Paget's disease or osteitis deformans is a bone metabolic disease characterized by an increased bone resorption followed by an increased but irregular bone formation. This results in a weakened, deformed bone and an increase bone mass with collagen fibers forming an irregular and pathologic mosaic instead of the parallel symmetry that characterizes healthy bone. It is considered as a non-neoplastic disorder that mimics a bone tumor. Its origin is still uncertain; however, it has been postulated that genetic or environmental factors are involved in its etiology. It rarely occurs in young patien

Bisphosphonates (BPs) are clinically used for the treatment of bone metabolic abnormalities because they are powerful inhibitors of bone resorption. Osteonecrosis of the jaw has been observed after tooth extraction in a considerable number of BP-treated cancer patients, but the reason for this is not known. We studied the effects of BP on extraction socket healing in rats that were pretreated with BP prior to tooth extraction. Male Wistar rats (approximately 5 weeks old) were divided into experimental (BP) and control groups. In both groups, maxillary right second molars were extracted under g

PURPOSE: To date, the therapy of bisphosphonate-associated osteonecrosis of the jaws (BONJ) has been hampered by the lack of imaging modalities that enable the extent of necrosis to be visualized. This study aims to demonstrate the feasibility of tetracycline fluorescence guided bone resection in the surgical management of BONJ.
PATIENTS AND METHODS: Following a 10-day preoperative administration of doxycycline in patients suffering from BONJ, sufficient doxycycline is incorporated into viable bone to be visualized with a certified medical lamp emitting exciting light at 400 to 460 nm.

Bone grafting has become an essential part of implant dentistry. Thanks to the advancement of grafting technology, areas that could only be managed with removable dentures or teeth-supported fixed prostheses can now be successfully treated with implant-supported prosthesis. Nonetheless, the complications associated with bone grafting procedures have slowly emerged to be one of the main challenges to many clinicians. A Medline literature search of articles published from 1984 to 2006 related to advanced bone grafting complications were selected and analyzed. To make it easier to understand, imp

Over the past 4 years, numerous cases of osteonecrosis of the jaw in patients treated with bisphosphonates have been reported. Since 1998, children and adolescents with osteogenesis imperfecta have received bisphosphonates to increase their bone density and reduce the incidence of bone fractures. The results have been convincing, but recent reports of osteonecrosis of the jaw have caused great concern when these patients require dental extractions. The dental records of 15 children and adolescents with osteogenesis imperfecta, involving 60 dental extractions, mostly of primary teeth, done betw

Osteoporotic fractures may lead to pain, disability, impaired quality of life, and increased risk of death, with annual health-care costs of $17 billion or more. Oral bisphosphonates, the first-line treatment for postmenopausal osteoporosis (PMO), increase bone mineral density and reduce the risk of fracture, but dosing requirements are complex and compliance and persistence are poor. The newest bisphosphonate treatment option, intravenous (IV) zoledronic acid (ZOL) every 12mo, has been shown to reduce the risk of vertebral, hip, and other nonvertebral fractures. Long-dosing intervals and 100%

A prospective study was performed in 80 patients receiving bisphosphonates in order to determine frequency of occurrence, risk factors, clinical presentation, radiology, pathology and proper treatment of osteonecrosis of the jaw (ONJ). Of 80 patients, 22 (28%) developed ONJ. There were 11 male and 11 female patients. Median age was 65 years. Ten patients (46%) had multiple myeloma (MM), 5 (23%) had breast cancer and 7 (32%) had other malignancies. Of 22 patients with ONJ, 14 patients (64%) received zoledronate, 3 (14%) received pamidronate, 4 (18%) received pamidronate later followed by zoledr

Osteoporosis belongs to the ten most common diseases. There is no doubt about the importance of primary prophylaxis. Regarding postmenopausal women the following subjects will be discussed: How can the fracture risk be evaluated? What kind of pharmacotherapy is available? An osteodensitometry will be performed if risk factors are present. Most of the laboratory testing can be done in a primary care unit to exclude secondary forms of osteoporosis. Guidelines concerning the decision to start a therapy are based on the individual risk profile and on the osteodensitometric score. The pharmacothera

Osteoporosis is a systemic disease of the bones with increasing incidence in the elderly. Over the age of 50 years bone mineral density continuously decreases resulting in osteoporotic fracture. Osteoporosis is positively correlated with late-onset hypogonadism and increases under androgen deprivation therapy. The evaluation of osteoporosis should be done in cooperation with an endocrinologist. Measurement of bone mineral density is recommended before starting androgen deprivation therapy. Patients with fracture and/or decreased bone mineral density 2.5 or more standard deviations below normal

Women with breast cancer, especially those receiving aromatase inhibitors, are at higher risk for bone loss and fracture. Postmenopausal women may already have multiple risk factors for fracture, and breast cancer therapies compound these risks. Current guidelines for bone health management in women with breast cancer are based on bone mineral density (BMD) measurements; however, many risk factors for fracture are independent of bone mineral density. Thus, consideration of other risk factors for fracture, including cancer therapies, aromatase inhibitor therapy, and age, may be necessary to acc

PURPOSE: Bisphosphonate (BP)-associated osteonecrosis of the jaw (ONJ) is an emerging problem with few therapeutic options. Our pilot study of BP-ONJ investigated a possible role for hyperbaric oxygen (HBO(2)) therapy.
PATIENTS AND METHODS: A total of 16 patients, ranging in age from 43 to 78 years, with BP-ONJ were treated with adjunctive HBO(2) between July 2003 and April 2006. Staging was based on the size and number of oral lesions. Clinical response after treatment and at distant follow-up; the odds of remission, stabilization, or relapse; and time to failure analysis were calculated.

Long-term treatment with glucocorticoids can result in drug-related complications, among which osteoporosis is one of the most frequently encountered problems. Each patient treated with a dose of 7.5 mg or more of prednisone daily for at least 3 months can be affected. During the prolonged steroid use bone formation is inhibited while its resorption increases and negative calcium balance with secondary hyperparathyroidism occurs. In the affected bone, multiple focuses of osteomalacia and avascular necrosis are also described. The bone fracture risk is much higher than it can be suspected on th

One of the main features of McCune-Albright syndrome is bone fibrous dysplasia (BFD) often associated with severe clinical outcomes, such as bone pain, bone deformities and pathological fractures. Medical treatment with bisphosphonates started 15 years ago. Recent trials in pediatric patients with BFD have shown encouraging results. We evaluated long-term efficacy and safety of pamidronate treatment of BFD in children and adolescents with MAS. The drug was administered at 4 month-1 year intervals according to alkaline phosphatase levels. The study included 14 patients (10 females and 4 males b

BACKGROUND: Sacral insufficiency fractures (SIFs) are often overlooked in elderly patients presenting with low back and pelvic pain following no or minimal trauma.
OBJECTIVE: The aim of this review is to raise awareness and outline the clinical presentation, methods of diagnosis and treatment of SIFs.
DISCUSSION: Insufficiency fractures represent a special category of stress fractures that occur in bones with reduced mineral content and elastic resistance. SIFs, a well-defined subgroup of the latter group, are not uncommon, but lack of clinical suspicion results in many being undiagnosed. SI

Almost 70-80% of the patients with Multiple Myeloma (MM) in advancer phase, of the disease show osteolytic lesions and/or pathologic fractures, with or without secondary osteoporosis. An accelerated osteoclast-mediated bone absorption is believed to be the main cause of bone damage in MM. Osteoclast can be activated by a variety of microenvironmental factors. Bisphosphonates (BF) induce the apoptosis of osteoclasts and inhibit osteoclastogenesis, thus preventing bone absorption. As well as BFs, the so-called second-generation BF (N-BF) may impair the activity of osteoclast. Neridronic acid (NE

A 56-year-old man was admitted to our hospital for liver tumor of 7 cm in diameter, located mainly in the anterior segment. He underwent a hepatic resection of anterior and median segment of the liver in January 2002. Pathologically, the tumor was moderately differentiated intrahepatic cholangiocarcinoma (ICC) with vascular invasion. Hepatic arterial chemotherapy as a neoadjuvant therapy was performed. In December 2002, bone metastases in the 12th thoracic spine and iliac bone were found. Therefore, he received radiotherapy (total of 30 Gy) and a monthly bisphosphonate infusion. In May 2003, m

Bisphosphonates are commonly used in medicine to maintain bone density in patients with certain nonneoplastic diseases or cancers. A serious adverse effect of bisphosphonates that has substantial dental significance is osteonecrosis that appears to uniquely affect the mandible and maxilla without occurring in other bones of the skeleton. Patients with bisphosphonate associated osteonecrosis of the jaws may present with pain and exposed necrotic bone. This has substantial clinical implications because surgical procedures (including extractions or endodontic surgical procedures) are contraindica
