Use of comb-filtered speech to demonstrate to parents how children adapt to hearing with an implant.

Parents' knowledge of cochlear implantation (CI) is very limited. How could we graphically demonstrate the problems faced by implanted children to their parents?Parallels between speech perception of implanted patients and speech perception after comb filtration by normal hearing persons mean that it is possible to evaluate self-training of inexperienced subjects to perceive spectrally deprived speech and to show the complexities of implanted children's acoustic perception to the children's relatives. Speech consisted of five bands of the 50 Hz width in the frequency range of 200 to 6250 Hz. E…

Decision making for solitary vestibular schwannoma and contralateral Meniere's disease.

The existence of dual inner ear pathology such as unilateral Meniere's disease (MD) with a contralateral vestibular schwannoma (VS) is very rare, but provides the otologist with a significant management dilemma. In this study, we present 5 cases of unilateral disabling MD with a contralateral VS in the better hearing ear. Conservative management of the VS is mandated unless there are impending complications, with management directed toward controlling the vertigo attributed to MD. If and when the VS requires intervention, or the hearing in that ear deteriorates to unserviceable levels, cochlea

BACKGROUND AND EVOKED ACTIVITY IN THE AUDITORY PATHWAY: EFFECTS OF NOISE-SHOCK PAIRING.

Unanesthetized cats with electrodes permanently implanted along the auditory pathway were presented first with suistained " white" noise stimuli and then with the same noise paired with subcutaneous electric shocks. Pairing noise with shock decreased both background and noise-evoked activity inthe inferior colliculus, and, in some cats, also in the cochlear nucleus, trapezoid body, and at the round window. No changes occurred in medial geniculate or auditory cortex recordings. The effects in the inferior colliculusdo not depend on changes in the degree of arousal of the animal, on change in th

Functional near-infrared spectroscopy (fNIRS) biomarkers of tinnitus severity within tinnitus subtypes.

Tinnitus, the perception of sounds not present externally, can significantly impair quality of life. Currently there are no clinically available objective measures to enable monitoring of tinnitus-related changes in brain activity. Our previous work demonstrated sensitivity of a non-invasive imaging technique called functional near-infrared spectroscopy (fNIRS) to changes in brain activity associated with tinnitus presence and severity. In this study we aimed to assess whether fNIRS features could quantify tinnitus severity on a continuous scale, a requirement for effective monitoring of treat

[Acoustic neurinoma with preserved inner ear function. Interesting audiologic constellation of findings].

Neurofibromatosis (NF) is a syndrome with multiple neuromas. With the central form or NF2 very often bilateral acoustic neuromas are found. Tumours derive from Schwann cells of the eighth cranial nerve; symptoms are tinnitus, hearing loss and vertigo. Therapy is surgical, if possible with the protection of remaining hearing function. We report the case of a 28 year old female patient with neurofibromatosis type 2. After complete resection of the left AN and partial resection of the right AN the patient suffered from bilateral complete deafness, although almost normal DPOAE can be recorded on o

Application of Artificial Intelligence in Otology: Past, Present, and Future.

Artificial Intelligence (AI) is a concept whose goal is to imitate human intellectual activity in computers. It emerged in the 1950s and has gone through three booms. We are in the third boom, and it will continue. Medical applications of AI include diagnosing otitis media from images of the eardrum, often outperforming human doctors. Temporal bone CT and MRI analyses also benefit from AI, with segmentation accuracy improved in anatomically significant structures or diagnostic accuracy improved in conditions such as otosclerosis and vestibular schwannoma. In treatment, AI predicts hearing outc

X-Linked Gusher Disease DFNX2 in Children, a Rare Inner Ear Dysplasia with Mixed Hearing and Vestibular Loss.

Conductive hearing losses are typically present in disorders of the external/middle ear. However, there is a rare group of inner ear conditions called third windows that can also generate a conductive hearing loss. This is due to an abnormal connection between the middle and the inner ear or between the inner ear and the cranial cavity. X-linked gusher disorder is an extremely rare congenital inner ear dysplastic syndrome with an abnormal connection due to a characteristic incomplete cochlear partition type III and an incomplete internal auditory meatus fundus. The disorder is inherited in an

Microscopic characteristics of round window problems in otology.

Round window membrane ruptures and perilymph leakage can present the complication or concomitant of sudden deafness and barotrauma. Pathological involvement of the round window membrane can occur in many otological diseases such as otosclerosis, otitis media, tumors, etc. Microscopic manipulations might result in an inadvertent round window membrane lesion and cochlear deafness. In recent years purposeful surgical lesions, including procedures for vertigo and cochlear implant surgery, are being made in the round window membrane. Examples of pathology involving the round window in human tempora

Effect of radiotherapy on retro-cochlear auditory pathways.

OBJECTIVES: The cochlea may be damaged by modern conventional radiotherapy (RT) for head and neck cancers when the ear is included in the radiation field. It is unclear however, if the retro-cochlear auditory pathways are affected as well, which has clinical significance in cochlear implantation. This study aims to investigate the integrity of the retro-cochlear auditory pathways in patients who had received RT for nasopharyngeal carcinoma. STUDY DESIGN: Prospective study. METHODS: Patients who were newly diagnosed with nasopharyngeal carcinoma and treated by RT alone were studied. Evoked re…

Wireless Power Transfer Strategies for Implantable Bioelectronics.

Neural implants have emerged over the last decade as highly effective solutions for the treatment of dysfunctions and disorders of the nervous system. These implants establish a direct, often bidirectional, interface to the nervous system, both sensing neural signals and providing therapeutic treatments. As a result of the technological progress and successful clinical demonstrations, completely implantable solutions have become a reality and are now commercially available for the treatment of various functional disorders. Central to this development is the wireless power transfer (WPT) that h

Temporal bone investigations on landmarks for conventional or endosteal insertion of cochlear electrodes.

CONCLUSION: Our anatomical findings place special emphasis on the requirement to follow an infero-anterior approach to the round window, to expose the scala tympani safely for 'normal' cochlear implantation. It is also known how easily the basilar membrane may be accidentally damaged, despite exercising considerable caution in the approach used. With regard to an 'endosteal electrode' it can be stated that there are no really specific indicators to locate the spiral ligament, or each of the scalae, on the lateral aspect of the tissue layer encasing the cochlea. For the concept of an endosteal …
