Comparison of simultaneous bilateral CI surgery vs. sequential CI surgery regarding operative time, perioperative morbidity, and anesthesia risk in children.

OBJECTIVES: In recent years, simultaneous bilateral cochlear implantation (CI) has become the preferred procedure for children with bilateral deafness. The aim of this study is to compare simultaneous bilateral implantation with sequential bilateral CI concerning duration of surgery and anesthesia, perioperative morbidity, mortality, and anesthesia risk. METHODS: A retrospective data analysis was conducted on 132 children who were implanted between 2019 and 2024 and aged under 10 years at the time of either simultaneous or second CI. The age at implantation, duration of surgery and anesthesia…

Spiral ganglion neuron degeneration in aminoglycoside-deafened rats involves innate and adaptive immune responses not requiring complement.

Spiral ganglion neurons (SGNs) transmit auditory information from cochlear hair cells to the brain. SGNs are thus not only important for normal hearing, but also for effective functioning of cochlear implants, which stimulate SGNs when hair cells are missing. SGNs slowly degenerate following aminoglycoside-induced hair cell loss, a process thought to involve an immune response. However, the specific immune response pathways involved remain unknown. We used RNAseq to gain a deeper understanding immune-related and other transcriptomic changes that occur in the rat spiral ganglion after kanamycin

Istradefylline Mitigates Age-Related Hearing Loss in C57BL/6J Mice.

Age-related hearing loss (ARHL) is the most common sensory disorder among older people, and yet, the treatment options are limited to medical devices such as hearing aids and cochlear implants. The high prevalence of ARHL mandates the development of treatment strategies that can prevent or rescue age-related cochlear degeneration. In this study, we investigated a novel pharmacological strategy based on inhibition of the adenosine A2A receptor (A2AR) in middle aged C57BL/6 mice prone to early onset ARHL. C57BL/6J mice were treated with weekly istradefylline (A2AR antagonist; 1 mg/kg) injections

Deep Volumetric Segmentation of Murine Cochlear Compartments from Micro-Computed Tomography Images.

Local drug delivery to the inner ear via micropump implants has the potential to be much more effective than oral drug delivery for treating patients with sensorineural hearing loss and to protect hearing from ototoxic insult due to noise exposure or cancer treatments. Designing micropumps to deliver appropriate concentrations of drugs to the necessary cochlear compartments is of paramount importance; however, directly measuring local drug concentrations over time throughout the cochlea is not possible. Recent approaches for indirectly quantifying local drug concentrations in animal models cap

Pejvakin, a Candidate Stereociliary Rootlet Protein, Regulates Hair Cell Function in a Cell-Autonomous Manner.

Mutations in the Pejvakin (PJVK) gene are thought to cause auditory neuropathy and hearing loss of cochlear origin by affecting noise-induced peroxisome proliferation in auditory hair cells and neurons. Here we demonstrate that loss of pejvakin in hair cells, but not in neurons, causes profound hearing loss and outer hair cell degeneration in mice. Pejvakin binds to and colocalizes with the rootlet component TRIOBP at the base of stereocilia in injectoporated hair cells, a pattern that is disrupted by deafness-associated PJVK mutations. Hair cells of pejvakin-deficient mice develop normal root

Diagnosis and Management of Congenital Sensorineural Hearing Loss.

Hearing loss is the most common sensory disorder in the USA. The diagnosis of congenital hearing loss starts with newborn hearing screening, … gnostic tests. Management of congenital hearing loss involves early fitting of amplification. Early cochlear implantation, preferably before 2 years of age, should be strongly considered for children with bilateral severe hearing loss.

Therapeutic efficacy of intra-cochlear administration of methylprednisolone after acoustic trauma caused by gunshot noise in guinea pigs.

The therapeutic efficacy of cochlear infusion of methylprednisolone (MP) after an impulse noise trauma (170dB SPL peak) was evaluated in guinea pigs. The compound action potential threshold shifts were measured over a 14 days recovery period after the gunshot exposure. For each animal, one of the cochlea was perfused directly into the scala tympani with MP during 7 days via a mini-osmotic pump, whereas the other cochlea was not pump-implanted. The functional study of hearing was supplemented by histological analysis. Forty eight hours after the trauma, significant differences between auditory

On the design of a MEMS piezoelectric accelerometer coupled to the middle ear as an implantable sensor for hearing devices.

The presence of external elements is a major limitation of current hearing aids and cochlear implants, as they lead to discomfort and inconvenience. Totally implantable hearing devices have been proposed as a solution to mitigate these constraints, which has led to challenges in designing implantable sensors. This work presents a feasibility analysis of a MEMS piezoelectric accelerometer coupled to the ossicular chain as an alternative sensor. The main requirements of the sensor include small size, low internal noise, low power consumption, and large bandwidth. Different designs of MEMS piezoe

Current Device-based Clinical Treatments for Tinnitus.

Device-based clinical treatments for tinnitus are predominantly sound based and include ear-level sound generators, hearing aids, cochlear implants, and tinnitus treatment devices. They are intended for patients with bothersome tinnitus. Bothersome tinnitus is characterized by problems with sleep, concentration, and mood. Most people with bothersome tinnitus have hearing loss and would benefit from amplification; however, not all patients are willing to use hearing aids. Tinnitus treatment devices are available to assist those who are not good candidates for amplification, and include devices

Controversies in the Evaluation and Management of Otosclerosis.

Controversies have been associated with the etiology, diagnosis, evaluation, and management of otosclerosis since Valsalva first described stapes fixation as a cause of hearing loss. Although the exact mechanism of the bone remodeling associated with otosclerosis remains uncertain, stapedotomy has been accepted as the surgical treatment of most patients with stapedial otosclerosis. There remains a disparity of opinion, however, regarding the role of preoperative imaging, surgical technique, implant selection, and medical therapy for cochlear otosclerosis. In addition, opinions vary regarding t

Vascular mapping of the retroauricular skin - proposal for a posterior superior surgical incision for transcutaneous bone-conduction hearing implants.

BACKGROUND: Passive transcutaneous osseointegrated hearing implant systems have become increasingly popular more recently. The area over the implant is vulnerable due to vibration and pressure from the externally worn sound processor. Good perfusion and neural integrity has the potential to reduce complications. The authors' objective was to determine the ideal surgical exposure to maintain perfusion and neural integrity and decrease surgical time as a result of reduced bleeding. METHODS: The vascular anatomy of the temporal-parietal soft tissue was examined in a total of 50 subjects. Imaging

Enhancement of interaural level differences improves sound localization in bimodal hearing.

Users of a cochlear implant together with a contralateral hearing aid-so-called bimodal listeners-have difficulties with localizing sound sources. This is mainly due to the distortion of interaural time and level difference cues (ITD and ILD), and limited ITD sensitivity. An algorithm is presented that enhances ILD cues. Horizontal plane sound-source localization performance of six bimodal listeners was evaluated in (1) a real sound field with their clinical devices, (2) in a virtual sound field, under direct computer control, and (3) in a virtual sound field with ILD enhancement. The results

In-vitro and in-vivo measurement of the animal's middle ear acoustical response by partially implantable fiber-optic sensing system.

The main obstacle in realization of a totally implantable hearing aid is a lack of reliable implantable microphone. In this paper we have described a potentially miniature fiber-optic vibrometer based on a modified Michelson interferometer, designed to serve as a middle-ear microphone for totally implantable cochlear- or middle-ear hearing aids. A model of the sensing system was used for in-vitro and in-vivo investigation of acoustical response of sheep's middle-ear ossicles. Surgical and implantation procedure of introducing the sensing optical fiber into the middle-ear and its aiming at the

Effects of electrical brainstem stimulation on tinnitus.

The primary aim of the auditory brainstem implant (ABI), an investigational device, is to restore auditory sensations in neurofibromatosis-2 patients deafened by bilateral acoustic neuroma surgery. An electrode is placed on the dorsal cochlear nucleus after the tumor is removed through the translabyrinthine approach. This procedure was performed on 18 patients: 13 were completely deafferentated after bilateral tumor removal and had received the ABI during surgery on the second-sided tumor; 5 were implanted at first-sided tumor removal. Three of these 5 patients had usable hearing remaining on

"Cool" and "Hot" executive functions in deaf children: the executive brain battery (EBB).

Although previous studies have suggested that language deprivation may affect the development of executive functions (EFs), there are no assessment tools adapted to the language needs of deaf-and-hard-of-hearing (DHH) children. The present study had two objectives: (1) to examine the feasibility of the Executive Brain Battery in assessing 40 prelingually deaf-and-hard-of-hearing children between 6 and 12 years, and (2) to explore whether some sociodemographic and clinical variables could be associated with the performance of deaf children. The results showed that all tasks included in the Exec
