Giant supratentorial enterogenous cyst: report of a case, literature review, and discussion of pathogenesis.

OBJECTIVE AND IMPORTANCE: To describe a histologically well-documented adult case of a giant supratentorial enterogenous cyst (EC). Fewer than 15 cases of supratentorial ECs are on record: 8 associated with the brain hemispheres or the overlying meninges, 4 with the sellar region, and 2 with the optic nerve. CLINICAL PRESENTATION: A 31-year-old woman complained of long-standing mild left brachial and crural motor deficit precipitated by headache and signs of intracranial hypertension. Magnetic resonance imaging revealed a huge cyst overlying the frontoparietal brain. INTERVENTION: Symptoms w

Prognostic Factors for Neurological Outcome After Surgery for Idiopathic Spinal Cord Herniation: A Systematic Review and Published Individual Patient Data Analysis.

OBJECTIVE: Idiopathic spinal cord herniation (ISCH) is a rare cause of progressive thoracic myelopathy, for which surgical management is widely accepted for symptomatic patients, but prognostic determinants remain poorly defined. This study aimed to synthesize the available surgical literature on ISCH, through a systematic review of published individual patient data (IPD), and to identify clinical, anatomical, and intraoperative factors associated with postoperative motor and sensory outcomes. METHODS: A systematic review of the literature was performed in accordance with PRISMA guidelines an

Treatment of extramedullary tumors: morbidity and long-term results.

OBJECTIVE: Surgery on extramedullary tumors has a place among the most gratifying operations in neurosurgery. However, little data exist on permanent morbidity and long-term results. This paper provides these data and analyzes which factors influence them and how they can be managed. METHODS: Among 2081 patients with tumors of the spinal canal presenting between 1991 and 2024, 605 patients were identified with intradural extramedullary tumors, of whom 500 patients underwent 570 operations. Multiple regression was used to identify factors influencing resection and morbidity rates. Short-term r

Pituitary adenomas with subarachnoid invasion: clinical characteristics, radiographic features, surgical management, and outcomes.

OBJECTIVE: A subset of pituitary adenomas invade the subarachnoid space, encase critical neurovascular structures, and portend significant surgical morbidity. Resection of these lesions requires a modified surgical approach involving an expanded bony exposure and meticulous microsurgical dissection to avoid injury to the involved neurovascular structures. The goal of this study was to elucidate the clinical and radiographic features of subarachnoid adenomas and to characterize the outcomes after resection of these challenging tumors. METHODS: Data were collected from 436 consecutive pituitary

Double dural puncture epidural improves quality of labour analgesia without increasing adverse effects in primipara: A randomised controlled trial.

BACKGROUND: Neuraxial anaesthesia is considered a routine method of reducing labour pain. Over the past few decades, several improvements in neuraxial labour analgesia have been studied and evaluated. OBJECTIVE: This trial compared the analgesic effects and adverse effects between double dural puncture epidural (DDPE) and epidural for labour analgesia, and analysed the feasibility and safety of using DDPE for labour analgesia. DESIGN: A single-centre, prospective, single-blind, randomised controlled trial. SETTING: Huai'an First People's Hospital, affiliated to Nanjing Medical University, f

Tranexamic acid vs. embolization of the meningeal artery as an adjunctive therapeutic regime to reduce the recurrence rate after surgical relief of chronic subdural hematomas (TABASCO)-a randomized controlled trial.

BACKGROUND: Chronic subdural hematoma is a persistent, bloody to serous fluid retention in the subdural space between the dura mater and arachnoid mater, usually caused by an initial trauma, and is one of the most common traumatic intracranial hemorrhages in western industrialized nations. In the event of a compressive effect on the brain, the hematoma is usually relieved by means of a burr hole trephination. In view of the high postoperative recurrence rate, conservative treatment methods have been investigated both as a competitor to surgery alone in cases where the indication for surgery is

The Spatial Relationship between Spinal Osteoarthritis and CSF Venous Fistulas in Patients with Spontaneous Intracranial Hypotension.

BACKGROUND AND PURPOSE: CSF venous fistula leads to spontaneous intracranial hypotension. The exact mechanisms underlying the development of CSF venous fistula remain unclear: Some researchers have postulated that underlying chronic intracranial hypertension may lead to damage to spinal arachnoid granulations, given that many patients with CSF venous fistulas have an elevated body mass index (BMI). However, individuals with higher BMIs are also more prone to spinal degenerative disease, and individuals with CSF venous fistulas also tend to be older. CSF venous fistula tends to occur in the low

Assessment of Reversibility of Transverse Venous Sinus Stenosis in Patients With Papilledema.

BACKGROUND: Bilateral transverse venous sinus stenosis (TVSS) or stenosis of a dominant venous sinus has been found to be very sensitive radiological findings in patients with intracranial hypertension (IH), yet there is still an ongoing debate whether they constitute reversible or permanent phenomena. Thus, the purpose of this study was to investigate the reversibility of TVSS in patients with IH, including conservatively treated patients with signs of IH as defined by the presence of papilledema. METHODS: This was a retrospective chart review of all patients diagnosed with IH between 2016 a

[Microanatomical Investigation of the Subtemporal Transtentorial Approach].

OBJECTIVE: To study the microanatomic structure of the subtemporal transtentorial approach to the lateral side of the brainstem, and to provide anatomical information that will assist clinicians to perform surgeries on the lateral, circumferential, and petroclival regions of the brainstem. METHODS: Anatomical investigations were conducted on 8 cadaveric head specimens (16 sides) using the infratemporal transtentorial approach. The heads were tilted to one side, with the zygomatic arch at its highest point. Then, a horseshoe incision was made above the auricle. The incision extended from the m

Fourth ventricular subarachnoid stent for Chiari malformation type I-associated persistent syringomyelia.

OBJECTIVE: Syringomyelia (syrinx) associated with Chiari malformation type I (CM-I) is commonly managed with posterior fossa decompression, which can lead to resolution in most cases. A persistent syrinx postdecompression is therefore uncommon and challenging to address. In the setting of radiographically adequate decompression with persistent syrinx, the authors prefer placing fourth ventricular subarachnoid stents that span the craniocervical junction particularly when intraoperative observation reveals arachnoid plane scarring. The objective of this study was to evaluate the safety and effi

Early life adversity drives sex-specific anhedonia and meningeal immune gene expression through mast cell activation.

Exposure to early life adversity (ELA) in the form of physical and/or psychological abuse or neglect increases the risk of developing psychiatric and inflammatory disorders later in life. It has been hypothesized that exposure to ELA results in persistent, low grade inflammation that leads to increased disease susceptibility by amplifying the crosstalk between stress-processing brain networks and the immune system, but the mechanisms remain largely unexplored. The meninges, a layer of three overlapping membranes that surround the central nervous system (CNS)- dura mater, arachnoid, and piamate

Pathological and Incidental Findings in 403 Taiwanese Girls With Central Precocious Puberty at Initial Diagnosis.

Background: Whether girls with central precocious puberty (CPP) should undergo routine brain magnetic resonance imaging (MRI) to identify potential intracranial pathologies is controversial. Aims: To evaluate the brain MRI results of girls with CPP and identify the clinical and endocrine predictors of brain abnormalities. Methods: This retrospective study obtained data from pediatric endocrine clinics at Chang Gung Children's Hospital. From 1997 and 2017, 403 girls were consecutively diagnosed with CPP. After the exclusion of patients with a history of central nervous system (CNS) insults or a

Neurocutaneous melanocytosis (melanosis).

Neurocutaneous melanosis (NCM; MIM # 249400; ORPHA: 2481], first reported by the Bohemian pathologist Rokitansky in 1861, and now more precisely defined as neurocutaneous melanocytosis, is a rare, congenital syndrome characterised by the association of (1) congenital melanocytic nevi (CMN) of the skin with overlying hypertrichosis, presenting as (a) large (LCMN) or giant and/or multiple (MCMN) melanocytic lesions (or both; sometimes associated with smaller "satellite" nevi) or (b) as proliferative melanocytic nodules; and (2) melanocytosis (with infiltration) of the brain parenchyma and/or lep

Starling resistors, autoregulation of cerebral perfusion and the pathogenesis of idiopathic intracranial hypertension.

Two critical functions for the control of intracranial fluids dynamics are carried on the venous side of the perfusion circuit: the first is the avoidance of cortical veins collapse during the physiological increases of cerebrospinal fluid (CSF) pressure in which they are immersed. The second, is the generation of an abrupt venous pressure drop at the confluence of the cortical veins with the dural sinuses that is required to allow a CSF outflow rate balanced with its production. There is evidence that both of these effects are ensured by a Starling resistor mechanism (a fluid dynamic construc
