BACKGROUND: Atrial septal defects (ASDs) are among the most common congenital heart defects, comprising about 10 % of cases in children. Transcatheter closure of ASDs has become a preferred treatment due to its minimally invasive nature, quicker recovery, and reduced risk compared to surgical closure. Despite its advantages, the procedure carries a risk of post-procedural arrhythmias, particularly bradyarrhythmias and tachyarrhythmias, occurring in approximately 7-10 % of patients. The development of these arrhythmias can be attributed to mechanical interference with conduction tissue or the f

INTRODUCTION: Pregnancy after bariatric surgery is a high-risk pregnancy. Post-bariatric complications may require emergency surgical intervention. During laparoscopic surgery, anesthetic drugs and formation of pneumoperitoneum influence maternal hemodynamics and may impair the perfusion of the utero-placental unit. Subsequent changes of the fetal heart rate (FHR) might be recognized with intra-operative fetal monitoring and guide maternal positioning and hemodynamic management to maintain fetal wellbeing. However, current fetal monitoring guidelines do not include FHR interpretation during ge

Arthroscopic surgery, especially of the knee, is a minimally invasive procedure with low rates of complications. Rarely, this procedure can be associated with traumatic arteriovenous fistula (AVF) formation secondary to popliteal arterial injury. Here, we present the case report of a 33-year-old woman with symptomatic iatrogenic popliteal arteriovenous fistula formation following right knee arthroscopy. The patient has a 10-year history of left knee pain secondary to an unspecified knee injury. Magnetic resonance imaging (MRI) revealed loose bodies within the knee joint and a medial meniscal t

Fibrodysplasia ossificans progressiva (FOP) is a rare disease in which heterotopic ossification (HO) is formed in muscles, tendons and ligaments. Traumatic events, including surgery, are discouraged as this is known to trigger a flare-up with risk of subsequent HO. Anesthetic management for patients with FOP is challenging. Cervical spine fusion, ankylosis of the temporomandibular joints, thoracic insufficiency syndrome, restrictive chest wall disease, and sensitivity to oral trauma complicate airway management and anesthesia and pose life-threatening risks. We report a patient with FOP suffer

OBJECTIVE: To assess the training of medical students and their confidence in urethral catheter placement, given growing evidence of unnecessary urology consults and iatrogenic injury.
SUBJECTS AND METHODS: A third-year medical school class was queried about their attitudes and knowledge of catheter placement before and after the Clinical Biennium. The Clinical Biennium introduces hands-on skills prior to clinical clerkships. Urethral catheterisation is one of the skill stations that students rotate through, and urology residents provide a didactic session and supervised simulation. Confidenc

BACKGROUND: Fine needle aspiration (FNA) biopsy has been rarely used in oral and oropharyngeal lesions. The goal of this study was to assess the value and accuracy of FNA biopsy in the diagnosis of oral and oropharyngeal lesions particularly in regards to discriminating benign from malignant tumors.
METHODS: Sixteen cases of FNA biopsies obtained of various intraoral and oropharyngeal masses or lesions performed at our institution during the eight-year period from 1998 to 2006 were retrospectively reviewed. The aspiration cytologic diagnoses were correlated with the histologic examination of

Background Addressing the opioid epidemic in the United States requires minimizing the overprescription of opioid pain medications, which poses a challenge for urologic surgeons, as postoperative pain is common after procedures such as inflatable penile prosthesis (IPP) placement. This research aimed to evaluate the impact of an ultra-restrictive opioid prescription protocol (UROPP) on pain management and opioid usage following IPP implant surgery. Methodology Patients undergoing IPP surgery were treated perioperatively using a UROPP with 500 mg acetaminophen and 15 mg intravenous ketorolac ev

BACKGROUND: Pediatric choledocholithiasis is traditionally managed with an endoscopy-first approach, where Endoscopic Retrograde Cholangiopancreatography (ERCP) is performed before laparoscopic cholecystectomy (LC). However, ERCP carries risks such as pancreatitis, bleeding, infection, and perforation, and may even be an unnecessary procedure should stones pass spontaneously. An alternative surgery-first approach, utilizing intraoperative cholangiogram (IOC), laparoscopic common bile duct exploration (LCBDE), power flushing, and glucagon, may offer a more efficient treatment pathway. COVID-19 diagnosis relies on molecular testing for SARS-CoV-2 via nasopharyngeal swab in the presence of suggestive clinical, radiological and laboratory findings. Since bronchoalveolar lavage liquid (BAL) collected during fibrobronchoscopy may increase test sensitivity compared to nasopharyngeal swabs, it was performed during the 2020 pandemic in clinically or radiologically suspected cases. Our aim was to determine whether clinical features, chest computed tomography (CT) findings or laboratory tests may predict patients testing positive for SARS-CoV-2 at BAL after a negative nasopharyngeal

BACKGROUND: Patient safety is fundamental in high quality healthcare systems but despite an excellent record of perinatal care in Sweden some children still suffer from substandard care and unnecessary birth injuries. Sustainable patient safety improvements assume changes in key actors' mental models, norms and culture as well as in the tools, design and organisation of work. Interventions positively affecting team mental models on safety issues are a first step to enhancing change. Our purpose was to study a national intervention programme for the prevention of birth injuries with the aim to

The establishment of a reliable large animal model of acute hepatic failure (AHF) is critical for the evaluation of supportive therapies such as bioartificial liver support systems (BALSS). Large animal surgical models bear some resemblance to the clinical syndrome of AHF in humans. However, these are inherently complex and are predisposed to inter-individual variation. The development of considerable skill is required and no precise standardization criteria for such models have been defined as yet. This study investigates a surgically induced ischemic model of AHF in 15 female Landrace pigs.

BACKGROUND: Axillary lymph node dissection (ALND) is an integral part in the therapy of breast cancer. Axillary lymph node involvement and tumour size are the most important prognostic factors. Restriction of ALND to level I and II (Berg) reduced high morbidity. The increasing proportion of patients with early breast cancer and negative axillary nodes led to a more selective method to avoid unnecessary ALND. The sentinel lymph node (SLN)--the first draining lymph node of a tumour--represents the status of the axilla. A negative SLN prevents from further completion ALND.

BACKGROUND: The introduction of SLNB has allowed accurate staging in early-stage breast carcinomas and has minimized the number of unnecessary ALNDs. Intraoperative frozen-section analysis is a fundamental component of the sentinel lymph node biopsy (SLNB) procedure. Some patients have positive nodes on frozen-section analysis and thus undergo a conventional axillary lymph node dissection (ALND) at the time of the SLNB. A few patients have negative nodes on frozen-section analysis but have subsequent evidence of metastases on final pathologic examination. The purpose of our study was 2-fold: t

Many developing countries face serious obstacles that have hindered establishment of successful cervical cancer control programs. Various countries are now seeking to strengthen cytology services and identify simple low-cost screening strategies; but any real gains in reducing cervical cancer incidence and mortality will also require effective treatment of women with preinvasive disease. Despite a trend toward conservative outpatient approaches for treating cervical dysplasia in industrialized countries, clinicians in many developing countries still rely primarily on invasive inpatient methods
