We report a case of 68-year-old female patient who suffered two Gamma 3 nail breakages following trochanteric fixation failure revision surgery. A patient was primarily treated with sliding hip screw. Revision surgeries of reverse trochanteric nonunion were performed with short Gamma 3 nails and bone grafting. The nail failures occurred 11 and 7 months after implantation. Supracondylar fractures are commonly encountered in the pediatric population. An impacted radial or ulnar column may result in the deviation of the elbow axis in the frontal plane clinically seen as cubitus varus or valgus. BACKGROUND: Intramedullary reaming and nailing of long bones impairs the endosteal circulation, often causing necrosis of the inner region of the bone cortex. We hypothesized that compensatory hypertrophy of the periosteal microcirculation may develop in response to mechanical destruction of the endosteum, and that this may affect bone survival in these circumstances. Brucellosis is a zoonotic infection with a wide spectrum of clinical presentations. We present a rare case of brucellosis with osteoarticular involvement complicated by a femoral intramedullary nail protruding the suprapatellar pouch. Although Brucella melitensis is a rare cause of brucellar septic arthritis, it should not be excluded from the list of suspected organisms in endemic areas for the disease.

In 1996, the AO/ASIF developed the proximal femoral nail (PFN) as an intramedullary device for the treatment of unstable per-, intra- and subtrochanteric femoral fractures. The author analyses the treatment of 107 injured patients with 148 double and 203 single diaphyseal fractures of long bones of various localization. Special attention has been paid to double fractures in the light of experimental studies carried out by the author in which a number of formerly unknown regularities of blood supply of the fragments has been revealed, which stipulates the peculiarities of the therapeutic tactics. Indications for various methods of treatment with regard to the classification groups are stated. Long-term results are given.

Ten patients with humeral shaft fractures and no clinical or radiographic signs of healing after at least six weeks' immobilisation were treated by flexible intramedullary nailing using a closed retrograde technique. The outcome of intramedullary nailing in 17 femoral fractures was studied in children aged 5 to 12 years. By this age independent vascularity of the capital femoral epiphysis has become established.
