Video-assisted thoracoscopic surgery for lung cancer after induction therapy.

Background Although thoracoscopic surgery is widely performed for early-stage lung cancer, only a few small studies have evaluated the role of video-assisted thoracoscopic surgery in patients with locally advanced lung cancer who had received preoperative chemotherapy. Methods Among 1655 patients who underwent anatomical lung resection for lung cancer between January 2009 and December 2014 in our institution, we retrospectively examined the short- and long-term outcomes of 110 (6.6%) who had undergone induction therapy. Thoracoscopic surgery was performed in 79 of these patients and thoracotom

Narrative review of descending necrotizing mediastinitis, focusing on the etiology, classification-guided surgical management, treatments, and prognosis.

BACKGROUND AND OBJECTIVE: Descending necrotizing mediastinitis (DNM) is a rare, life-threatening infection characterized by the rapid spread of deep neck infection (DNI) along the continuous fascial planes into the mediastinum. Although diagnostic and therapeutic advances have improved patient outcomes, the prognosis remains unsatisfactory in advanced cases or in the absence of timely and appropriate treatment, mainly due to delayed diagnoses and difficulty in selecting an appropriate surgical approach. This review summarizes the current knowledge on the pathophysiology, anatomical classificat

Bronchoscopy increased the risk of postoperative pulmonary complications in patients undergoing lobectomy for lung cancer: a retrospective cohort study.

BACKGROUND: Postoperative pulmonary complications (PPCs) are a leading cause of morbidity and mortality, particularly following thoracic surgeries like lobectomy for lung cancer. These complications not only increase patient suffering but also lead to longer hospital stays and higher healthcare costs. Bronchoscopy, a procedure that enables airway visualization, serves both diagnostic and therapeutic functions. In postoperative care, bronchoscopy helps clear secretions, identify and resolve airway issues, and assess lung re-expansion. This study aimed to investigate whether patients admitted to

Thoracoscopic Resection of a Giant Solitary Fibrous Tumor in the Chest Cavity After Preoperative Arterial Embolization: A Case Report.

Giant hypervascular solitary fibrous tumors of the pleura (SFTPs) present significant challenges due to their size and vascularity and potential involvement of adjacent structures. Careful preoperative planning of the surgical approach is required, depending on the tumor size and location. Furthermore, intraoperative bleeding may significantly complicate surgical procedures in patients with hypervascular tumors. Herein, we report a case of a giant hypervascular SFTP with a pulmonary artery shunt that was successfully managed by thoracoscopic excision after preoperative embolization of the vasc

South African Thoracic Society consensus statement on transbronchial lung cryobiopsy for interstitial lung disease.

BACKGROUND: Surgical lung biopsy (SLB), performed via open lung biopsy or video-assisted thoracoscopic surgery, has traditionally been the gold standard for diagnosing interstitial lung disease (ILD) when histological confirmation is necessary. Transbronchial forceps biopsy, while less invasive, often yields small, artifact-prone specimens that are insufficient for conclusive histopathological analysis. Transbronchial lung cryobiopsy (TBLC) has emerged as a minimally invasive alternative, offering a higher diagnostic yield and superior tissue integrity due to the retrieval of larger, en bloc s

Comparison of outcomes of surgical and other invasive treatment modalities for malignant pleural effusion in patients with pleural carcinomatosis.

BACKGROUND: Treatment modalities for malignant pleural effusion (MPE) are diverse. The objectives were to analyze actual clinical data from patients with MPE and pleural carcinomatosis and to compare the outcomes of different treatment modalities with regard to effectiveness, survival, morbidity, and mortality as well as the duration of hospitalization. METHODS: Patients with pathologically proven pleural carcinomatosis or MPE from 2018 to 2020 were included in this retrospective-observational study with additional questionnaires. We identified four treatment modalities: (I) video-assisted th

Distribution of transpulmonary pressure during one-lung ventilation in pigs at different body positions.

Background. Global and regional transpulmonary pressure (PL) during one-lung ventilation (OLV) is poorly characterized. We hypothesized that global and regional PL and driving PL (ΔPL) increase during protective low tidal volume OLV compared to two-lung ventilation (TLV), and vary with body position. Methods. In sixteen anesthetized juvenile pigs, intra-pleural pressure sensors were placed in ventral, dorsal, and caudal zones of the left hemithorax by video-assisted thoracoscopy. A right thoracotomy was performed and lipopolysaccharide administered intravenously to mimic the inflammatory respo

Factors Affecting Postoperative Lung Expansion in Patients with Pyogenic Empyema.

BACKGROUND: In patients with parapneumonic empyema, decortication is usually preferred to ensure functional lung re-expansion. However, there could be patients exhibiting incomplete postoperative lung expansion and inadequate drainage despite decortication. Therefore, we evaluated factors affecting postoperative lung expansion in patients undergoing decortication. METHODS: A total of 221 patients with pyogenic empyema who underwent video-assisted thoracoscopic surgery (VATS) between January and October 2016 in our hospital were reviewed in terms of surgical success. The following factors were

Pleuropulmonary complications of rheumatoid arthritis: a thoracic surgeon's challenge.

Approximately 1% of patients with rheumatoid arthritis have chronic lung disease develop, which can lead to complications, including pneumothorax and bronchopleural fistula. Given the inflammatory changes found, along with the immunosuppressant regimen used in management, these complications are often recalcitrant to initial surgical maneuvers. Our goal in reviewing these patients is to demonstrate the escalation of therapeutic interventions that may be needed to ensure successful resolution of this challenging disease process.

Transthoracic repair of Morgagni's hernia: a 20-year experience from open to video-assisted approach.

BACKGROUND: Foramen of Morgagni's hernia is an uncommon congenital diaphragmatic hernia. Repair is mostly performed through laparotomy. We prefer the transthoracic approach, which allows better and safer control during thoracic dissection, although it is considered more painful and related to greater morbidity. In recent years we introduced the transxiphoid hand-assisted videothoracoscopic approach, which combines the advantages of the thoracic route with a mini-invasive procedure facilitated by one hand inside the chest. METHODS: A retrospective review was performed over a 20-year period (19

The usefulness of three-dimensional video-assisted thoracoscopic esophagectomy in esophageal cancer patients.

BACKGROUND: The three-dimensional video-assisted (3D-VA) system is known to provide depth perception and the precise measurement of anatomical spaces, unlike the two-dimensional video-assisted (2D-VA) system. However, the advantages of the 3D-VA system in thoracoscopic esophagectomy remains unclear. METHODS: We retrospectively analyzed data from 104 patients who underwent thoracoscopic esophagectomy for esophageal cancer from 2016 to 2017. We performed thoracic esophagectomy using either the 2D-VA or 3D-VA system during this period. Whenever the 3D-VA system was available in our surgical cent
