Parotid gland sparing in patients undergoing bilateral head and neck irradiation: techniques and early results.

PURPOSE: To minimize xerostomia in patients receiving bilateral head and neck irradiation (RT) by using conformal RT planning to spare a significant volume of one parotid gland from radiation. METHODS AND MATERIALS: The study involved 15 patients with head and neck tumors in whom bilateral neck radiation was indicated. The major salivary glands and the targets (tumor, surgical bed, metastases to lymph nodes, and the locations of lymph nodes at risk for metastases) were outlined on axial computed tomography images. Beam's-eye view (BEV) displays were used to construct conformal beams that deli

The value of sonography in the management of cystic neck lesions.

Eight cases of cystic neck lesion of differing causes in whom sonography was carried out as part of investigation are described. These are thyroid cyst with haemorrhage, thyroglossal cyst, branchial cyst, cystic hygroma, parotid abscess of the neck secondary to malignant external otitis, cystic degeneration of a recurrent malignancy in the neck, sebaceous cyst and a cold abscess. They showed that sonography is better than clinical judgement in obtaining a diagnosis. Therefore, we recommend that ultrasound should be performed in all neck lesions prior to needle aspiration or open excisional bio

Cervicofacial actinomycosis.

Actinomycotic infections are unusual, but the actual incidence is likely to be significantly higher than records indicate. The disease may complicate trauma of many types to the respiratory and digestive tracts, including operative procedures. This possibility should encourage more frequent use of anaerobic cultures in inflammatory diseases, particularly posttraumatic, and should prompt consideration of actinomycosis in the differential diagnosis of infections, especially in the cervicofacial area. We report four cases that demonstrate the variable course of this infection. Treatment is highly

Changing trends in the epidemiology and surgical treatment of benign parotid gland tumours: a 10-year retrospective comparison in a tertiary referral centre of southeast Bavaria.

BACKGROUND: Benign salivary gland tumors are in general rare. Pleomorphic adenomas (PA) and Warthin’s tumors (WT) make up to 90% of these tumors. Their epidemiology shows significant regional differences but also changes over time. In addition to that, new developments in the surgical approach, like extracapsular dissection (ED), have been introduced. METHODS: We retrospectively analyzed epidemiological and clinical data of 347 patients (184 male and 163 female) treated in the ENT-Department of the University Hospital of Regensburg in 2010/11 and 2020/21 and assessed their changes over time. Could a multimodal fusion model integrating CT radiomics and systemic inflammatory markers improve preoperative risk stratification of parotid masses? A retrospective exploratory study.

OBJECTIVES: To develop and internally validate a multimodal fusion model integrating clinical features, lymphocyte-to-monocyte ratio (LMR), and venous-phase CT radiomic data for preoperative benign-versus-malignant risk stratification of parotid masses (PMs), and to exploratorily evaluate its potential adjunctive role relative to standard diagnostic modalities including fine-needle aspiration biopsy (FNAB). METHODS: A retrospective analysis of 490 patients with histopathologically confirmed PMs (2013-2022) was performed, with random assignment to training (70%) and internal validation (30%) c

Dosimetric Results for Organs at Risk From Radiation Therapy Dose De-Escalation to the Subclinical Region in Human Papillomavirus-Associated Oropharyngeal Cancer: A Propensity Score-Matched Analysis.

PURPOSE: The impact of radiation therapy (RT) dose de-escalation on organs at risk (OARs) for human papillomavirus-associated oropharyngeal cancer (OPC) is not known. This study retrospectively assessed OAR dosimetric outcomes of de-escalated RT for OPC. METHODS AND MATERIALS: We conducted a retrospective analysis of dosimetric data for patients with T1 to 4, N0 to 3 human papillomavirus-associated OPC treated with intensity modulated RT at a single academic institution between August, 2014 and January, 2023. All patients received 70 Gy to gross disease. The low-dose group received 30 Gy to t

Competing-risks nomograms for predicting cause-specific mortality in parotid-gland carcinoma: A population-based analysis.

INTRODUCTION: Parotid-gland carcinoma (PGC) is a relatively rare tumor that comprises a group of heterogeneous histologic subtypes. We used the Surveillance, Epidemiology, and End Results (SEER) program database to apply a competing-risks analysis to PGC patients, and then established and validated predictive nomograms for PGC. METHODS: Specific screening criteria were applied to identify PGC patients and extract their clinical and other characteristics from the SEER database. We used the cumulative incidence function to estimate the cumulative incidence rates of PGC-specific death (GCD) and

FNA diagnostics of secondary malignancies in the salivary gland: Bi-institutional experience of 36 cases.

OBJECTIVE: Fine-needle aspiration (FNA) is a key diagnostic method in the evaluation of salivary gland lesions. Secondary tumors of salivary glands represent only 5% of all malignancies of major salivary glands. The goal of our study was to examine the cytological and clinical features of secondary tumors sampled by FNA. MATERIALS AND METHODS: A series of 36 secondary tumors from the pathology departments of two university hospitals are presented. Clinical referrals to FNA, cytological features, immunohistochemical results, and histopathological diagnoses were reviewed in all cases. RESULTS:

Partial Superficial Parotidectomy With Retrograde Dissection of the Facial Nerve for Clinically "Benign" Parotid Tumors.

BACKGROUND: To review experience with partial superficial parotidectomy (PSP) and retrograde dissection of the facial nerve as a treatment for benign parotid tumors. METHODS: Retrospective cohort study of all patients presenting with a suspected benign primary parotid tumor undergoing parotid surgery. RESULTS: There were 214 cases retrieved. Postoperative facial nerve weakness occurred in 33% of patients; all were temporary. Increased extent of surgical resection (P < .001), deeper tumors (P = .05), and close tumor proximity to the facial nerve (P = .007) significantly correlated with postop

Growth rate characteristics of Warthin's tumours of the parotid gland.

The aim of this study was to evaluate growth characteristics of parotid gland Warthin's tumours. The medical records of 134 patients who had a cytological or histopathological diagnosis of Warthin's tumour between 1997 and 2013, at a single tertiary care centre, were reviewed retrospectively. Thirteen of these patients underwent observation with 30 serial computed tomography or magnetic resonance imaging scans of the head and neck, with 24 Warthin's lesions identified. The mean length of time between scans was 882 days, and mean initial and final sizes per lesion were 3.9 cm(3) and 5.6 cm(3),

Parotid glands in whole-brain radiotherapy: 2D versus 3D technique for no sparing or sparing.

PURPOSE: Whole-brain radiotherapy is performed as standard therapy in patients with multiple brain metastases and in patients with more favourable prognosis such as lymphoma and small-cell lung carcinoma in the prophylactic setting. Because of the prognosis and the total prescribed dose, the parotid glands are not usually regarded as an organ at risk in whole-brain radiotherapy. Long-term survival can be expected in some of these patients and late toxicity can be observed, becoming essential to evaluate organs at risk, particularly the parotid glands even in metastatic patients. We compared th

Perfusion characteristics of parotid gland tumors evaluated by contrast-enhanced ultrasound.

PURPOSE: Contrast enhanced ultrasound (CE-US) is a promising imaging modality for non-invasive analysis of parotid gland lesions because their vascularisation differs from normal gland tissue. This clinical study should further investigate CE-US as a diagnostic tool for parotid gland tumors. MATERIALS AND METHODS: 39 patients underwent CE-US measurements after intravenous application of a contrast agent (SonoVue, Bracco, Italy) before surgical tumor resection. Time-intensity curves gradients were calculated and parameters of intratumoral microcirculation were analysed. The vascularisation par

[Volumetric and geometric changes of parotids occurring during IMRT for nasopharyngeal carcinoma (NPC) using daily CBCT].

OBJECTIVE: To study the volumetric and geometric changes of parotids occurring during IMRT treatment for NPC. METHODS: The pre-treatment CBCT images of 10 patients were obtained using kV-CBCT and transmitted back to TPS. The gross tumor volumes (GTVs) and volumes of parotids were manually drawn slice by slice on the CBCT images by the same person using image analysis software system Tomcon 3.0. For position changes, the center-of-mass (COM) were calculated using Pinnacle 8.0 software. Then the distance from the parotid to the atlas was assessed. RESULTS: The volumetric changes of GTVs and pa

Preoperative fine needle aspiration cytologic diagnosis of spindle cell myoepithelioma of the parotid gland: a case report.

BACKGROUND: There are only rare case reports of preoperative fine needle aspiration cytologic (FNAC) diagnosis of myoepithelioma of the salivary gland. Myoepitheliomas with pure spindle cell morphology may simulate a variety of benign or malignant spindle cell soft tissue tumors. CASE: A 54-year-old woman presented with a history of progressively increasing swelling in the right parotid region. The clinical diagnosis was parotid malignancy. Routine FNAC yielded highly particulate material. The smears were cellular, with tissue fragments, clusters of spindle cells and numerous small globules a

Assessment of absorbed dose to thyroid, parotid and ovaries in patients undergoing Gamma Knife radiosurgery.

Stereotactic radiosurgery was originally introduced by Lars Leksell in 1951. This treatment refers to the noninvasive destruction of an intracranial target localized stereotactically. The purpose of this study was to identify the dose delivered to the parotid, ovaries, testis and thyroid glands during the Gamma Knife radiosurgery procedure. A three-dimensional, anthropomorphic phantom was developed using natural human bone, paraffin and sodium chloride as the equivalent tissue. The phantom consisted of a thorax, head and neck and hip. In the natural places of the thyroid, parotid (bilateral si
