Osteitis is a misnomer: a histopathology study in primary chronic rhinosinusitis.

BACKGROUND: The histological features of osteitis in chronic rhinosinusitis (CRS) in animal studies induced by bacterial inoculation into maxillary sinuses revealed inflammatory involvement of the underlying bone matrix and/or the Haversian system; however, human studies do not mention these findings. The objective of this study was to investigate the inflammatory characterization of osteitis in CRS. METHOD: A prospective study of primary CRS patients undergoing sinus surgery was conducted (August 2012 to April 2013). Bone-mucosa samples were taken from a predetermined site that correlated to

Carbon ion therapy for advanced sinonasal malignancies: feasibility and acute toxicity.

PURPOSE: To evaluate feasibility and toxicity of carbon ion therapy for treatment of sinonasal malignancies. First site of treatment failure in malignant tumours of the paranasal sinuses and nasal cavity is mostly in-field, local control hence calls for dose escalation which has so far been hampered by accompanying acute and late toxicity. Raster-scanned carbon ion therapy offers the advantage of sharp dose gradients promising increased dose application without increase of side-effects. METHODS: Twenty-nine patients with various sinonasal malignancies were treated from 11/2009 to 08/2010. Acc

Pedicled facial buccinator (FAB) flap: a new flap for reconstruction of skull base defects.

BACKGROUND: The expansion of endoscopic endonasal skull base surgery has resulted in an increased demand for reconstructive options. Reconstruction with vascularized tissue has proven indispensable for reliably separating the cranial contents from the paranasal sinuses following extended endoscopic endonasal approaches (EEA). The introduction of the Hadad-Bassagasteguy flap (vascular pedicle nasoseptal flap, HBF) at our institution decreased our postoperative cerebral spinal fluid (CSF) leak rates from more than 20% to less than 5%. The HBF is not always available, as the nasoseptal area or it

Using Surgical Navigation to Capture Objective Metrics During Endoscopic Sinus Surgery: A Proof-of-Concept Study.

BACKGROUND: Evaluation of otolaryngology resident technical performance has historically relied on subjective perioperative feedback. Despite various assessment tools attempting to provide consistent surgical skill evaluations, current methods remain inherently subjective. AIM: This proof-of-concept pilot study aimed to assess if a 3-dimensional surgical navigation system could capture objective surgical data during endoscopic sinus surgery (ESS). A secondary aim was to analyze if there were any differences between novice and practiced trainees. METHODS: Otolaryngology residents performed su

Blood Eosinophil Count is the Dominant Clinical Marker for type 2 Inflammatory Severity in CRSwNP.

OBJECTIVE: Severe type 2 eosinophilic chronic rhinosinusitis with nasal polyps (CRSwNP) is challenging to treat and susceptible to recurrence post-surgery. This study aimed to evaluate the relationship between clinical markers and tissue type 2 inflammatory severity in patients with CRSwNP. METHODS: Adult patients who underwent endoscopic sinus surgery for bilateral CRSwNP were prospectively enrolled. Tissue eosinophil count (TEC) was evaluated. Expression levels of type 2 cytokines, including IL-5 and IL-13, in nasal polyps were determined using real-time PCR. Correlations between clinical m

Promoting Osseointegration of Dental Implants in Dog Maxillary Sinus Floor Augmentation Using Dentin Matrix Protein 1-Transduced Bone Marrow Stem Cells.

BACKGROUND: Beta-tricalcium phosphate (β-TCP) has been employed successfully as a synthetic graft material in maxillary sinus floor augmentation (MSFA) for placing dental implants. However, the lack of osteogenic and osteoinductive properties of this substitute invariably results in bone regeneration of low quality and quantity. The purpose of this study was to determine whether loading dentin matrix protein-1 (DMP1) gene-modified bone marrow mesenchymal stem cells (BMSCs) onto β-TCP promoted bone regeneration and osteointegration of dental implants in MSFA of dogs. METHODS: BMSCs were transd

Primary sinonasal lymphoma in immunocompetent patients: A 10 years retrospective clinicopathological study.

INTRODUCTION: Sinonasal tumors occur in the nasal cavity or paranasal sinuses (PNS). These tumors are rare and lymphomas are even rarer. Lymphoma of the nose and PNS may mimic benign processes and may manifest either in an isolated fashion or in conjunction with systemic diseases. B-cell lymphomas, a more favorable diagnosis, account for the majority of cases, whereas T-cell and extranodal natural killer lymphoma are associated with rapid disease progression and death. MATERIALS AND METHODS: All patients with sinonasal lymphomas who were nonreactive for HIV and were operated and treated in ou

Evaluation of Malassezia and Common Fungal Pathogens in Subtypes of Chronic Rhinosinusitis.

BACKGROUND: Fungal hypersensitivity and fungal microbiome dysbiosis are possible etiologies of chronic rhinosinusitis. The sinus fungal microbiome is not well characterized; novel sinus-associated fungi, including Malassezia, have only recently been described. The goals for this study were to verify Malassezia as a dominant component of the sinus microbiome, to speciate sinus Malassezia, and to compare select fungal species in chronic rhinosinusitis (CRS) subtypes with known fungal association to chronic rhinosinusitis with polyps (CRSwNP) and healthy controls. METHODS: Twenty-eight patients

Antiadhesive effect of mixed solution of sodium hyaluronate and sodium carboxymethylcellulose after blow-out fracture repair.

Treatment of blow-out fractures is aimed at the prevention of permanent diplopia and cosmetically unacceptable enophthalmos. Porous polyethylene sheets are one of the most common alloplastic implants for blow-out fracture repair. Because adhesion between the porous polyethylene and the orbital soft tissue can result in restrictions of ocular motility, prevention of postoperative adhesion is important in the reconstruction of blow-out fractures. The purpose of this study was to find out the effect of the mixed solution of sodium hyaluronate and sodium carboxymethylcellulose (HACMC) on postopera
