John Howship (1781-1841) and growing skull fracture: historical perspective.

In the late 18th and early 19th centuries, Dr. John Howship, a pioneering British surgeon, described the clinical features and pathophysiology of various surgical disorders of the human body. His critical contributions to pediatric neurosurgery came in 1816 when he first described the features of an important childhood condition following head trauma, what he referred to as parietal bone absorption. This condition as depicted by Dr. Howship was soon to be christened by later scholars as traumatic cephalhydrocele, traumatic meningocele, leptomeningeal cyst, meningocele spuria, fibrosing osteiti

Lumbar radiculopathy in ankylosing spondylitis with dural ectasia.

We present a 57-year-old man with ankylosing spondylitis (AS) and significant dorsal lumbar dural ectasia (diverticulum). An L5 nerve root monoradiculopathy associated with AS has not been previously reported. The quantity and extent of such ectasia is variable but may be related to cauda equina syndrome. Long-term follow-up of these cases may reveal whether or not solitary nerve lesions gradually lead to cauda equina syndrome. We recommend that asymptomatic or symptomatic patients with dural ectasia should be closely observed without need for immediate surgical intervention.

Gabapentin for treatment of pain and tremor: a large case series.

BACKGROUND: Several anticonvulsant agents, including carbamazepine, phenytoin, and valproate, are effective in some patients for the treatment of pain and tremor. This study reports on a trial of the newly introduced anticonvulsant, gabapentin, for pain and tremor control. METHODS: A large case series of patients with centrally mediated pain, peripherally mediated pain, migraine, and tremor were treated in an open-label study with gabapentin (maximum of 2,700 mg/day). RESULTS: Thirty-nine patients (65%) had moderate-to-excellent improvement in symptoms, with the best responses occurring in p

Comparing Glymphatic Function Measures: Diffusion Tensor Image Analysis Along Perivascular Spaces (DTI-ALPS) versus Intrathecal Contrast-Enhanced MRI.

Background Diffusion-tensor image (DTI) analysis along the perivascular space (ALPS) is a noninvasive but not well-validated method proposed to evaluate glymphatic function. Purpose To assess ALPS indexes across participant groups and compare them with measures of cerebrospinal fluid (CSF) tracer enrichment and clearance obtained by intrathecal contrast-enhanced MRI (glymphatic MRI [gMRI]). Materials and Methods This secondary analysis of a prospective observational study included participants who underwent clinical work-up for CSF disorders from September 2015 to December 2019. MRI scans were

Incidence and management of postoperative pseudomeningocele and cerebrospinal fluid leak after Chiari malformation type I decompression.

OBJECTIVE: The optimal surgical management of Chiari malformation type I (CM-I) remains controversial and heterogeneous. The authors sought to investigate patient-specific, technical, and perioperative features that may affect the incidence of CSF-related complications including pseudomeningocele and CSF leak at their institution. METHODS: The authors performed a single-center, retrospective review of all adult patients with CM-I who underwent posterior fossa decompression. Patient demographics, operative details, and perioperative factors were collected via electronic medical record review.

Alobar holoprosencephaly with cebocephaly in a neonate: A rare case report from Northern Tanzania.

INTRODUCTION AND IMPORTANCE: Holoprosencephaly is a rare brain malformation consisting of impaired midline cleavage of the embryonic forebrain presenting with variable features of craniofacial dysmorphism. It affects 1 in 10,000 live births occurring more in females than males. We present a case of alobar HPE and aim to raise awareness on the importance of early prenatal detection and counselling. CASE PRESENTATION: We present a case of 3200-gram female baby, born by spontaneous vaginal delivery with APGAR scores of 5 and 6 in the first and fifth minute of life respectively. On admission, the

Association of lipocalin-type prostaglandin D synthase with disproportionately enlarged subarachnoid-space in idiopathic normal pressure hydrocephalus.

BACKGROUND: Idiopathic normal pressure hydrocephalus (iNPH) is a treatable cause of dementia, gait disturbance, and urinary incontinence in elderly patients with ventriculomegaly. Its unique morphological feature, called disproportionately enlarged subarachnoid-space hydrocephalus (DESH), may also be a diagnostic feature. Lipocalin-type prostaglandin D synthase (L-PGDS) is a major cerebrospinal fluid (CSF) protein produced by arachnoid cells, and its concentration in the CSF is reportedly decreased in iNPH. L-PGDS acts as a prostaglandin D2-producing enzyme and behaves as a chaperone to preven

Recurrence of intracranial meningiomas: did better methods of diagnosis and surgical treatment change the outcome in the last 30 years?

OBJECTIVE: Meningiomas are benign intracranial tumors growing from the arachnoid cap cells. Although their behavior is usually benign, they tend to recur even after total removal, and their recurrence is dependent on different aspects. METHODS: Between 1991 and 2002, 463 patients with an intracranial meningioma were operated in the Department of Neurosurgery, University of Kiel, Kiel, Germany. We compared the outcome of these patients after operation and the different methods of radiation therapy and chemotherapy with the data from Buhl (1994), who analysed 661 patients with intracranial meni

Transumbilical approach for shunt insertion in the pediatric population: an improvement in cosmetic results.

OBJECTIVE: In spite of the recent interest in endoscopic third ventriculostomy, ventriculoperitoneal (VP) shunt is still the gold standard in treating non-obstructive hydrocephalus in children. The peritoneal cavity remains the optimal site for cerebrospinal fluid (CSF) diversion. Shunt insertion and re-interventions carry a high risk of inaesthetic abdominal scars and long-term morbidity. We report a technique of transumbilical shunt insertion, which provides better cosmetic results and without many more complications. This approach has been performed for a long period in a wide variety of in

Aplasia of the internal carotid artery.

BACKGROUND: The majority of previous reports on this rare agenesis of the internal carotid artery (ICA) have been limited to reporting upon its association with other congenital anomalies case by case. In order to collectively summarize this congenital anomaly of ICA, we have reviewed nine cases of ICA aplasia and their associated abnormalities. METHODS: Nine cases of ICA aplasia were reviewed. The diagnosis of aplasia or agenesis of the ICA was based on angiographic findings and the presence of an absent or hypoplastic bony carotid canal by temporal bone computed tomography (TBCT). Their pre

Delivery of imaging agents into brain.

Delivery of diagnostic agents to the central nervous system (CNS) poses several challenges as a result of the special features of CNS blood vessels and tissue fluids. Diffusion barriers exist between blood and neural tissue, in the endothelium of parenchymal vessels (blood-brain barrier, BBB), and in the epithelia of the choroid plexuses and arachnoid membrane (blood-CSF barriers), which severely restrict penetration of several diagnostic imaging agents. The anatomy of large vessels can be imaged using bolus injection of X-ray contrast agents to identify sites of malformation or occlusion, and

Detection frequency of cystic rhomboid lip in a nonoperative clinical population: an MR imaging morphologic study.

PURPOSE: The cystic rhomboid lip (CRL), a diverticular expansion of the fourth ventricular lateral recess at the foramen of Luschka, is familiar to posterior fossa surgeons but remains incompletely characterized on Magnetic Resonance (MR) imaging. We aimed to determine its MR imaging detection frequency and morphologic features in a nonoperative referral population and to evaluate a practical recognition framework against surgically confirmed cystic cerebellopontine angle (CPA) mimics. METHODS: In this retrospective 2-center study, 662 consecutive patients who underwent thin-section skull bas

Prenatal Diagnosis and Perinatal Outcomes of Posterior Fossa Anomalies in a Tertiary Referral Center: A Five-Year Experience.

Background and Objectives: Posterior fossa anomalies (PFAs) represent a heterogeneous group of congenital malformations involving the cerebellum and adjacent structures. Prenatal counseling remains challenging because neurodevelopmental outcomes vary substantially according to the specific anomaly. This study aimed to evaluate ultrasonographic and fetal magnetic resonance imaging (MRI) characteristics, associated anomalies, genetic test results, and perinatal and neurodevelopmental outcomes of prenatally diagnosed PFAs in a tertiary care population. Materials and Methods: This retrospective st

Risk factors for postoperative motor complications in middle-third parasagittal meningioma surgery.

BACKGROUND: Middle one-third parasagittal meningiomas are surgically demanding due to their proximity to the motor cortex and complex venous anatomy. Preservation of neurological function while achieving maximal safe resection remains a key challenge. This study aimed to identify clinical, radiological, and intraoperative predictors of postoperative motor weakness in patients with middle one-third parasagittal meningiomas. METHODS: A retrospective review was conducted on 103 consecutive patients who underwent microsurgical resection for middle one-third parasagittal meningiomas at a tertiary

Macrostructural Brain Abnormalities in Spinal Muscular Atrophy: A Case-Control Study.

BACKGROUND AND OBJECTIVES: Most individuals with spinal muscular atrophy (SMA) on disease-modifying therapies continue to have chronic motor impairment. Insights into brain involvement in SMA may open new pathways for adjunctive therapies to optimize outcomes. We aimed to characterize macrostructural brain abnormalities detected by MRI in individuals with SMA compared with peer controls. METHODS: We conducted a cross-sectional case-control study of children and adults with a confirmed genetic diagnosis of 5q SMA, and peer controls matched by age and sex. Brain MRIs acquired on a 3T MRI scanne
