BACKGROUND: Lower trapezius transfer (LTT) has been demonstrated to restore external rotation (ER) in patients with brachial plexus palsies. In certain cases of cuff tear arthropathy, patients exhibit preserved forward elevation but lack active ER. This study evaluates the clinical outcomes of LTT, augmented with a semitendinosus tendon autograft and anchored to the infraspinatus insertion using an arthroscopically assisted technique and, to determine whether pain relief and functional improvement are maintained at minimum 7.5-year follow-up compared to the 11-month follow-up.

BACKGROUND: Irreparable rotator cuff tears remain a challenge for orthopedic surgeons. Reverse shoulder arthroplasty (rTSA) has become an increasingly common option for these patients. However, in patients lacking arthritis and with a functionally balanced shoulder, alternative and less invasive surgical options have been pursued. In this study, we seek to investigate the outcome of arthroscopically placing an allograft on the undersurface of the acromion for pain relief.
METHODS: A retrospective review was conducted of all patients who underwent a bursal acromial reconstruction using a derma

OBJECTIVE: This study evaluated articular cartilage degeneration in the humeral head using a multimodal approach integrating Raman spectroscopy, mechanical testing, biochemical analysis, and micro-computed tomography (µCT). The goal was to identify sensitive biomarkers of early matrix-level degeneration that precede overt radiographic changes in glenohumeral osteoarthritis (OA).
METHODS: Eleven humeral heads were collected from cadaveric shoulders and stratified by the presence of a focal chondral defect and rotator cuff (RC) tear status, yielding four specimens without visual cartilage degen

PURPOSE: To investigate whether 2 months of a postoperative intra-articular corticosteroid injection after arthroscopic rotator cuff repair (ARCR) in rotator cuff tear (RCT) patients with stiffness would improve the functional scores and range of motion of the operated shoulder without capsular release during ARCR.
METHODS: From March 2020 to September 2021, a total of 73 patients who had RCT with stiffness and were scheduled for ARCR were enrolled prospectively and randomly allocated into 2 groups. The patients who underwent ARCR with capsular release were allocated to group 1 (n = 37). The

BACKGROUND: Although a recent systematic review found that latissimus dorsi tendon transfer (LDTT) granted comparable outcomes in shoulders with massive rotator cuff tears (mRCTs) versus those with failed rotator cuff repair (RCR), some studies found inferior outcomes after failed RCR.
PURPOSE: To compare the clinical and functional outcomes, as well as complication rates, of patients who underwent LDTT for the treatment of mRCTs or failed RCR.
STUDY DESIGN: Case series; Level of evidence, 4.
METHODS: The authors retrieved the records of a consecutive series of 258 patients (n = 150, arthro

OBJECTIVE: To evaluate the humeral head bone volume of patients with cuff tear arthropathy (CTA) and examine the therapeutic effect of zoledronate in a rat modified model of CTA (mCTA).
DESIGN: The bone mass in patients with CTA was measured using Hounsfield units from CT images. The mCTA was induced by transecting the rotator cuff, biceps brachii tendon, and superior half of the joint capsule in adult rat shoulders. A single subcutaneous injection of zoledronate was followed by bone histomorphometry and immunohistochemistry of the humeral head, as well as the Murine Shoulder Arthritis Score

INTRODUCTION: Repeated transfers and wheelchair propulsion in patients with a neurological deficit of the lower limbs overloads the upper limbs mechanically, particularly the shoulders, which become weight-bearing. Under these conditions, arthroplasty implants are subjected to large stresses, even though this indication is controversial in such a context. We hypothesized that joint replacement in weight-bearing shoulders will relieve pain and improve range of motion, with a positive impact on function and autonomy, without increasing the complication rate relative to the able-bodied population

PURPOSE: The primary aims of this study were to (1) assess the preoperative expectations of patients undergoing shoulder surgery, and (2) determine the relationship between preoperative patient demographics, PROs, and preoperative patient expectations. It was hypothesized that younger patients with worse function and worse health status had higher expectations of shoulder surgery.
METHODS: Data from a total of 319 patients (319 shoulders) from 2015-2018 were analyzed. Patients completed a series of questionnaires covering demographics and patient-reported outcome measures. Expectations of tre

BACKGROUND: Supraspinatus (SSP) tendon ruptures requiring surgical repair are common. Arthroscopic suture anchor fixation has gradually replaced transosseous repair in supraspinatus tendon tear. Our objective was to compare mechanical properties between transosseous and anchor supraspinatus repair in the first 6 postoperative weeks in a rabbit model.
METHODS: One hundred and fifty-two rabbits had one supraspinatus tendon repaired either with an anchor suture 1 week after detachment or with transosseous sutures. Rabbits were euthanized at 0, 1, 2, 4 or 6 postoperative weeks. Experimental and c

Post-capsulorrhaphy arthropathy is a long-term sequela that can develop after open anterior stabilization surgeries, which have historically been performed. The purpose of this study was to compare anatomic total shoulder arthroplasty (ATSA) with reverse total shoulder arthroplasty (RTSA) for the treatment of post-capsulorrhaphy arthropathy. There were 19 patients in the ATSA cohort and 20 in the RTSA cohort. All patients included had a minimum of 2 years of follow-up (ATSA mean, 59.5 months; RTSA mean, 43.8 months). Patients were followed clinically (American Shoulder and Elbow Surgeons score

BACKGROUND: The chondral print (CP) sign is a chondral change on the humeral head underneath the long head of the biceps (LHB) tendon. Several suggested causative links have been described, but the pathologic mechanism remains unclear.
METHODS: We designed this prospective cohort association study of 102 consecutive shoulder arthroscopies to investigate proposed associations of CP with LHB, rotator cuff, labral pathology, and other chondral lesions. Data collection was by a specifically designed pro forma, and statistical analysis was performed.
RESULTS: We identified 24 patients (23.5%) wit

CONTEXT: Muscle fatigue due to repetitive and prolonged overhead sports activity is considered an important factor contributing to impingement-related rotator cuff pathologic conditions in overhead athletes. The evidence on scapular and glenohumeral kinematic changes after fatigue is contradicting and prohibits conclusions about how shoulder muscle fatigue affects acromiohumeral distance.
OBJECTIVE: To investigate the effect of a fatigue protocol resembling overhead sports activity on acromiohumeral distance and 3-dimensional scapular position in overhead athletes.

Arthroscopic acromioplasty has become a common procedure for the treatment of chronic impingement syndrome. Short-term studies have consistently shown good results; however, long-term data have not previously been reported in the literature. Of 102 consecutive patients undergoing arthroscopic acromioplasty at our institution from 1984 to 1988, 82 patients (83 shoulders) were available for evaluation. The patients were reviewed after a mean of 8 years, 5 months from their original surgery. There were 16 failures (19%) with unsatisfactory results following surgery; 13 patients (15%) required fur

BACKGROUND: There is an ever-increasing demand for widespread implementation of patient-reported outcomes. However, adoption is slow owing to limitations in clinical infrastructure and resources within busy orthopaedic practices. Prior studies showed the single alpha-numeric evaluation (SANE) score to correlate at a single point in time with the American Shoulder and Elbow Surgeons (ASES) score. However, no study has validated the SANE in terms of test-retest reliability, responsiveness, or clinical utility.
PURPOSE: To validate SANE with the ASES across a sample of patients with common ortho

CONTEXT: The body of knowledge concerning shoulder kinematics in patients with rotator cuff tears is increasing. However, the level of understanding regarding how pain and tear size affect these kinematic patterns is minimal.
OBJECTIVE: To identify relationships between pain associated with a full-thickness rotator cuff tear, tear size, and scapulohumeral rhythm (SHR) and to determine whether pain and tear size serve as predictors of SHR.
DESIGN: A test-retest design was used to quantify pain and SHR before and after a subacromial lidocaine injection. Correlation and multivariate analyses we
