Open Treatment of Radial Head Fractures.

Radial head fractures may commonly be treated by (1) open reduction and internal fixation (ORIF), (2) radial head excision, or (3) radial head replacement. If there is no associated elbow instability with lateral ulnar collateral ligament (LUCL) injury, the preferred approach is via a split in the extensor digitorum communis (EDC) origin. This provides a wide exposure but limits the risk of injury to the LUCL and associated instability. The radial head is fixed, excised, or replaced. Open treatment of radial head fractures begins with the following steps: (1) a laterally based incision is cent

Use of elastic stable intramedullary nailing for treating unstable forearm fractures in children.

BACKGROUND: The majority of the pediatric diaphyseal forearm fractures are treated with closed reduction and the application of an above elbow cast for a varying period, depending on the age of the child. Some forearm diaphyseal fractures require osteosynthesis mainly because of the presence of an unacceptable angular deformity, fracture instability, presence of an open injury or failure of conservative management in the form of redisplacement in the cast after manipulation. METHODS: We present our experience with elastic stable intramedullary nailing (ESIN) for displaced and unstable pediatr

Management of Head and Neck Injuries by the Sideline Physician.

Although rare, sport-related injuries to the head and neck can be life threatening; therefore, timely and appropriate treatment is critical. Preparation is key for the sideline physician and begins well before arriving on the sideline. Knowing the athletic trainer and support staff, establishing a chain of command and emergency action plan, and having all the appropriate equipment readily available are important for game or practice preparedness. At the athletic event, physicians should have a clear line of sight to the field of play and easy access to reach the field when necessary. When perf

Early Results of the A.L.P.S. Proximal Humerus Locking Plate.

BACKGROUND: Open reduction internal fixation of displaced 3 and 4-part proximal humerus fractures is an effective treatment modality particularly for the younger age group, but it is not without complications. Fixed angled locking screw plates are the mainstay of fixation. The A.L.P.S. Proximal Humerus Plating System is a new implant that has smooth locking pegs in the place of humeral head screws to help reduce glenoid damage in the event of cut-out and is designed to sit low on the humerus in order to reduce the risk of subacromial impingement. METHODS: Retrospective analysis of 15 consecut

Lower extremity soft tissue reconstruction and amputation rates in patients with open tibial fractures in Sweden during 1998-2010.

BACKGROUND: The rates of soft tissue reconstruction and amputation after open tibial fractures have not been studied on a national perspective. We aimed to determine the frequency of soft tissue coverage after open tibial fracture as well as primary and secondary amputation rates. METHODS: Data on all patients (> = 15 years) admitted to hospital with open tibial fractures were extracted from the Swedish National Patient Register (1998-2010). All surgical procedures, re-admissions, and mechanisms of injury were analysed accordingly. The risk of amputation was calculated using logistic regressi

Comparative Assessment of Functional Outcomes of Surgical Treatment in Patients with Distal Radial Fractures.

BACKGROUND: Unstable and comminuted distal radial fractures require surgical treatment by percutaneous insertion of Kir-schner wires, open reduction and fixation with a non-locking or locking plate or with an external device. The aim of this paper was to try to answer the following question: are there differences in functional treatment outcomes in patients after surgery with the use of Kirschner wires vs LCP plate fixation? MATERIAL AND METHODS: The study group included 100 patients after surgical treatment by closed reduction and simple fixation with Kirschner wires (50 patients) and by ope

Surgical Treatment of Periprosthetic Acetabular Fractures After Hip Arthroplasty: A Report of Two Cases.

Bipolar hemiarthroplasty (BHA) and total hip arthroplasty (THA) are standard treatments for hip disorders in the elderly. However, a rare postoperative complication known as periprosthetic acetabular fracture (PPAF) can occur, potentially affecting hip joint stability and implant longevity. The management of PPAF is guided by the Della Valle and Paprosky classifications, which assess implant stability and the extent of bone loss. In this study, we report two cases of PPAF occurring after BHA and THA. In Case 1, early open reduction and internal fixation were performed, successfully avoiding st

The use of the pediatric physeal slide-traction plate in the treatment of neer-horwitz grade IV proximal humeral fractures in children: A case report and literature review.

BACKGROUND: Proximal humeral fractures (PHFs) are rare in children. Currently, the recommended surgical methods for severely displaced PHFs are closed reduction and percutaneous fixation using K-wires or intramedullary nailing, which can't provide firm internal fixation, especially for older and high-weight children. This study aimed to introduce a novel surgical approach, pediatric physeal slide-traction plate fixation (PPSP), for Neer-Horwitz grade IV PHFs in children. CASE SUMMARY: A 9-year-old boy presented with left shoulder pain and swelling due to a car accident. Physical examination r

Aggressive Postoperative Rehabilitation With Platelet-Rich Plasma Therapy for a Lauge-Hansen Supination-External Rotation Type IV Ankle Fracture: A Case Report of Early Return to Sports.

Supination-external rotation (SER) type IV ankle fractures, as classified by Lauge-Hansen (L-H), are highly unstable due to frequent injuries of the deltoid and distal tibiofibular ligaments. These fractures typically require open reduction and internal fixation (ORIF), followed by prolonged immobilization and delayed weight bearing. Such protocols often result in extended recovery periods, delaying the return to competitive sports. In the clinical setting, there is a growing demand for approaches that enable athletes to resume sports quickly. Platelet-rich plasma (PRP) therapy has demonstrate

Outcomes Associated With Scapholunate Ligament Injury Following Intra-Articular Distal Radius Fractures.

PURPOSE: The purpose of this study was to evaluate a series of intra-articular distal radius fractures (DRFs) to determine whether patients without radiographic evidence of scapholunate (SL) ligament injury have a difference in outcomes in comparison with patients with radiographic evidence of SL ligament injury and no ligament repair or reconstruction. Our hypothesis is that there are no significant differences in outcomes between patients after treatment of their intra-articular DRF. METHODS: A retrospective analysis of patients from a single institution who sustained an intra-articular DRF

Recurrent Dislocation Following Reverse Total Shoulder Arthroplasty for Acute Proximal Humeral Fracture: Salvage With Large-Head Bipolar Hemiarthroplasty and Brachial Plexus Neuropraxia.

Reverse shoulder arthroplasty (RSA) is commonly performed for complex proximal humeral fractures in older adults when fixation or anatomic reconstruction is unlikely to provide reliable outcomes. Recurrent instability following RSA remains a devastating complication, particularly in medically frail patients with poor bone quality and compromised soft tissue balance. Salvage strategies after repeated failed revisions are limited and often associated with poor functional outcomes. Functional outcome of patients of tibial fracture treated with solid nail (SIGN nail) versus conventional hollow nail - A randomized trial.

BACKGROUND: Trauma related disabilities disproportionately affects low and middle income countries due to lack of resources, skills and optimal implants. Despite adequate animal studies, biomechanical studies, cohort studies and comparison studies we are not aware of any randomized trial to compare the functional outcome of SIGN (Surgical Implant Generation Network, US) solid nailing with a hollow nailing for tibial shaft fracture. METHODS: Sixty patients (≥16 years) of closed and Gustilo grade I traumatic fractures of the leg were randomized into SIGN solid nailing or hollow nailing group. C
