Improvement in 2-year survival for ventricular assist device patients after implementation of an intensive surveillance protocol.

BACKGROUND: This study was conducted to determine the effect of a disease-management model termed an "intensive surveillance protocol" (ISP) on survival in ventricular assist device (VAD) patients. This intervention consisted of a formalized, protocol-driven, multi-disciplinary team approach to VAD patient follow-up initiated August 1, 2006. The goal was to attain an internal program benchmark of 70% survival at 2 years. Historically, 2-year survival after VAD implant has been sub-optimal, and no patient management algorithms have been formally tested to determine their effect on 2-year surviv

Surgical treatment of complex cardiac anomalies: the 'one and one half ventricle repair'.

OBJECTIVE: One and one half ventricle repair is a surgical option for congenital cardiac anomalies characterised by right ventricle (RV) hypoplasia and/or dysplasia. METHODS: From March 1994 to March 2001, eight patients (mean age 9.1 years, range 7 months to 35 years) with hypoplastic and/or dysplastic RV underwent correction of their intracardiac anomaly in association with a BCPS (one and one half ventricle repair). Preoperative diagnoses included: Ebstein's anomaly of tricuspid valve (TV) in two, inlet ventricular septal defect (VSD) in association with straddling/overriding TV in two pat

Cook's balloon versus dinoprostone for Labour induction of term pregnancies with fetal GROWth restriction: study protocol for a randomised controlled trial in tertiary maternity hospitals in Spain (COLIGROW study).

INTRODUCTION: Fetal growth restriction (FGR) affects about 3%-5% of term pregnancies. If prenatally detected and anterograde umbilical artery flow is preserved (stage I), it is recommended to deliver at term (≥ 37+0 weeks). In the absence of contraindications, the vaginal route is preferred, and labour induction is usually required. It has been postulated that mechanical methods for cervical ripening may have an optimal profile for the induction of term FGR fetuses since they are associated with less uterine stimulation than the standard pharmacological methods, and therefore, could be better

Value of multimodal ultrasound assessment of placental dysfunction in gestational diabetes mellitus: a prospective study.

BACKGROUND: Gestational diabetes mellitus (GDM) induces progressive placental structural and functional abnormalities that are often undetectable by conventional ultrasound. Accurately assessing these changes is crucial for preventing adverse perinatal outcomes. This study aimed to evaluate the diagnostic value of multimodal ultrasound in assessing placental structural and functional alterations in pregnancies complicated by GDM, and to identify reliable sonographic biomarkers for evaluating placental abnormalities and clinical risk stratification. METHODS: In this prospective study, 177 preg

PP082. Ophthalmic artery doppler for identification of severe preeclampsia in pregnancies complicated by hypertension.

INTRODUCTION: Hypertension is the most common disorder that occurs during pregnancy and one of the main causes of maternal mortality worldwide. Among the hypertensive disorders that affect pregnancy, preeclampsia (PE) is the major cause of maternal mortality, maternal morbidity, perinatal death, prematurity and intrauterine growth restriction. The acute cerebral complications in PE account for at least 75% of maternal death. Central nervous system hemodynamic modifications in PE are a controversial issue. Neurological involvement in PE-eclampsia syndrome fulfills the criteria of posterior reve

Thoracic Aortic Strain is Irrelevant Regarding Endograft Sizing in Most Young Patients.

BACKGROUND: The introduction of electrocardiogram gating in computed tomography (CT) angiography imaging of aortic disease raised questions whether diameters measured on diastolic images can lead to the undersizing of aortic endografts. As previous studies suggested that young patients may have the highest risk of an unintended undersize, the aim of our study was to analyze the strain of the thoracic aorta in a young patient cohort. METHODS: We assessed the descending aorta of 52 patients (35 men, mean age 41.1 ± 7.3 years) who underwent coronary CT angiography (CCTA) with suspected coronary

Associations of large artery structure and function with adiposity: effects of age, gender, and hypertension. The SardiNIA Study.

UNLABELLED: In the context of obesity epidemic, no large population study has extensively investigated the relationships between total and abdominal adiposity and large artery structure and function nor have such relationships been examined by gender, by age, by hypertensive status. We investigated these potential relationships in a large cohort of community dwelling volunteers participating the SardiNIA Study. METHODS AND RESULTS: Total and visceral adiposity and arterial properties were assessed in 6148 subjects, aged 14-102 in a cluster of 4 towns in Sardinia, Italy. Arterial stiffness was

Revisiting the tension-free paradigm: Axially prestretched elastomeric nanofibrillar grafts restore artery-like axial biomechanics in a preclinical model.

Healthy arteries function under substantial axial prestretch (AP), the ratio of in vivo to unloaded ex vivo length. Loss of AP promotes tortuosity and disturbed hemodynamics associated with vascular pathology, yet artery-like AP has not been incorporated in vascular reconstructions, because conventional substitutes are axially stiff and cannot safely sustain physiological AP. Here, we present Axially Prestretched Elastomeric Nanofibrillar Grafts (APENGs), fabricated by ultrahigh-speed (43,000 rpm) rotational collector electrospinning of biomedical-grade polyurethane. APENGs matched native arte

Clinical Outcomes of Isolated Abnormal Umbilical Artery Doppler Findings in Normally Grown Fetuses: A Retrospective Cohort Study.

BACKGROUND: Umbilical artery Doppler (UAD) is widely used to assess placental function in pregnancies complicated by fetal growth restriction (FGR), but the clinical significance of an abnormal UAD in a fetus that is normally grown at the time of diagnosis is less certain. This study describes the subsequent Doppler course and pregnancy outcomes of such cases. MATERIALS AND METHODS: We performed a retrospective cohort study at Luton and Dunstable University Hospital of pregnancies with abnormal UAD identified between January and December 2025. Abnormal UAD was defined as an umbilical artery p

Left ventricular unloading therapy using Impella 5.5 after emergency surgery for acute myocardial infarction mechanical complication: a case report.

BACKGROUND: Following an acute myocardial infarction (AMI), surgery for left ventricular free wall rupture (LVFWR) and ventricular septal rupture (VSR) has a high in-hospital mortality rate, which has not improved significantly over time. Unloading the LV is critical to preventing excessive stress on the repair site and avoiding problems such as bleeding, leaks, patch dehiscence, and recurrence of LVFWR and VSR because the tissue is so fragile. We present two cases of patients who used Impella 5.5 for LV unloading following emergency surgery for AMI mechanical complications. CASE PRESENTATION

Role of Bedside Transcranial Ultrasonography in the Assessment of Cerebral Hemodynamics in Decompressive Craniectomy Patients with Cranioplasty: A Single Centre Experience.

BACKGROUND: Intracerebral hemorrhage is a leading cause of death and disability worldwide. After intracerebral hemorrhage, cerebral blood flow (CBF) becomes extremely low approaching ischemic thresholds. Concurrently, CBF velocities become strongly correlated to CBF itself post-injury. Identification of such hemodynamic disturbances can be used to predict outcome immediately post-injury when indices are measured using transcranial doppler ultrasonography (TCD). TCD permits non-invasive assessment of different CBF velocities as well as pulsatility index (PI). Abnormal measurement of such indice

[Guillain-Barré Syndrome and its association with infectious factors].

The aortic valve, located between the left ventricle and the aorta, allows for unidirectional blood flow, preventing backflow into the ventricle. Aortic valve leaflets are composed of interstitial cells suspended within an extracellular matrix (ECM) and are lined with an endothelial cell monolayer. The valve withstands a harsh, dynamic environment and is constantly exposed to shear, flexion, tension, and compression. Research has shown calcific lesions in diseased valves occur in areas of high mechanical stress as a result of endothelial disruption or interstitial matrix damage(1-3). Hence, it

Downregulation of miR-223 and miR-153 mediates mechanical stretch-stimulated proliferation of venous smooth muscle cells via activation of the insulin-like growth factor-1 receptor.

Autologous venous grafts, used to circumvent occluded coronary arteries during coronary artery bypass, often develop thrombosis and neointimal hyperplasia. During neointimal hyperplasia, vascular smooth muscle cells (VSMCs), exposed to substantially higher pressure and hemodynamic forces, proliferate and extracellular matrix accumulate causing narrowing of the vessel lumen. Activation of insulin-like growth factor-1 receptor (IGF-1R) has been confirmed to be critically involved in mechanical stretch-stimulated VSMC proliferation. However, the comprehensive mechanisms responsible for activation
