Case report of 18F-FDG PET/CT features of hypoglycemic encephalopathy.

RATIONALE: Hypoglycemia may cause diverse neurological manifestations, ranging from focal neurological deficits to irreversible coma. Severe and persistent hypoglycemia can lead to hypoglycemic encephalopathy (HE). Imaging findings of HE at different stages of 18F-FDG positron emission tomography/computed tomography (PET/CT) have rarely been reported. Herein, we describe a case of HE occurring in the medial frontal cortex, cerebellar cortex, and dentate nucleus using 18F-FDG PET/CT images from different periods. 18F-FDG PET/CT has a high value in displaying the lesion range and indicating the

Anticonvulsant activity of Nymphaea lotus Linn. extract in mice: The role of GABAergic-glutamatergic neurotransmission and antioxidant defence mechanisms.

Epilepsy remains an unmet medical need affecting more than 50 million people worldwide with about 125,000 mortality annually and more prevalent in low- and middle-income countries. Nymphaea lotus (also known as water lilly) is an aquatic plant used traditionally to treat convulsive episodes in Southwestern Nigeria. This study was undertaken to evaluate anticonvulsant activity of aqueous Nymphaea lotus extract (ANL) and ethanol Nymphaea lotus extract (ENL) on chemical-induced seizures in mice as well as possible mechanisms of action. Vehicle (10 mL/kg, p.o.), ANL (50-200 mg/kg, p.o.), ENL (50-2

Electrographic seizures and brain hyperoxia may be key etiological factors for postconcussive deficits.

Repetitive mild traumatic brain injuries (RmTBIs) are increasingly recognized to have long-term neurological sequelae in a significant proportion of patients. Individuals that have had RmTBIs exhibit a variety of sensory, cognitive, or behavioral consequences that can negatively impact quality of life. Brain tissue oxygen levels ([Formula: see text]) are normally maintained through exquisite regulation of blood supply to stay within the normoxic zone (18-30 mmHg in the rat hippocampus). However, during neurological events in which brain tissue oxygen levels leave the normoxic zone, neuronal dy

Prefrontal networks dynamically related to recovery from major depressive disorder: a longitudinal pharmacological fMRI study.

Due to lacking predictors of depression recovery, successful treatment of major depressive disorder (MDD) is frequently only achieved after therapeutic optimization leading to a prolonged suffering of patients. This study aimed to determine neural prognostic predictors identifying non-remitters prior or early after treatment initiation. Moreover, it intended to detect time-sensitive neural mediators indicating depression recovery. This longitudinal, interventional, single-arm, open-label, phase IV, pharmacological functional magnetic resonance imaging (fMRI) study comprised four scans at impor

Prefrontal activation during a working memory task differs between patients with unipolar and bipolar depression: A preliminary exploratory study.

BACKGROUND: To identify bipolar disorder during the initial stages of a depressive episode has always been a great clinical challenge. Patterns of functional brain activity may underlie the differences in the neural mechanisms of bipolar depression (BD) and unipolar depression (UD). This study aimed to investigate the differences in neural activity between BD and UD patients during executive task. METHODS: We performed a 52-channel near-infrared spectroscopy (NIRS) scan in 39 patients with BD, 35 patients with UD, and 36 healthy controls (HCs). The relative concentration changes in oxygenated

From the psychosis prodrome to the first-episode of psychosis: No evidence of a cognitive decline.

Cognitive deficits have an important role in the neurodevelopment of schizophrenia and other psychotic disorders. However, there is a continuing debate as to whether cognitive impairments in the psychosis prodrome are stable predictors of eventual psychosis or undergo a decline due to the onset of psychosis. In the present study, to determine how cognition changes as illness emerges, we examined baseline neurocognitive performance in a large sample of helping-seeking youth ranging in clinical state from low-risk for psychosis through individuals at clinical high-risk (CHR) for illness to early

[Criminal responsibility in schizophrenia with special attention to the mental state during the criminal act--a study based upon the activation-recurrence phenomenon].

In this article, psychiatric arguments regarding criminal responsibility in schizophrenia are reviewed, and it is pointed out that the arguments have been influenced by the theory of process psychosis. Moreover, two cases judged on the basis of the activation-recurrence phenomenon are exemplified, and fundamental issues are discussed. Investigating the mental state just before the crime helps when deliberating over criminal responsibility. A study of the activation-recurrence phenomenon, conducted by Shinkai and Miyasaka, contributes toward clarifying the recurrence of the micropsychotic state

Association Between Cognitive Function and Health Care Costs 3 Months and 6 Months After Initiating Antidepressant Medication for Depressive Disorders.

BACKGROUND: Major depressive disorder is one of the most common and disabling mental health disorders and is associated with substantial costs in terms of direct health care utilization and workplace productivity. Cognitive dysfunction, which alone substantially increases health care costs, is commonly associated with major depressive disorder. However, the health care costs of cognitive dysfunction in the context of depressive disorder are unknown. Recovery from mood symptoms is not always associated with resolution of cognitive dysfunction. Thus, cognitive dysfunction may contribute to healt

Circulating immune cells phenotypes discriminate bipolar disorder from major depressive disorder during a depressive episode: A machine learning elastic-net approach.

MDD and BD are characterized by differences in peripheral immune markers, in frequencies of T helper, T cytotoxic, Natural Killer (NK) and T regulatory cells that may contribute to the development of mood disorders. However, few studies have used immune cells populations to discriminate MDD from BD with inconsistent findings. We applied an elastic-net machine learning pipeline predictive model using 5-fold nested cross-validation based on a wide panel of immune cell populations to discriminate between MDD and BD. The sample included 124 MDD and 89 BD hospitalised for depressive episode, and 40

Brain Ketone Bodies in Health, Evolution and Disease.

Ketone bodies (KBs) are the only energy substrates oxidized by the brain, whose concentration in the circulation can greatly increase when a physiological situation requires it. For example, when an adult human fasts for two days, circulating KBs rise twenty-fold from ~0.1 to ~2 mM. As a fuel, KBs provide the brain with acetyl-CoA that produces ATP or glutamate, notably in certain brain regions. Remarkably, KBs activate the expression of their own cerebral transporters and KB-utilizing enzymes so that circulating levels determine cerebral utilization of KBs. Throughout evolution, the energetic

The Physics, Information, and Computation of Perennial Learning: Kolmogorov Complexity, Information Distance, and Port-Hamiltonian Thermodynamics.

Real-world autonomous agents learn under nonstationarity, safety constraints, and finite energetic budgets. We develop a framework for perennial learning-agents that continuously refine their models while provably controlling the cost of forgetting-by unifying three classical pillars: Kolmogorov complexity, which equates scientific discovery with algorithmic compression; Landauer's principle, which assigns a minimal thermodynamic cost of kBTln2 per erased bit to every irreversible model update; and port-Hamiltonian (PH) dynamics, whose (J-R)∇H decomposition separates zero-cost reversible infer

Serotype-specific Immunity and Association With Pneumococcal Carriage Following 3+1, 3+0, and 2+1 Schedules of 10-valent Pneumococcal Conjugate Vaccine.

BACKGROUND: Correlates of protection for pneumococcal carriage, a precursor for invasive pneumococcal disease, are not well-defined. The relationship between memory B cells (Bmem) with carriage remains unclear. We assessed serotype-specific Bmem, IgG, and opsonophagocytic (OPA) responses following different infant schedules of 10-valent pneumococcal conjugate vaccine (PCV10, Synflorix) and their association with pneumococcal carriage. METHODS: We analyzed secondary outcome data from a phase II/III randomized controlled trial in Vietnam (trial registration NCT01953510). Infants received PCV10

Longitudinal Outcomes of Oral Glutamatergic Augmentation in Trauma-Related Dissociation: A Six-Month Case Study.

Trauma-related dissociation can present with derealisation, depersonalisation, abrupt lapses in awareness, memory gaps, and fugue-like states, and it may be difficult to treat with standard approaches alone. Glutamatergic interventions have been proposed as neuroplasticity-focused strategies because N-methyl-D-aspartate (NMDA) receptor antagonism and downstream alpha-amino-3-hydroxy-5-methyl-4-isoxazolepropionic acid (AMPA)-linked signalling may influence synaptic plasticity. A 25-year-old woman with documented complicated grief related to her paternal grandfather's sudden death, longstanding

A prospective, randomized, single-blinded study comparing the efficacy and safety of dexmedetomidine and propofol for sedation during endoscopic retrograde cholangiopancreatography.

BACKGROUND: Balanced propofol sedation is extensively used in endoscopic retrograde cholangiopancreatography (ERCP), but sedation-related adverse events (SRAEs) are common. In various clinical settings, the combination of dexmedetomidine with opioids and benzodiazepines has provided effective sedation with increased safety. The aim of this investigation was to compare the efficacy and safety of dexmedetomidine and propofol for sedation during ERCP. METHODS: Forty-one patients were randomly divided into two groups: the dexmedetomidine (DEX) group and the propofol (PRO) group. Patients in the D

Pre-treatment predictors of cognitive side-effects after treatment with electroconvulsive therapy in patients with depression: A multicenter study.

BACKGROUND: Electroconvulsive therapy (ECT) is a highly effective treatment for major depressive episodes (MDE). However, ECT-induced cognitive side-effects remain a concern. Identification of pre-treatment predictors that contribute to these side-effects remain unclear. We examined cognitive performance and individual cognitive profiles over time (up to six months) following ECT and investigated possible pre-treatment clinical and demographic predictors of cognitive decline shortly after ECT. METHODS: 634 patients with MDE from five sites were included with recruitment periods between 2001 a
