Postoperative chylothorax: differences between patients who received median sternotomy or lateral thoracotomy for congenital heart disease.

BACKGROUND: Chylothorax after surgery for congenital heart disease is rare. We wanted to compare the different presentations of chylothorax in patients who received median sternotomy or lateral thoracotomy. PATIENTS AND METHODS: We retrospectively studied pediatric patients with congenital heart disease who received palliative or corrective surgeries and developed postoperative chylothorax between January 1992 and July 2003. Patients were divided into two groups by the type of surgery: median sternotomy and lateral thoracotomy. The average daily fluid amount (mL/kg/24 hours), latency period,

Traumatic sternal fracture: outcome following admission to a Thoracic Surgical Unit.

INTRODUCTION: We reviewed our experience of the in-hospital management and early follow-up of patients admitted with a traumatic sternal fracture to a Thoracic Surgical Unit. PATIENTS AND METHODS: Over a 7-year period, 73 consecutive patients (51 males) with a median age of 51 (range 17-84) years were admitted through the Emergency Department with an acute traumatic sternal fracture. The patients were hospitalised for cardiorespiratory monitoring, pain control and physiotherapy. Outpatient follow-up occurred 6 weeks after discharge. RESULTS: The median hospital stay was 2 days (range 1-15 da

First-line treatment of deep sternal infection by a plastic surgical approach: superior results compared with conventional cardiac surgical orthodoxy.

A majority of cardiac surgeons manage deep sternal infection with sternal wound debridement, rewiring, and closed drainage, with or without antibiotic saline tube irrigation (the traditional approach). The authors' experience with the traditional approach was unsatisfactory; therefore, they undertook a radical change in management: an immediate plastic surgical approach. Hence, deep sternal infection was managed by immediate debridement followed by a bilateral pectoralis major myocutaneous advancement flap with greater omental transposition (PMOFR). This is the first such study reporting the e

[Closure of atrial septal defect via small lateral thoracotomies, partial and full length sternotomies].

Surgical closure of atrial septal defect is considered a low risk procedure. Various minimally invasive surgical techniques spread over the conventional full-length sternotomy. But the risk profile and complication rates of the minimally invasive closure of atrial septal defect are not revealed sufficiently. We present the results of atrial septal defect surgery via small thoracotomies: right anterolateral, right posterolateral, partial sternotomies and full-length median sternotomies. Ninety four patients underwent surgical closure of atrial septal defect via minimal approaches through the pe

Morbidity and validity of the hemiclamshell approach for thoracic surgery.

OBJECTIVE: This is a prospective study to evaluate the indications and outcome of the hemiclamshell incision (longitudinal partial sternotomy combined with an antero-lateral thoracotomy) as used for a consecutive series of patients requiring surgery for various thoracic pathologies not ideally approached by postero-lateral thoracotomy, sternotomy or thoracoscopy. METHODS: All patients with a hemiclamshell incision performed between 1994 and 1998 were prospectively analyzed regarding indications, postoperative morbidity and outcome (clinical examination and pulmonary function testing) in order

Different approaches for minimally invasive closure of atrial septal defects.

BACKGROUND: To improve the acceptance of cosmetic results after closure of atrial septal defects, anterior or lateral thoracotomies are preferred rather than median sternotomies. Along with the availability of minimally invasive techniques, a further reduction in incision length appeared feasible while preserving thoracic stability. METHODS: Various minimally invasive approaches differing in the type of incision and mode of cannulation have been applied under conditions of normothermic ventricular fibrillation. In technique 1 (n = 5), a right parasternal mini-incision was combined with a cent

Median sternotomy single stage complete unifocalization for pulmonary atresia, major aorto-pulmonary collateral arteries and VSD-early experience.

OBJECTIVE: It is a prospective study to assess the results of median sternotomy, single stage complete unifocalization and repair for ventricular septal defect (VSD), pulmonary atresia and major aorto pulmonary collateral arteries (MAPCAs). METHODS: From June 97 to August 98, 20 patients were treated with single stage complete unifocalization and repair. Their ages ranged from 6 months to 11 years. Through median sternotomy, all MAPCAs were dissected and looped. On cardiopulmonary bypass, MAPCAs were anastomosed to native pulmonary arteries (PAs) or to MAPCAs. VSD was closed if possible and R

Cardiac cause of hypoxaemia in a kyphoscoliotic patient.

This report presents the case of a 73 yr-old female in whom kyphoscoliosis, osteoporotic vertebra compression fractures and sternal injury resulted in severe respiratory failure and hypoxaemia. Pulmonary function testing showed moderate restrictive pattern and rare mismatches were found on lung ventilation/perfusion scanning. Transoesophageal echocardiography with contrast studies showed abnormal anatomic mediastinal interactions which led to right-to-left interatrial shunt, through patent foremen ovale. First-intention treatment, because of orthopaedic and respiratory surgical restraints, was

[Aortic valve prosthesis implantation from a mini approach].

The authors share their experiences with performing the first in Russia operation of making the aorta valve prosthetics from a mini-access--transverse sternotomy. It is concluded that operations through such accesses are safe for the patient and relatively easy for the surgeon. Its main advantage consists in quick surgical rehabilitation of the patient and good cosmetic effect.

Serious complications from active compression-decompression cardiopulmonary resuscitation.

Complications arising from techniques of cardiopulmonary resuscitation (CPR) were reviewed by analysing the autopsy protocols of 25 patients who died after standard (Std) CPR and 31 who died after active compression-decompression (ACD) CPR, 15 of them preceded by Std CPR. The results can be summarised as follows: After Std CPR (n = 25) rib fractures were detected in 28%, sternal fractures in 16%, and no injuries in 68%. After ACD-CPR (n = 16) rib fractures occurred in 68%, sternal fractures in 68% and no injuries in 25%. After ACD-CPR following Std CPR(n = 15) rib fractures were detected in 93

Isolated sternal fracture: an audit of 10 years' experience.

We have reviewed 162 consecutive cases of sternal fracture admitted to the Leicester Royal Infirmary over a 10 year period. There were no incidences of cardiogenic shock or arrhythmia developing in patients who had sustained an isolated sternal fracture, irrespective of the aetiology. There were three deaths, three ITU admissions and one arrhythmia, all occurring in patients with severe thoracic injuries, or other associated injury. Our series confirms the observations of other authors, that patients with isolated sternal fractures, especially those sustained by car occupants wearing seatbelts

Sternitis and mediastinitis after coronary artery bypass grafting. Analysis of risk factors.

As part of a quality control program, we analyzed possible risk factors in the development of sternitis and mediastinitis after coronary artery bypass grafting. From 1 January 1990 through 31 December 1991, 1,368 consecutive coronary artery bypass grafting procedures were performed at our institution, either alone or in combination with other procedures. Twenty-three patients (1.7%) developed sternitis and/or mediastinitis; 7 (30.4%) of these patients died in an early postoperative phase. Univariate analysis revealed the following statistically significant (p < or = 0.05) risk factors: perfusi

Preoperative management of neonatal tetralogy of Fallot with absent pulmonary valve syndrome.

In tetralogy of Fallot with absent pulmonary valve syndrome, aneurysmal enlargement of the central pulmonary arteries can be associated with severe bronchial stenosis and respiratory distress in the neonatal period. The ventilatory status of these patients must be stabilized before diagnostic and therapeutic interventions can be performed. This report describes the experience with prone positioning and emergency sternotomy in the preoperative care of a neonate.

[Osteoarticular infections associated with catheterization of the subclavian vein].

Access to a central way through catheterization of the subclavian vein is a widely used technique. Not uncommonly, the procedure is followed by infective complications among which clavicular osteomyelitis and septic sternoclavicular arthritis represent a rare eventuality. We report two cases of staphylococcic bacteremia produced after subclavian vein catheterization. Both patients presented septic sternoclavicular arthritis and osteomyelitis of the sternal manubrium. The isolated microorganisms were Staphylococcus aureus and Staphylococcus epidermidis methicillin-resistant, respectively. The c

Does sternal fracture increase the risk for aortic rupture?

We retrospectively reviewed the records of 99 patients who suffered sternal fractures between 1968 and 1987. Patients ranged in age from 5 to 86 years. The most common cause of injury was a motor vehicle accident. The 99 patients were compared with a concurrent series of 2,106 patients with chest injuries and no sternal fractures. Traumatic aortic rupture occurred in 2 of 99 patients with sternal fractures (2%) and in 75 of 2,106 patients without sternal fracture (3.6%). This difference was not statistically significant by the Fisher exact test (p = 0.326). We conclude that traumatic aortic ru

Current status of vascular grafting in supraaortic trunks. Personal experience.

Two-hundred forty-nine patients underwent prosthesis implantation for supraaortic trunk disease. The most frequently used technique for lesions of the innominate artery was direct repair via sternotomy. Mortality was very low (2.8%) and at 97.3% late patency was excellent (mean follow-up: six years). Extra-anatomical bypasses were reserved for all other lesions of the subclavian and common carotid arteries or contraindications to the direct approach. Patency for carotido-subclavian bypass (81 patients) and subclavio-subclavian bypass (134 patients) was 71% (mean follow-up: 4.6 years) with one

High-energy irradiation in the management of chondrosarcoma.

We present a retrospective analysis of seven patients with chondrosarcoma of the bone treated by high-energy irradiation between 1961 and 1976. Its major role in this series was prevention of local recurrence in cases with inadequate resection. In three of the five cases in which radiation therapy was adjuvant rather than primary treatment, long-term local control was obtained in a dose of 5,000 to 6,500 rads in five to six weeks. Although primary treatment of chondrosarcoma is surgical, high-dose radiation therapy is indicated when surgical resection is not possible. Chondrosarcoma can respon
