Functional improvements after orthodontic-surgical reconstruction in a patient with multiple maxillofacial fractures.

Patients with multiple craniofacial fractures often suffer from stomatognathic problems after their primary treatment, because administering emergency care is the clinician's highest priority. Therefore, optimal bone repositioning is sometimes difficult because bone fixation is delayed. Moreover, neither an adequate radiographic examination nor an evaluation of primary occlusion is available during the repair of fractured bones. The lack of these assessments can also lead to occlusal dysfunction after bone fixation. As a result, patients with craniofacial fractures often require occlusal recon

Distalization of the mandibular dentition of an adult with a skeletal Class III malocclusion.

This case report describes the orthodontic treatment of an 18-year-old woman with a skeletal Class III malocclusion and a midline deviation. The treatment plan consisted of distalizing the mandibular dentition asymmetrically and producing space for retraction of the mandibular anterior teeth. Short Class III elastics, an open-coil spring, and the mulitloop edgewise archwire technique were used, combining the entire maxillary dentition as integrated anchorage. The active treatment period was 26 months. Normal overbite and overjet were obtained, and facial balance was improved.

Craniofacial team management in Apert syndrome.

INTRODUCTION: Apert syndrome is one of the rarest of the craniosynostosis syndromes. Affected persons have extensive structural and functional impairments, some of which can be life threatening. Management requires team care from infancy to adulthood. The purposes of this article are to assess the outcomes in individuals with Apert syndrome after completion of treatment and to review current protocols for craniofacial team care and dental, orthodontic, and orthognathic surgical management. METHODS: This was a retrospective cohort study of 8 subjects with Apert syndrome. Cephalograms at 2 time

Orthodontic treatment of a patient with oral-facial-digital syndrome.

Oral-facial-digital syndrome is characterized by heterogeneous clinical features involving malformations of the face, oral cavity, and digits. The syndrome has been reported only in women, suggesting that it is lethal in males. Affected females have malformations of the oral cavity (cleft palate, lip and tongue, abnormal dentition, and hamartomas), face (hypertelorism and milia), and digits (syndactyly, brachydactyly, and polydactyly). In this article, we report the orthodontic treatment of a 21-year-old woman with oral-facial-digital syndrome.

Multiple congenitally missing teeth treated with autotransplantation and orthodontics.

Treatment of children with several congenitally missing teeth is challenging, because growth and development of the oral structures must be considered. The treatment options include retaining the deciduous teeth and postponing treatment until later or extracting the deciduous teeth and doing one of the following: allowing the space to close spontaneously, closing the space orthodontically, or in patients whose growth is finished, using a prosthetic or implant replacement. One other viable option, if donor teeth are available, is autotransplantation. The treatment plan for patients with missing

Treatment of skeletal open-bite malocclusion with lymphangioma of the tongue.

Lymphangioma of the tongue causes massive tongue enlargement, leading to difficulties in swallowing and mastication, speech disturbances, airway obstruction, and skeletal deformities such as open-bite malocclusion. Early reduction of tongue volume improved the excessive open bite in a young girl, but it was not sufficient to redirect the original hyperdivergent growth pattern. Orthodontic camouflage treatment was therefore rendered. Long-term evaluation after tongue-reduction surgery and orthodontic treatment is presented.

Conservative orthodontic treatment for a patient with a unilateral condylar fracture.

Trauma to the mandible often causes condylar fracture. This article reports the conservative treatment of a 10-year-old girl with a unilateral condylar fracture, highlighting the diagnostic aspects involved and the strategy used. The conservative approach used for this patient-bionator followed by full fixed orthodontic appliances-provided adequate esthetic and functional results. The outcomes throughout the 7-year follow-up and the remodeling process of the condyle observed in the panoramic radiographs proved the success of this treatment.

Severity of alveolar cleft before palatoplasty affects vertical maxillofacial growth in 6-year-old patients with complete unilateral cleft lip and palate.

INTRODUCTION: In this study, we investigated the association between the severity of alveolar clefts in infants and the succeeding maxillofacial growth in patients affected by complete unilateral cleft lip and palate. METHODS: The subjects were 35 nonsyndromic patients affected by unilateral cleft lip and palate (n = 27) or bilateral cleft lip and palate (n = 8). Plaster models and lateral cephalograms taken at 6 years of age were analyzed and compared with the Japanese norms. In the unilateral cleft lip and palate patients, the relationship between the width of the alveolar cleft before pala

Treatment of ankylosed maxillary central incisors by segmental osteotomy with autogenous bone graft.

This case report describes the treatment of a 16-year-old girl with ankylosed maxillary central incisors that were noticeably infraoccluded and labially displaced. We performed a segmental osteotomy with an autogenous bone graft in a single-stage surgery to align and level the ankylosed teeth. The dento-osseous segment was successfully repositioned with satisfactory periodontal results.

Attempted traction of impacted and ankylosed maxillary canines.

The aim of this article is to report the clinical orthodontic treatment of an adult patient with 2 impacted maxillary canines. Traction was applied to the impacted teeth; however, after 7 months, the teeth were found to be ankylosed and were extracted. The extraction spaces were closed by moving the posterior teeth mesially with mini-implant anchorage. The results were satisfactory, with the premolars in the functional position of the canines.

Esthetic periodontal surgery for impacted dilacerated maxillary central incisors.

Clinicians do not frequently see impacted dilacerated maxillary incisors in their patients. When they do, there are several diagnostic and management challenges for correcting root dilacerations. An unfavorable esthetic outcome might occur as a result of soft-tissue complications during surgical eruption procedures. We present 2 patients with an impacted and dilacerated maxillary central incisor. Computed tomography scans with 3-dimensional reformation were used to accurately assess the positions of the dilacerated teeth, the degree of dilaceration, and the stage of root formation. The therapy

Arrested root formation of 4 second premolars: report of a patient.

The shape and size of tooth roots are genetically and phylogenetically predetermined. Clinical defects in root formation can manifest in the form of shortened roots caused by either root agenesis or root resorption. We report on a patient who came at age 7 years for space management. In the 2-year period after the initial visit, maxillary arch expansion was performed, followed by serial extractions of all 4 first premolars. A radiograph taken about 18 months after the serial extraction showed that although the crowns of all 4 second premolars had erupted fully into the arch, the roots were onl

Upper lip changes and gingival exposure on smiling: vertical dimension analysis.

INTRODUCTION: Our objectives were to evaluate and quantify upper lip soft-tissue changes in the vertical dimensions both at rest and at maximum smile, and to examine the correlation between upper labial vestibular attachment height and maxillary gingival exposure on smiling. METHODS: Seventy-two volunteers (36 men, 36 women) aged 20 to 40 (mean, 30.49 years) were recruited for this study. For each subject, 9 measurements of upper lip position and maxillary incisor crown height at rest and in maximum smile were recorded. RESULTS: A statistically significant sexual dimorphism was apparent in m

Smile esthetics from the layperson's perspective.

INTRODUCTION: Computer-based smile esthetic surveys based on slider technology allow more precise control of variables and the possibility of obtaining continuous data. Variations in the perception of smiles from different facial perspectives have not been resolved. The objective of this study was to quantify the ideal and the range of acceptable values for smile variables judged by laypersons from a full-face perspective for comparison with lower-face data. METHODS: Mirrored and symmetric male and female full faces previously determined by peers to be of average attractiveness were used. Nin

Effect of maxillary anteroposterior position on profile esthetics in headgear-treated patients.

INTRODUCTION: Headgears have been used to treat Class II malocclusions for over a century. The purpose of this retrospective study was to investigate the profile esthetic changes resulting from headgear use in growing Class II patients with protrusive, normal, and retrusive maxillae. METHODS: Profile silhouettes were created from pretreatment and posttreatment lateral cephalometric tracings of growing Class II patients treated with headgear followed by conventional fixed appliances. Ten patients had an initially protrusive maxilla (FH:NA, >92°), 10 had an initially normally positioned maxilla

Permutation method for evaluating topographic craniofacial correlations.

INTRODUCTION: Correlations between cephalometric measurements are frequently assumed to represent biologic associations. However, a significant portion of such correlations might arise from purely geometric dependencies, when measurements share common landmarks. Analytic calculation of this topographic component is difficult. The purpose of this study was to propose a permutation method for evaluating the topographic component of cephalometric correlations. METHODS: The method consisted of creating a virtual sample of cephalometric tracings (landmark configurations) from the original biologic
