A diagnostic pitfall: Advanced labyrinthitis ossificans simulating congenital inner ear malformation on CT and MRI.

Labyrinthitis ossificans (LO) is a rare pathological ossification of the membranous labyrinth, typically arising after inflammatory or infec … ed on clinical and radiological features. Treatment options depend on the ossification stage; while cochlear implantation remains feasible in early or partial ossification, advanced cases may require alternative rehabilitation strategies. Early diagnosis is crucial for optimizing hearing outcomes. This report emphasizes the importance of combining clinical history, aud …

[Wolfram-like syndrome: a case report].

Different from classical autosomal recessive Wolfram syndrome, Wolfram-like syndrome is an autosomal dominant disorder caused by a heterozygous mutation in the WFS1 gene. In this case, a 7-year-old male child presented to the eye clinic due to vision loss that could not be corrected, discovered during a routine examination. The child had experienced hearing impairment since early childhood, leading to cochlear implantation. Ophthalmic examination revealed optic disc atrophy in both eyes. Optical coherence tomography imaging demonstrated a distinctive thickening of the outer plexiform layer wit…

Promontory electrical stimulation in labyrinthectomized ears.

Bilateral deafness can occur in patients with Menière's disease who have undergone a labyrinthectomy in one ear. To investigate the feasibility of a cochlear implant in the labyrinthectomized ear, promontory electrical testing by transtympanic needle was performed in six patients who had undergone a unilateral transmastoid labyrinthectomy 6 weeks to 5 years previously. All patients had a behavioral response to the stimulus, and each described a different pitch percept with the four frequencies used. Five of the patients demonstrated an electrically evoked middle latency response. These data ar

Listening-Related Fatigue in Adolescents With Prelingual Deafness and Device Experience.

This study evaluated listening-related fatigue among adolescents with prelingual hearing loss (HL) and long-term device use; and examined demographic, audiological, and functional factors associated with increased fatigue. A total of 271 16-19-yr-old adolescents fitted with their first hearing device by 3 years of age, and their parents completed the self- and parent-report versions of the Vanderbilt Fatigue Scale, respectively. Of these, 53.9% (n=136) used hearing aids (HA), 46.0% (n=116) used at least one cochlear implant (CI), <2% (n=4) used bone conduction (BC) devices, and 7% (n=15) were

A Novel Algorithm to Derive Spread of Excitation Based on Deconvolution.

OBJECTIVE: The width of the spread of excitation (SOE) curve has been widely thought to represent an estimate of SOE. Therefore, correlates between psychophysical parameters, such as pitch discrimination and speech perception, and the width of SOE curves, have long been investigated. However, to date, no relationships between these objective and subjective measurements have yet been determined. In a departure from the current thinking, the authors now propose that the SOE curve, recorded with forward masking, is the equivalent of a convolution operation. As such, deconvolution would be expecte

Amplification of the poorer ear by StereoBiCROS in case of asymmetric sensorineural hearing loss: effect on tinnitus.

Tinnitus is prevalent among patients suffering from Single-Sided Deafness (SSD) and Asymmetrical Hearing Loss (AHL). In addition to bothersome tinnitus in the poorer ear, these patients also report issues with understanding speech in noise and sound localization. The conventional treatment options offered to these patients to improve auditory abilities are cochlear implantation, bone conduction devices or Contralateral Routing Of Signal (CROS) hearing aids. It was recently found that the benefit of cochlear implantation for tinnitus associated with AHL/SSD was greater than the other two approa…

Wegener's granulomatosis causing bilateral facial paralysis and deafness.

Bilateral facial paralysis (BFP) is an uncommon condition that typically occurs as a manifestation of systemic disease. We present a female patient with Wegener's granulomatosis (WG), particularly upper respiratory and ear impairment who develops hypoacusis and BFP, resistant to immunosuppressive therapy and steroid boluses. Her imaging tests showed no involvement of the facial nerve as it passed through the ear structures. The patient finally improved the BFP; however, deafness is permanent and she has entered into a cochlear implant program. Published papers on BFP are rare and they make no

[Auditory rehabilitation path for children].

The suspicion of a hearing defect is nowadays raised during auditory screening, which is performed on all newborn immediately after birth. Final diagnosis requires several examination visits, during which clinical examinations are combined with electrical examinations of auditory responses. Auditory rehabilitation is executed with hearing aids applied in both ears. If conventional auditory rehabilitation with hearing aids does not ensure sufficient hearing for the development of speech, the child can be provided with a cochlear implant. Children with a hearing defect should regularly be monito

Toward the development of a cortically based visual neuroprosthesis.

Motivated by the success of cochlear implants for deaf patients, we are now facing the goal of creating a visual neuroprosthesis designed to interface with the occipital cortex as a means through which a limited but useful sense of vision could be restored in profoundly blind patients. We review the most important challenges regarding this neuroprosthetic approach and emphasize the need for basic human psychophysical research on the best way of presenting complex stimulating patterns through multiple microelectrodes. Continued research will hopefully lead to the development of and design speci

Osseointegration timing for Baha system loading.

OBJECTIVES: The process of osseointegration for creating a biological bond between titanium oxide and bone is time dependent. However, different surgeons have used very varied time frames before loading the implant. The waiting time in dental implant loading ranged from immediate to 6 months. The Baha system (Cochlear Limited, Englewood, CO) traditional waiting period consisted of 3 months for adults and 4 to 6 months for children. The purpose of the study was to evaluate the safety of reducing the waiting time to 6 weeks in adults. STUDY DESIGN: Retrospective study of patients who underwent
