Total hip arthroplasty in patients with human immunodeficiency virus infection: pathologic findings and surgical outcomes.

BACKGROUND: An increased incidence of osteonecrosis of the femoral head has been reported in patients infected with human immunodeficiency virus (HIV). The purpose of this study was to review the pathologic specimens of HIV-positive patients who had undergone total hip arthroplasty (THA) and compare them with those of THA patients who were HIV-negative. The surgical outcomes of these HIV-positive patients were also reviewed. METHODS: 40 HIV-positive patients who underwent 54 THAs at our institution were identified. The primary pathologic diagnosis for the femoral heads of these patients was o

Infectious complications following breast reconstruction with expanders and implants.

The incidence of infection following breast reconstruction with expanders and implants ranges from 1 to 24 percent. Numerous factors associated with infection have been described; however, a one-variable at time setting and multifactorial analysis have not been performed. The purpose of this study was to analyze a set of factors that may predispose women to infection of the expander or implant. Between 1997 and 2000, a total of 168 implant reconstructions were performed in 130 women at a single institution. The mean age for all women was 48.2 years (range, 25 to 77 years). The factors that wer

Pneumococcal septic arthritis: review of 190 cases.

This article reports 13 cases of pneumococcal septic arthritis and reviews another 177 cases reported since 1965. Of 2407 cases of septic arthritis from large series, 156 (6%) were caused by Streptococcus pneumoniae. Mortality was 19% among adults and 0% among children. Pneumococcal bacteremia was the strongest predictor of mortality. At least 1 knee was involved in 56% of adults. Polyarticular disease (36%) and bacteremia (72%) were more common among adults with septic arthritis caused by S. pneumoniae than among adults with other causative organisms. Only 50% of adults with pneumococcal sept

An interdisciplinary approach to the prevention and treatment of groin wound complications after lower extremity revascularization.

BACKGROUND: If not effectively treated, groin wound infections following lower extremity revascularization (LER) may result in graft or limb loss. METHODS: A retrospective review was performed of all patients who underwent muscle flap transposition by a single surgeon after LER between 2006 and 2010. RESULTS: Twenty-nine muscle transposition flaps were performed in 24 patients (21 sartorius, 6 rectus femoris, and 2 gracilis). Nineteen were for treatment of groin wound infections, two for treatment of lymphocele, one for coverage of exposed graft in the setting of pyoderma gangrenosum, and se

Sequential bioelectric programming of Macrophages via ferroelectric nanostructured surface to combat implant infection and promote bone integration.

Implant-associated infections impair osseointegration by suppressing macrophage-mediated antibacterial responses and blocking reparative phenotype transition, while conventional antimicrobial strategies may compromise bone regeneration. Although electrical stimulation holds promise for macrophage modulation, achieving spatiotemporally controlled bioelectrical regulation remains a challenge. Herein, we engineered a ferroelectric barium titanate-nanostructured titanium implant (ferroTi) that enables sequentially controlled electrical manipulation of macrophages for coordinated antibacterial acti

Corynebacterium macclintockiae: a potential pathogen of cardiac implantable electronic device-related endocarditis: Case report.

Accurate diagnosis of cardiac implantable electronic device infections is important for preventing severe complications. While non-diphtherial Corynebacterium species such as C. striatum and C. jeikeium are well recognized pathogens, C. macclintockiae remains poorly characterized. Recent genomic studies suggest that some isolates identified as C. jeikeium may in fact be C. macclintockiae, thereby posing diagnostic challenges. An 82-year-old man with a cardiac resynchronization therapy defibrillator presented with weight loss, hypotension, and inflammation. Blood cultures yielded Gram-positive

Emerging resistance in staphylococci following long-term dalbavancin treatment for prosthetic joint infections.

BACKGROUND AND OBJECTIVES: Dalbavancin, a lipoglycopeptide with a half-life of 150-200 h, is a promising treatment option for prosthetic joint infections (PJIs) and other orthopaedic implant-associated infections (IAIs) caused by multidrug-resistant staphylococci. However, in vitro studies have shown that when exposed to low concentrations of dalbavancin, staphylococcal strains resistant to dalbavancin can emerge, potentially affecting its effectiveness in cases of recurrent infection. To investigate whether dalbavancin-resistant staphylococci emerge in vivo following long-term dalbavancin tre

Eradication of MRSA biofilm-associated implant infections by low-immunogenic sustained-release lysostaphin fused to thermosensitive polypeptides.

Staphylococcal bacteria, particularly methicillin-resistant Staphylococcus aureus (MRSA), are responsible for intractable infections-especially those associated with implants-through biofilm formation, presenting a worldwide menace. Antibacterial enzymes like lysostaphin (Lst) are promising for combating staphylococcal infections but limited by poor stability, high immunogenicity, and suboptimal pharmacokinetics, which hamper their clinical translation as antibiotic alternatives. Herein, we report a strategy of fusing thermosensitive elastin-like polypeptides (ELPs) to Lst to overcome these in

[Intraosseous vancomycin in total knee arthroplasty].

INTRODUCTION: intravenous antibiotic prophylaxis has significantly reduced the incidence of periprosthetic joint infection (PJI) in knee surgeries. However, for patients colonized with methicillin-resistant Staphylococcus aureus (MRSA) or those at risk of colonization, prophylaxis should include vancomycin. Intraosseous (IO) administration of vancomycin could enhance its effectiveness in total knee arthroplasty (TKA). MATERIAL AND METHODS: a retrospective review was conducted, including 143 patients at risk of PJI scheduled for TKA who received IO vancomycin along with intravenous (IV) cefazo

Targeted antimicrobial regimens for Gram-negative prosthetic joint infections: a prospective multicenter study.

Fluoroquinolones (FQs) are considered the most effective antimicrobial treatment for Gram-negative prosthetic joint infection (GN-PJI). Alternatives are needed due to increasing FQ resistance and side effects. We aimed to compare different targeted antimicrobial strategies for GN-PJI managed by debridement, antibiotics, and implant retention (DAIR) or one-stage revision surgery (1SR) and to review the literature of oral treatment options for GN-PJI. In this prospective, multicenter, registry-based study, all consecutive patients with a PJI caused by a Gram-negative microorganism (including mix

Staphylococcus aureus bacteraemia, cardiac implantable electronic device, extraction, and the risk of recurrent infection; a retrospective population-based cohort study.

BACKGROUND: Patients with cardiac implantable electronic device (CIED) and Staphylococcus aureus bacteraemia (SAB) are at risk of having CIED infection, pocket infection or endocarditis. To avoid treatment failures, guidelines recommend that the CIED should be extracted in all cases of SAB butrecent studies indicate low extraction rates and low risk of relapse. The aim of the study was to describe a Swedish population-based cohort of patients with CIED and SAB, the rate of extraction, and treatment failure measured as recurrent SAB. METHODS: Patients identified to have SAB in the Karolinska L

Emergence of a dalbavancin induced glycopeptide/lipoglycopeptide non-susceptible Staphylococcus aureus during treatment of a cardiac device-related endocarditis.

In the present study, we demonstrated the emergence of dalbavancin non-susceptible and teicoplanin-resistant Staphylococcus aureus small colony variants which were selected in vivo through long-term treatment with dalbavancin. A 36-year-old man presented with a cardiac device-related S. aureus endocarditis and received long-term therapy with dalbavancin. Consecutively, two glycopeptide/lipoglycopeptide susceptible and two non-susceptible S. aureus isolates were obtained from blood cultures and the explanted pacemaker wire. The isolates were characterized by: standard typing methods, antimicrob

Superantigens in Staphylococcus aureus isolated from prosthetic joint infection.

Staphylococcus aureus is a common cause of prosthetic joint infection (PJI). The prevalence of superantigens (SAgs) among PJI-associated S. aureus is unknown. Eighty-four S. aureus isolates associated with PJI isolated between 1999 and 2006 were studied. SAg genes, sea, seb, sec, sed, see, seg, seh, sei, and tst, were assayed by PCR. Seventy-eight (92.9%) isolates carried at least 1 SAg gene studied, with 61 (72.6%) harboring more than 1. seg was most commonly (70.2%), and seh was least frequently (4.8%) detected. tst-positive isolates were associated with early infection and increased erythro

Delayed infection after a zygoma fracture fixation with absorbable plates.

A 48-year-old man who had received a bioresorbable plate fixation for a zygomatic bone fracture 13 months earlier visited our clinic complaining of sudden facial swelling. The facial computed tomographic scan showed the soft tissue swelling without any bony abnormality, and the symptoms did not improve after 1 week of antibacterial therapy. The patient had a diagnosis of a late infection caused by unresorbed plates, and exploratory surgery was performed. Partially resorbed plates and screws were seen, and we removed the remnants of such completely. The symptoms were relieved after the operatio

Lack of efficacy of antibiotic-impregnated shunt systems in preventing shunt infections in children.

OBJECTIVE: Shunt infection is a common and serious complication of cerebrospinal fluid (CSF) shunting most commonly caused by skin flora contamination at surgery. Recent studies indicate that the use of antibiotic-impregnated (AI) shunt systems may reduce the risk of postoperative shunt infections. We evaluated the incidence of shunt infections associated with the use of AI shunt catheters and compared it with the incidence associated with standard non-AI catheters. MATERIALS AND METHODS: All shunt procedures performed by one surgeon using AI catheters were reviewed. An equal number of consec

Biofilm theory can guide the treatment of device-related orthopaedic infections.

Direct observations of the surfaces of orthopaedic prostheses that have failed and of bone affected by osteomyelitis with and without the presence of a prosthesis have shown that the bacteria that cause these infections live in well-developed biofilms. The cells within these matrix-enclosed surface-associated communities are protected from host defenses and antibiotics, and clinical experience has shown that they must be removed physically before the infection can be resolved. The biofilm etiology of these diseases demands new diagnostic methods because biofilm cells typically do not grow on a
