), wild-type, vehicle-treated, and P8A-treated (CCL2 analog) mice. In a histopathologic analysis, the amounts of orthodontic tooth movement and numbers o

Impact of verbal explanation and modified consent materials on orthodontic informed consent.

INTRODUCTION: Comprehension of informed consent information has been problematic. The purposes of this study were to evaluate the effectiveness of a shortened explanation of an established consent method and whether customized slide shows improve the understanding of the risks and limitations of orthodontic treatment. METHODS: Slide shows for each of the 80 subject-parent pairs included the most common core elements, up to 4 patient-specific custom elements, and other general elements. Group A heard a presentation of the treatment plan and the informed consent. Group B did not hear the presen
